Summary Description: Simultaneous Loss of Low-Beam Headlights
Summary
On May 14, 2014, General Motors (GM) submitted a Part 573 defect information report initiating NHTSA Safety Recall 14V-251 to remedy the subject vehicles.GM will remedy a defect condition in the underhood bussed electrical center (UBEC) by modifying the vehicle's electrical harnessing.GM also announced a customer satisfaction bulletin to address this issue on model year (MY) 2008-2013 Chevrolet Corvettes.Both programs will be conducted free of charge.According to GM, when the engine heats up and cools down (from normal use), the plastic material in the UBEC housing expands at a different rate than an electrical conductor routed within it.The difference in expansion rates results in bending of the conductor, a control circuit for the low beam headlights.After multiple thermal cycles, often taking several years of use, the conductor fractures resulting in an open circuit.When this occurs the low-beam headlights will not illuminate.After the engine is turned off and the UBEC cools, the conductor may temporarily reconnect and restore low-beam function, although subsequent failures will occur.This condition does not affect high-beam headlights, marker lamps, turn signals or fog lamps.Simultaneous loss of low-beam headlights could reduce the driver's visibility, increasing the risk of a crash. The ODI report counts noted above, involving the loss of low beam headlights, represents those received at the time of GM's recall filing; however NHTSA has since received additional complaints.GM submitted similar complaints in response to the agency's information request.Among the complaints reported to NHTSA, five alleged minor crashes in which the vehicle ran off the road or into roadside objects due to the sudden loss of headlights.Two of these complaints alleged minor injuries and three alleged property damage.Several other reports to NHTSA alleged close calls due to the loss of lighting.GM states that it has received no reports of crashes, injuries or fatalities as a result of this condition. GM's recall action appears to adequately address the safety risks in the subject vehicles of concern to ODI.Accordingly, the investigation is closed.In addition to recalling the subject vehicles, GM is covering MY 2008-2013 Corvettes, which contain essentially the same UBEC, under a customer satisfaction campaign.ODI notes that these vehicles exhibit a lower likelihood of failure due to their comparatively shorter service life. Summary
The Office of Defects Investigation (ODI) has identified 5 reports of vehicle headliners catching fire at the front of the vehicle near the sun visors on Jeep Grand Cherokees.Customers reported a range of fire conditions ranging from minor overheating to an open flame at the headliner and/or sun visor material while driving the vehicle. In some reports the fire spread to the front seats and/or door panels of the vehicle.In one report, the sunroof was damaged causing the glass to shatter.The cause of these fires is an electrical short in the vanity lighting wiring circuit that is routed to either one of the sun visors.The sun visors are mounted to the roof of the vehicle through the headliner with three screws.The sun visor wiring may be penetrated, or pierced by one of these screws either during initial vehicle assembly or subsequent headliner area repairs.The piercing causes an electrical short that could result in a fire.There is no dedicated fuse for the affected circuit so the electrical short can continue until the short clears or the vehicle is keyed off.The Dodge Durango uses the same headliner assembly, and Chrysler indicates there are similar headliner fire incidents affecting this model. Therefore, these vehicles have been added to the scope of the investigation.Analyzing the data received from Chrysler along with ODI reports indicates there may be a total of 52 unique fire incidents (including 3 injury allegations) on Grand Cherokee and Durango vehicles which ODI will continue to evaluate during the investigation.Based on ODI's evaluation of currently available data, this investigation is being upgraded toEngineering Analysis EA14-001.The ODI reports cited above can be reviewed at http://www-odi.nhtsa.dot.gov/owners/SearchSafetyIssues under the following identification (ODI) numbers: 10536895, 10556110, 10522665, 10522159, 10515592

ODI initially received 21 reports of rear light assembly failure due to a melted electrical connector or housing.Since opening the initial , ODI has received a total of 114 complaints reflecting an ongoing and increasing trend.In each report, one or both rear light assemblies fail completely (i.e., all lights housed within the assembly extinguish, including the stop, tail, and turn signal functions) when the electrical ground connection is compromised as a result of high resistance and subsequent thermal over-heating.According to Mercedes-Benz, the over-heating of the ground wire is caused by corrosion between the connectors in the ground circuit to the tail light.ODI is aware of a total of 5 reports alleging fire including one injury allegation as a result of this issue.Analysis of subject vehicle complaint data indicates an increased likelihood of failure as the vehicles age.As a result of the evidence presented, this investigation has been upgraded to EA13-008.
