The Office of Defects Investigations opened this Preliminary Evaluation to investigate complaints alleging an inadvertent deployment of side air bags on model year 2012 Nissan Versa vehicles.The three complaints indicate the driver or passenger curtain and seat mounted air bags deployed when the front door on the same (affected) side was shut.According to Nissan
, the defect identified involved the dissimilarity of the metals used in the side air bag sensor electrical connectors, gold versus tin materials.This combination may lead to fretting and oxidation on the connector pins of the Satellite Sensor and the harness which connects it to the Airbag Control Unit (ACU).Fretting and oxidation increases the likelihood of a momentary loss of connectivity, and thus communication from the sensor to the ACU.When communication is lost between the side impact sensor and the ACU, the system is designed to enter a ?backup state? mode of operation.The backup state allows the curtain and seat-mounted side air bags to deploy should the communication be accidentally lost during the course of a crash event. In this mode, the ?safing path? is used in the ACU algorithm for a deployment decision.By design, the safing path has a lower threshold for deployment compared to the normal state which requires both safing and trigger path thresholds to be met.Consequently, if this momentary communication loss occurs when the door is closed forcefully, it may cause the curtain and seat-mounted side air bag, as well as the seat belt pretensioner to deploy inadvertently.On March, 6, 2017, Nissan

Summary
The Office of Defects Investigation opened this investigation on September 3, 2016 based on four reports alleging the rear brakes locked or applied while driving without brake pedal application. One of the reports alleged a fire in the right rear wheel area.The vehicles subject to the investigation are equipped with an optional Electronic Parking Brake (EPB). In this system, an electric motor applies the parking brake by moving the rear brake pads (pads) against the brake rotors (rotors) when activated by the EPB control switch. The EPB is released by the control switch or by selecting a drive gear with the transmission shifter when the ignition is on.If the EPB has not been used within 1000 kilometers (621 miles) the system is designed to perform an Automatic Adjustment Function (AAF) of the pads to check for proper clearance. This function is a pad adjustment only and is preformed with the vehicle in park with the ignition off. It does not set the parking brake or turn the brake indicator light on. Hyundai has identified an error within the EPB control logic that can cause the pads to remain in contact with the rotors at various levels of force after the AAF is completed. This condition could cause the rear brakes to drag at the next drive cycle and may generate some levels of heat and smoke. The EPB control logic error affects model year 2016 vehicles produced from the start of production through March 16, 2016.Vehicles produced from March 17, 2016 incorporate an updated EPB control logic to prevent brake pad drag. On May 11, 2016 Hyundai
notified its dealer network of Service Campaign TFF to update the EPB control logic in the affected vehicle population. Dealers will reflash the EPB software to prevent brake pad drag.There are no reports of accidents or injuries as a result of this issue. The incident that alleged fire could not be confirmed as there were no repairs to the vehicle other than replacement of the rear brake rotors and pads.ODI is closing this investigation based on the manufacturer's Service Campaign of the affected vehicles and in consideration that the the effect of brake drag in this instance does not appear to present an unreasonable risk to motor vehicle safety. Background On October 1, 2015, SAF-Holland notified the agency that it is recalling certain 5-Inch Tapered Axles used in trailer applications, part numbers 24790175090, 24790745090, and 24790165090, manufactured December 13, 2011, to December 25, 2011.In the affected axles, the wheel end may separate from the axle due to an out of specification material thickness on the spindle.A wheel end that separates from the trailer axle increases the risk of a crash.This matter was assigned recall number 15E-080.In its 573 Defect Notification Report, SAF-Holland identified that it sold loose axles to Vanguard National Trailer Corporation designated to be installed as original equipment to multiple trailers manufacturers.On August 12, 2016, an Equipment Query (EQ) was opened to obtain a list of purchasers that were sold these axles so they are made aware of SAF-Holland?s safety defect decision and to ensure the follow-up on defect decisions are made as to the products that incorporated the defective equipment.On November 3, 2016, SAF-Holland responded via email and provided the requested purchaser list.RECALL MANAGEMENT DIVISION ACTION RMD sent letters to each purchaser between November 4, and December 2, 2016 and requested that a safety recall decision be made, that the requisite notice be filed, and that a recall campaign be conducted where appropriate.As a result, one company filed a defect report and is conducting a safety recall: ? The Office of Defects Investigation (ODI) is upgrading its Preliminary Evaluation of ARC Automotive Inc. (ARC) air bag inflators to an Engineering Analysis. ODI opened PE15-027 in July 2015 based on two injury incidents involving a driver air bag inflator rupture. One incident involved a 2002 Chrysler Town & Country that utilized a dual-stage air bag inflator. The other involved a 2004 Kia
Optima that utilized a single-stage inflator. Both driver air bag inflators were manufactured by ARC, a tier-two supplier of automotive air bag systems, at their manufacturing facility in Knoxville Tennessee. All ARC driver air bag inflators are a hybrid
design that fills the air bag by releasing an inert gas mixture stored in the inflator at high pressure. The gas mixture is augmented by an ammonium nitrate based propellant. The pressurized gas mixture and propellant are contained entirely within a hermetically sealed steel housing isolated from external atmospheric conditions.During the course of PE15-027, ODI requested information from ARC about which air bag module manufacturers used the subject ARC inflators. Based on the information received from ARC, ODI requested information from the identified air bag module manufacturers about which vehicle manufacturers used modules with the subject ARC inflators. That process identified two additional affected vehicle manufacturers, General Motors
and Hyundai
.Based on the age of the vehicles involved in the two incidents in the U.S., ODI focused on single- and dual-stage inflators manufactured by ARC from the start of production to September 2004. It is estimated that approximately 8 million inflators, both single- and dual-stage, were manufactured for use in vehicles produced by Chrysler
, GM, Kia
and Hyundai
for sale or lease in the United States during that time frame.In addition to identifying models using the subject ARC air bag inflator, ODI requested that the affected vehicle manufacturers supply the details for any type of complaint, field report, lawsuit, arbitration or other report involving any vehicle manufactured for sale or lease in the United States equipped with air bag modules utilizing an air bag inflator manufactured by ARC regardless of inflator type (dual vs. single) or position(i.e. driver, passenger, side, etc.). No additional field failures were reported to ODI by the vehicle manufacturers.Additionally, ODI received information from ARC regarding the design, manufacture and testing of the single- and dual-stage driver air bag inflators. In July 2016, ODI was informed by Transport Canada of a fatal incident involving a driver air bag rupture in a 2009 Hyundai
Elantra. It was determined that incident inflator was manufactured by ARC and had ruptured in substantially the same manner as the two previous incidents known to ODI.The driver air bag module in the subject 2009 Hyundai
Elantra utilized a single-stage inflator manufactured at ARC's facility in China. ARC confirmed that the inflator in the 2009 Hyundai
Elantra was substantially the same design as the single-stage inflator in the 2004 Kia
Optima and was assembled using substantially the same manufacturing process.The U.S. market model 2009 Hyundai
Elantra did not use the single-stage driver air bag inflator produced by ARC in China. It is unknown at this time if any of the inflators manufactured in China were used in vehicles produced for sale or lease in the United States. ODI's investigation will focus on determining the entire US population of ARC manufactured driver air bag inflators, single- and dual-stage, identification of affected vehicle manufacturers, and whether any single-stage driver air bag inflators manufactured at ARC's facility in China were used in vehicles produced for sale or lease in the United States.Additionally, ODI will conduct a program to recover the subject ARC inflators from vehicles in the field for further testing and evaluation in support of root cause analysis.
