In a letter dated September 26, 2013, BMW Group submitted a safety recall notice to NHTSA identifying a defect condition in the vacuum pump oil supply that may result in loss of power assist braking in 76,191 model year (MY) 2012 through 2014 BMW 3-Series, 5-Series, X1 SAV, X3 SAV, and Z4 vehicles equipped with 4-cylinder engines (NHTSA Recall 13V-454).According to BMW, movement of the seal disk within the camshaft may result in blocking of the oil supply to the vacuum pump, resulting in improper pump operation and loss of power assist braking.On July 11, 2013, BMW initiated a change to the camshaft geometry that prevented the seal disk from blocking the oil port.BMW has not received any braking complaints related to loss of power assist after the design change was implemented in production.The owners of all affected vehicles will be contacted by mail and asked to take their vehicle to a BMW dealer who will install a locking ring into the camshaft to keep the seal disk in its proper location.BMW expects to begin dealer and owner notification in November 2013 and complete dealer and owner notification in December when the remedy is expected to be available.Based on the actions taken by BMW, this investigation is closed.

In an October 28, 2013 letter to NHTSA, Ford Motor Company (Ford) submitted a recall notice identifying a defect in the Powertrain Control Module (PCM) software in approximately 2,456 model year (MY) 2012 through 2014 Ford Focus Electric vehicles manufactured from September 15, 2011 through August 23, 2013 (Recall No. 13V-523).According to Ford, PCM software faults may cause a complete loss of powertrain torque.When this condition occurs, drivers receive a red triangle indicator and the message Stop Safely Now in the instrument cluster.In addition, vehicle brake and steering systems will continue to operate normally and the vehicle can be restarted after going through a shutdown process.Ford and its supplier, Magna Powertrain, upgraded the PCM software to address powertrain shutdown conditions and issued a technical service bulletin on September 23, 2013 instructing dealers to reprogram the PCM on all affected vehicles. The Office of Defects Investigation (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers. In total, there were 171 unique reports indicating loss of motive power while driving, and the predominant failure mode identified involved loss of powertrain torque.Ford began owner notification on November 5, 2012.Owners of the recalled vehicles are notified to take their vehicle to a Ford or Lincoln dealer who will reprogram the PCM to the latest calibration. Summary
On March 25, 2014, Nissan North America (Nissan) submitted a Defect Information Report to NHTSA describing a defect of inadequate hose clamping force that may result in the internal transmission oil cooler (ITOC) hose detaching from the cooler.According to Nissan, transmission fluid leakage from a detached ITOC hose results in a progressive series of symptoms beginning with malfunction indicator lamp (MIL) illumination, followed by noise and vibration and culminating in loss of power due to transmission failure.Many of the incident vehicle drivers interviewed by ODI indicated that the loss of power occurred soon after the first symptoms were observed.The recall covers approximately 3,065 Pathfinder vehicles built from the start of MY 2013 production on April 18, 2012, through October 3, 2012, when Nissan implemented several production improvements to address concerns with ITOC hose detachment.Infiniti JX vehicles investigated in PE13-029 are not included in the recall as they are equipped with a different CVT transmission with a lower operating pressure.Owners of all potentially affected vehicles will be notified within 60 days to take their vehicles to a Nissan dealer.The dealer will install an oil hose repair kit at no charge to owners.ODI's analysis of failure data for the 3,065 Pathfinder vehicles that are included in Nissan's recall identified 12 Vehicle Owner Questionnaires (VOQs), 134 Nissan complaints and field reports and 259 warranty claims.This equates to a complaint/field report rate of 47.6 incidents per thousand vehicles (IPTV) and a warranty claim rate of 8.5 percent.The failure rates for the 120,474 subject vehicles that are not included in the recall are much lower, with complaint rates of 0.4 and 0.8 IPTV and warranty claim rates of 0.1 and 0.2 percent for the remaining MY 2013 Pathfinder and JX vehicles, respectively.This investigation is closed.The VOQ numbers associated with this closing are: 10558389, 10556011, 10546402, 10545032, , 10531793, 10509314, 10507077, 10503884, 10500834, 10499362, 10497685, 10496606, 10494583, 10493759, 10493701, 10490959.

On August 26, 2013, the Office of Defects Investigation (ODI) opened this preliminary evaluation after evaluating a Campaign Service Bulletin (CSB) 12-250-002 (NHTSA ID No. 10051812) issued by Spartan Chassis, Inc.The CSB related to sway bar end link failures on certain motorhome chassis manufactured between April 2, 2009 and January 14, 2013.In response to ODI's information request letter, Spartan conducted a detailed analysis of its claim data related to sway bar end link failures on the subject chassis.Spartan determined that a certain population of model MM motorhome chassis equipped with its model 1253 front suspension were experiencing an increased rate of failure.The population identified incorporated a design change that occurred in late 2009.This change increased the rebound travel of the front suspension resulting in a binding condition that could cause the sway bar end links to fail.A broken sway bar end link can potentially cause poor vehicle handling that could increase the risk of a vehicle crash.Based on the results of their analysis, Spartan decided to issue a defect notice and conduct a safety recall to replace the existing sway bar end links with new units and an improved mounting.Based on Spartan's decision to conduct a safety recall (14V-020), ODI is closing this investigation.
