Helmet City, Inc. (HCI) was an importer and worldwide distributor of motorcycle helmets, operated by President Al Sobel. In 2014 and 2015, OVSC conducted testing on various sizes of the HCI 100 and HCI 115 motorcycle helmets. All of the helmets tested failed some aspect of the compliance tests, including failures of impact attenuation tests. On September 23, 2015, NHTSA sent HCI Information Request letters containing reports showing testing failures for model 100 (sizes M and XL) and model 115 (size XL). On October 21, 2015, HCI's attorney indicated that HCI was looking into doing a Part 573 report for the subject helmets. After some additional correspondence between NHTSA and HCI - including NHTSA reminding HCI of the five-working day deadline to submit a Part 573 Safety Recall Report NHTSA received HCI's response to the information requests on November 12, 2015.Ultimately, on February 2, 2016, HCI submitted two Part 573 Safety Recall Reports to NHTSA for non-compliance in HCI helmet model 100 (sizes M and XL), Recall 16E-008, and model 115 (size XL), recall 16E-010. On April 13, 2016, NHTSA opened this AQ to investigate the timeliness of HCI's non-compliance determination and the scope of the 2016 recalls. The AQ was also opened to investigate and review HCI's compliance with reporting requirements under the National Traffic and Motor Vehicle Safety Act.Prior to NHTSA opening the AQ, Darsan Trading (established in February 2016) had purchased the inventory and trademark of HCI. It is NHTSA's understanding that the predecessor company originally responsible for the recall no longer legally exists. Darsan, the successor company, has thus far been forthcoming and cooperative with NHTSA, including taking over responsibility for and completing the quarterly reporting requirements for the aforementioned HCI recalls. The agency has concluded that continuing this investigation into HCI is not warranted and that this investigation should be closed. Closure of this AQ does not represent a determination by the agency as to HCI or Darsan's compliance with any applicable federal laws or regulations. The agency continues to monitor Darsan's handling of the recalls and its conformance with legal obligations including reporting requirements. The agency reserves the right to take further action if warranted by the circumstances.

Phaeton Motorhomes.During the investigation Tiffin
identified additional models that used the same subject windshield wiper components.The additional models include 2007-2011 MY Allegro, Allegro Bus, Phaeton, and Zephyr, 2007-2010 MY Allegro Bay and 2010-2011 MY Allegro Breeze motorhomes.As part of the investigation, fleet operators, professional drivers, law enforcement personal, engineers from the Vehicle Research Test Center and complainants were interviewed.ODI leveraged these interviews to demonstrate that the wiper failures represented a safety defect.Tiffin
concurred and during the PE process decided to conduct a voluntary safety recall (16V-532).Tiffin

On April 13, 2016, the Office of Defects investigation (ODI) opened this Preliminary Evaluation on 2015-2016 MY Freightliner Cascadia trucks, alleging windshield wiper motor failures. The investigation was opened based on EWR field reports describing windshield wipers that intermittently failed to operate, causing a loss of vision and increasing the risk of a crash. Several of the reports alleged multiple vehicles experiencing the failure, while multiple failures happen on the same vehicle. Many of the field reports indicated the windshield wiper problem was resolved once the wiper motor is replaced.Through communications with the manufacturer, ODI learned they had received 4 complaints, 12 field reports and 1,852 warranty reports involving the alleged defect on the subject vehicles. ODI was also made aware of a crash due to wiper failure during a rain storm. ODI interviewed both the owner and driver of the truck. The driver alleged he switched the wiper speed from intermediate to high and the wiper stopped working. During the loss of vision event, he lost control of his vehicle, hitting an embankment on the passenger side. The owner of the truck stated that after towing the vehicle back to their property, the function of the wipers was tested and they were found not to operate.The manufacturer informed ODI they had collected wiper motors from vehicles that allegedly had wipers fail and bench tested them. In each case they state the motor was found to operate normally. The manufacturer states they have not been able to duplicate the wiper failure problem as described in reports.This investigation has been upgraded to an Engineering Analysis (EA17-001) for ODI to gather additional information concerning wiper system performance. ODI will also study peer vehicles to understand how they perform relative to the subject vehicles.

Background In a Part 573 Safety recall report dated August 11, 2015, Marmon-Herrington notified NHTSA that it decided that a defect exists in certain drive axles, model numbers MT14, MT17, MT22, R17 and R22, manufactured July 28, 2014, to July 22, 2015.The affected axles are equipped with hex nuts on the flange yoke that may not have been properly tightened.If the hex nuts are not tightened to specification, the driveshaft may disconnect from the drive axle, resulting in a loss of propulsion and increasing the risk of a crash. indicated that the rearmost cup seal in the brake master cylinder may roll, resulting in leakage from the brake fluid
reservoir to the brake booster.According to Ford
's DIR, Depending on the amount of brake fluid
loss from the brake master cylinder into the brake booster, the driver will experience an audible chime, message center alert, red brake warning light in the instrument cluster and may begin to experience a change in brake pedal travel and feel.In the event that a loss of brake fluid
is substantial enough to reduce brake function to the front wheels, the driver may experience longer pedal travel, increased pedal effort, and extended stopping distance, increasing the risk of a crash.Full braking function would remain in the rear wheel brake circuit.Ford
's recall will replace the master cylinder and, if the master cylinder is leaking, also replace the brake booster.Seventy-three (73) of the complaints received by ODI contained vehicle identification number (VIN) information required to determine date of build and recall applicability.ODI's analysis found that 66 complaints to ODI and 278 complaints to Ford
involving vehicles built in the range covered by 16V-345, resulting in an overall complaint rate of 152.9 incidents per 100,000 vehicles.Twelve (12) crash incidents were reported, including 11 with VIN information.All 11 crash incidents with VIN information reported involve vehicles covered by Ford
's recall.ODI's analysis identified 7 ODI complaints and 36 Ford
complaints in the 194,139 subject vehicles built outside of the build range for 16V-345, resulting in a complaint rate of 22.1 incidents per 100,000 vehicles.Ford
indicated that warranty return analyses identified issues with minor internal leakage between circuits due to internal contamination caused by supplier assembly plant cleanliness concerns in these vehicles.Ford
assessed the internal leak condition as an early-life condition with minimal safety risk that does not result in loss of fluid or circuit failure.ODI will continue to monitor complaints in subject vehicles not included in 16V-345.No complaints have been identified in MY 2013 through 2014 Ford
