Summary Description: Loss of rear lighting assembly
Summary
The Office of Defects Investigation (ODI) opened an investigation based on consumer allegations of loss of rear lighting in model year (MY) 2008-2011 Mercedes Benz C-Class vehicles.Complainants alleged that one or both rear lighting assemblies fail to illuminate (i.e., all lights housed within the assembly extinguish, including the stop, tail, and turn signal functions) or illuminate dimly.In some reports, complainants also alleged that the failure was accompanied by a burning smell emanating from the trunk of the vehicle.When examined, these vehicles showed evidence of overheating at the lighting assembly harness connector, including damaged (blackened) wiring and plastic.The rear lighting failures typically involved one side of the vehicle however in some cases both assemblies failed (although probably not simultaneously).ODI identified 5 fire and one injury allegations during the course of this investigation.Two fires occurred while the vehicles were parked.One originated in the vicinity of the driver?s side light assembly where the fire and associated damage was confined to the trunk area, and the fire self extinguished due to oxygen starvation.As a result of the fire, the vehicle was repurchased by Mercedes
and salvaged.The second fire originated on the passenger's side and required fire department response to extinguish; the fire damaged the tail light and surrounding body panels as well as the trunk area and wiring.The vehicle was repaired and returned to service.Two firesoccurred while the vehicle was being driven.In the first instance, the driver detected an odor and pulled over to observe smoking and a small flame at the lighting assembly wiring in the trunk.The owner was able to extinguish the fire before any significant damage occurred.In the second instance, the driver smelled smoke, pulled over, and opened the trunk to find smoke and sparks emanating from the forward facing side of the tail light.The driver was able to extinguish the sparks and drive the vehicle to the dealer. A fifth fire was reported by a fleet manager who observed a vehicle with fire damage originating near one of the tail light assemblies.The injury incident occurred when a technician burned their hand while diagnosing a failed light.The technician touched the tail light wiring which was hot enough to burn him although the injury did not require medical attention.On April 10th, 2014, Mercedes
notified ODI that it would conduct a safety recall to remedy the rear light assemblies in approximately 252,867 MY 2008 - 2011 C-Class vehicles including the C300, C350 and C63 AMG
models.Each rear light assembly has separate bulbs that act as brake, tail lamp, backup and turn signal indicators.The bulbs are mounted in a bulb carrier

The Office of Defects Investigations (ODI) opened this Preliminary Evaluation based on its review of a service bulletin submitted by Utility Trailer Manufacturing Company (Utility).The Service Bulletin (SB-07-011), contained in Utility?s July 2013 579.5 monthly submission, instructed owners of subject trailers to conduct an inspection of the pintle hook loop.The inspection procedure involved striking the loop with a 10 pound sledge hammer and then inspecting it for cracks.Due to a material defect, Utility informed owners there was a potential for the loop (that secures the trailers safety chains) to crack and fail.If the loop fails, this redundant safety component becomes ineffective in retaining the trailer to the towing vehicle should the primary hitch fail or otherwise disconnect.A trailer that has separated from its towing vehicle presents a risk to motor vehicle safety.On July 30th 2014 Utility trailer submitted a voluntary safety recall notice (14V-463) for pintle hook safety chain loop failures.The recall involves certain model year 2012-2014 VS1RA, and 2013-2014 VS1DC, VS2DX, VS2RA, VS3DX, and VS3RA trailers manufactured from April 2011 to May 2013.Utility trailer dealers will inspect and replace the pintle hook safety chain loop as required.This Preliminary Evaluation is closed.

Elantra Touring vehicles built from December 15, 2009 through May 7, 2012 (see NHTSA recall 14V-310 for more information).These Elantra Touring vehicles have a common headliner bracket design while the earlier MY 2009-2010 Elantra Touring vehicles have a headliner bracket design that utilizes a shorter length bracket which, should it become separated from the headliner, is unlikely to be displaced into the path of the driver or any occupant.During this investigation, ODI identified a MY 2010 subject vehicle in a salvage yard with a deployed driver-side curtain air bag that showed the headliner bracket displaced downward into the driver's window area.Also, during a static side curtain air bag deployment test conducted by Hyundai
, the headliner bracket almost completely separated from the headliner.The headliner tested came out of a MY 2010 subject vehicle.Finally, ODI identified two warranty claims on the subject vehicles that indicate potential headliner bracket issues.Warranty records for these claims indicate vehicle owners complaining of noise or rattling coming from the headliner area and Hyundai

Summary
The subject vehicles (SV), model years 2012-2013 Tesla Model S, are emerging technology electric vehicles using a high voltage battery
(HVB) to provide propulsion energy. The HVB uses lithium-ion cells combined in 60 or 85 kWh capacities, and a control system that monitors the HVB and its liquid cooling system. The HVB is positioned across the width of the vehicle between the front and rear wheels and lies above a flat aluminum pan that forms the bottom of the SV's chassis. About two thirds of the SVs were manufactured with an air-assisted suspension system that actively controls ride height, including automatically lowering the vehicle at higher speeds.Two separate incidents in 2013 resulted in significant fires involving the SVs, one in Washington (Oct. 13) and one in Tennessee (Nov. 13). Both incidents involved active suspension equipped vehicles operating at highway speeds and reduced ride height running over debris in the roadway. In both incidents, the struck objects penetrated the aluminum pan at the forward area of the battery, damaging the lithium ion cells of the HVB. The SV's information display notified the driver of decreased battery performance and ultimately instructed the driver to stop the vehicle. The SVs were able to travel ~.8 and 1.8 miles after impact respectively. In both cases, smoke appeared shortly after the vehicle stopped and a fire developed in the HVB. Thermal runaway occurred in the HVB cells. The fires destroyed the vehicles but did not result in injuries.In the Tennessee incident, the object struck by the SV was determined to be a three-ball hitch that apparently fell from another vehicle. Tesla
performed a series of tests reconstructing this incident and determined that a similar shaped object contacting the forward edge of the HVB could be tripped and potentially penetrate the HVB case. As the object's opposite end digs into the pavement, vehicle momentum causes the object to impart upward force into the case, described by Tesla
as a piking effect. Tesla
's testing reproduced damage similar to that seen in the Tennessee incident, and also showed that a change in ride height strategy, which was implemented in Nov. 2013 via a telematic software update to prevent the SVs lowering at legal roadway speeds, mitigates the risk of battery compartment penetration when a three-ball hitch is struck.The object struck in the Washington incident was not identified. More severe damage to the incident vehicle and the unknown shape of the object raised concerns regarding the effectiveness of raising the ride height for objects other than a three-ball hitch. In a Mar. 10, 2014 meeting with ODI, Tesla
stated it would conduct a free-of-charge service campaign to modify the SVs by adding three new components to the vehicle's undercarriage to protect the HVB. A low-hanging transverse member and an additional underbody plate would be mounted forward of the HVB case and a third plate would overlap the leading edge of the case. Testing conducted by Tesla
demonstrated that these modifications improved protection from debris impacts.ODI was aware of two fires stemming from road debris impacts when the investigation began. According to Tesla
, the SV fleet had accumulated ~90M miles of service at the time it revised the ride height strategy. No further incidents have been identified, and Tesla
reports the Model S fleet has traveled an additional ~90M miles. ODI believes impacts with road debris are normal and foreseeable. In this case, Tesla
's revision of vehicle ride height and addition of increased underbody protection should reduce both the frequency of underbody strikes and the resultant fire risk. A def

The Office of Defects Investigation (ODI) is aware of 1 incident related to the alleged defect. By letter dated January 10, 2014, Almared, Inc. (doing business as A-1 Alternative Fuel Systems) has notified ODI that it has conducted a safety recall (NHTSA Recall No. 14V-040) to address a safety defect in 18 model year 2011-2012 Ford F-550 transit buses manufactured between May 2011 and April 2013. These vehicles have been altered by A-1 to contain one or more compressed natural gas (CNG) fuel containers. According to A-1, the brackets securing passenger side CNG fuel containers to the vehicle chassis may fracture, resulting in a CNG fuel container that is unsecured. A-1 has reportedly inspected all 18 affected vehicles and repaired all vehicles equipped with the defective mounting bracket by installing a new bracket of an improved design. The new bracket has added strength as a result of the following design revisions: the cantilevered mounting plate is reinforced with additional support plates, the cantilevered position is mounted higher on the bracket above the CNG fuel container center line position (thus reducing the moment arm), and it is constructed of heavier gauge steel plate that is also dimensionally wider, which provides for a greater bonding surface area with the vehicle chassis.Based on the recall action taken by Almared, Inc., this investigation is closed.
