The Office of Defects Investigation (ODI) opened this investigation in July 2016 based on forty-three (43) Vehicle Owner Questionnaires (VOQ).During the course of this investigation ODI received a total of 145 VOQs related to the alleged defect on the subject motorcycles.Owners allege they experience a sudden and complete loss of brakes on one or both wheels.To better understand the cause of the braking issue on the subject motorcycles, ODI requested NHTSA's Vehicle Research and Test Center (VRTC) conduct inspections and testing of affected vehicle braking systems.Through its work, VRTC determined that deposits were forming on the valves in the Hydraulic Control Unit (HCU) of the Antilock Braking System (ABS). These deposits could prevent the proper function of the HCU and render the brakes ineffective.To understand why deposits were forming in the HCU, Harley Davidson completed an analysis of the Dow 460 brake fluid
used as original equipment on the subject motorcycles and determined that it had stability issues if left in the brake system for an extended period of time.Harley Davidson found that if a brake fluid
flush was not conducted as outlined in the owner?s manual for the motorcycles, and was left in the system for a period of time beyond their recommendation; deposits could form in the HCU.Through further testing, Harley Davidson identified an alternate, commercially available, brake fluid
that did not have the same stability issues as the original fluid and could stop the proliferation of any current brake system deposits.ODI sent Information Request (IR) letters to Harley Davidson and Dow Chemical (the supplier of the brake fluid
on the subject motorcycles) to collect information about the subject bikes and the alleged defect.Harley Davidson responded in full on Sept 30, 2016 and Dow responded with a confidential submission on October 25, 2017.ODI analyzed the information contained in both IR responses and concluded that motorcycles manufactured during Model Years (MY) 2008, 2009, 2010, and 2011 were failing at a rate greater than their peers.Based on this analysis and the brake fluid
study conducted by Harley Davidson, a group of motorcycles was identified that were potentially affected by the instability of the original brake fluid
.2008 marked the introduction of ABS
on Harley Davidson motorcycles and a change in the Dow 460 brake fluid

Explorers pertaining to exhaust odor claims.ODI has identified three crashes and 41 injuries potentially linked this issue.The reported injuries range from unspecified to loss of consciousness, with the majority being nausea, headaches, or dizziness ? all of which can be symptomatic of carbon monoxide exposure.Additionally, Ford
provided 2,400 reports including owner complaints, warranty claims, dealer field reports, and legal claims, that involve 2,051 vehicles that may be connected to the exhaust odor issue.NHTSA's Vehicle Test and Research Center tested multiple vehicles, including complaint vehicles, during the investigation.ODI also conducted field inspections of complaint vehicles and crashes involving police units that occurred while the officers were on duty.When possible, data was collected to quantify carbon monoxide levels in the examined vehicles.Based on the information gathered to date, NHTSA upgraded this investigation to an Engineering Analysis (EA17-002).

was being operated in Autopilot mode at the time of the collision; 2) the Automatic Emergency Braking (AEB) system did not provide any warning or automated braking for the collision event; and 3) the driver took no braking, steering or other actions to avoid the collision.On June 28, 2016, NHTSA opened PE16-007 to ?examine the design and performance of any automated driving systems in use at the time of the crash.?The Office of Defects Investigation (ODI) analyzed the following subjects as part of NHTSA?s examination of the design and performance of Tesla
?s Autopilot system:1) Automatic Emergency Braking (AEB) system design and performance in the subject Tesla
and peer vehicles; 2) human-machine interface issues related to Autopilot operating mode; 3) data from crash incidents related to Tesla

has implemented in the Autopilot and AEB systems.NHTSA?s examination did not identify any defects in the design or performance of the AEB or Autopilotsystems of the subject vehicles nor any incidents in which the systems did not perform as designed.AEB systems used in the automotive industry through MY 2016 are rear-end collision avoidance technologies that are not designed to reliably perform in all crash modes, including crossing path collisions.The Autopilot system is an Advanced Driver Assistance System (ADAS) that requires the continual and full attention of the driver to monitor the traffic environment and be prepared to take action to avoid crashes.Tesla
's design included a hands-on the steering wheel system for monitoring driver engagement.That system has been updated to further reinforce the need for driver engagement through a strike out strategy.Drivers that do not respond to visual cues in the driver monitoring system alerts may strike out and lose Autopilot function for the remainder of the drive cycle.A safety-related defect trend has not been identified at this time and further examination of this issue does not appear to be warranted.Accordingly, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency will monitor the issue and reserves the right to take future action if warranted by the circumstances.For more information about the analysis, see the attached report.

On June 14, 2016, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE16-006 to investigate 135 complaints alleging incidents of yaw rate sensor malfunctions resulting in unexpected Vehicle Stability Control (VSC) activations and steering pull while driving in model year (MY) 2001-2002 Toyota Sequoia vehicles.ODI identified a total of 958 incidents in complaints provided by Toyota
or submitted to ODI by consumers.ODI conducted the following work to assess the alleged defect in the subject vehicles: 1) evaluated the design of the subject yaw rate sensor; 2) initiated vehicle testing and instrumentation; 3) performed failure analysis on subject components collected from the field; 4) analyzed field data for evidence of system faults; and 5) reviewed the performance of the subject system in fault simulations testing conducted by Toyota
.ODI review indicated that an overwhelming majority of incidents are related to flashing VSC slip indicators on the instrument panel accompanied by warning buzzers and grinding noise or vibration caused by pulsed brake application to one of the front wheels.Yaw rate sensor malfunctions resulting in VSC brake applications is accompanied by a brief duration of longitudinal deceleration with minimal changes in vehicle speed and will not result in abrupt lane change or loss of vehicle control due to steering controllability.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety related defect does not exist.For additional information, see the closing resume appendix and the investigation file for PE16-006.
