After a preliminary review of consumer complaints and other information related to alleged defects, NHTSA obtains information from the manufacturer(including data on complaints, crashes, injuries, warranty claims, modifications, and part sales) and determines whether further analysis is warranted.  If warranted, the investigator will conduct a more detailed and complete analysis of the character and scope of the alleged defect.

Summary

In a part 573 Safety Recall Report dated March 4, 2015, HaldexeBay Commercial Vehicle Systems notified NHTSA that it decided that a defect exists in certain Type 30/30 Long Stroke HaldexeBay Life Seal Actuators manufactured from December 17, 2013 to February 18, 2015. The affected actuators are equipped with an incomplete attachment of the internal rubber diaphragm to the actuation mechanism of the parking brake section of the Life Seal Actuator. If the brakes drag or overheat it may result in smoke or thermal damage to the vehicle and/or chassis components.On December 7, 2015 the Office of defects Investigation opened Equipment Query, EQ15-006, in order to obtain HaldexeBay's customer list and to contact the companies that purchased the defective actuators, notify them of the defect, and ensure thorough safety recalls are conducted where appropriate. Eighteen Information Request letters were sent out on May 3, 2016, to each corresponding company requesting a safety recall decision be made, the requisite notice filed, and recall campaign conducted where appropriate. As a result, the following five companies have filed defect reports and are conducting safety recalls:?MeritoreBay, Inc. (16E-004) ?John Evans Mfg. Summary

On May 10, 2016, FiateBay ChryslereBay America (FCAeBay) submitted a Part 573 Defect Information Report announcing NHTSA recall 16V290 involving 394,258 model year (MY) 2007-2010 JeepeBay Wrangler vehicles.Under the recall FCAeBay will replace the clockspring and install a steering wheel back cover and steering wheel column shroud.This recall also includes approx. 4,524 MY2007-2010 right hand drive (RHD) JeepeBay Wrangler vehicles commonly used for rural postal delivery services.In addition 7,180 MY 2011-2016 right hand drive JeepeBay Wrangler vehicles were also recalled under 16V288, the same remedy repair will be used.Upon review of the 1,942 MY 2007-2010 JeepeBay Wrangler consumer complaints, 17,195 warranty reports and field returned parts assessment, FCAeBay concluded that a defect exists in the driver side clockspring assembly, a device that both connects (electrically) the driver air bag to the control module and allows rotational movement between the steering wheel and fixed steering column.Clockspring failure may compromise the air bag circuit causing illumination of the air bag warning lamp, and may result in non-deployment of the driver air bag in a frontal crash.FCAeBay identified a contributing factor in that the recalled vehicles may be subjected to off-road dusty environments and/or usage with the vehicle roof top/doors removed thus allowing dust and moisture to enter the clockspring assembly.FCAeBay advised that the MY 2011 and later Wrangler vehicles had an improved steering column cover/shroud that may reduce the amount of dirt/moisture that can enter the clockspring assembly.During the investigation FCAeBay and the Office of Defects Investigation (ODI) also reviewed 193 MY 2011-2012 JeepeBay Wrangler consumer complaints, 3,918 warranty reports and field returned parts along with reports from other FCAeBay vehicles that use the same clockspring assembly.Based on this review, the rate of repair and the failure mode does not indicate a defect trend at this time in the later MY Wrangler or other FCAeBay vehicles.During the field return parts review, FCAeBay found a significant number of MY 2011-2012 Wrangler returned parts that operated normally, the so called ?no trouble found? or NTF parts where no fault could be identified through bench testing.On June 10, 2016, FCAeBay informed ODI of its intention to conduct a 15-yr, 150k mile extended warranty program due to the NTF issue and the MY 2011 and later vehicles having a somewhat elevated level of warranty replacement.Consumers will be notified of the program in writing.FCAeBay will collect clocksprings replaced under the warranty extension program to further examine and study the root cause failure.ODI will monitor FCAeBay?s analysis to determine if any further field action is warranted.Based on the actions taken by FCAeBay to recall the MY 2007-2010 JeepeBay Wrangler vehicles and the MY 2008-2016 right hand drive Wrangler vehicles, this investigation is closed.FCAeBay has also agreed to conduct a warranty extension and further evaluation of the MY 2011-2016 Wrangler.At the time of FCAeBay's field data submission in PE 15-025 (September 2015), ODI was aware of a total of 416 MY 2007-2010 and 23 MY 2011-2012 VOQs relevant to this issue.In addition, there have been an additional 90 and 16 VOQs (respectively) received since the recall announcement (i.e., thru May 2016).The reports noted above can be viewed at SaferCar.gov under the reference (ODI) numbers cited on the attached listing.

Summary Description: Front Tire Overload


Summary

As detailed in the opening resume for this investigation dated November 16, 2015, Blue Bird Corporation (BBC) conducted recall 07V-586 with two subsequent amendments to address overloaded front axles and tires on the subject vehicles.Following the remedy installations, NHTSA received nine petitions and seven consumer complaints (many from the same owners who had filed petitions) alleging the remedial actions taken by BBC were inadequate.At the time, NHTSA was aware of two consumer complaints and three crashes of vehicles that received the 07V-586 recall remedy.One of the complaints was similar to the petitions in that it focused on utility loss and not safety issues.The second complaint (received May 2014) described a crash that occurred in May 2013 that was also identified in media reports. The second and third crashes occurred in August 2013 and January 2015. All three crashes allegedly resulted from failure of one of the front tires.During this investigation, NHTSA requested information from BBC including details about the three referenced crashes and if they were aware of any additional failures.BBC responded that they were not aware of any other failures and had only received crash information on one of the incidents from sources other than NHTSA.In order to understand why the tires failed prior to or during each of the three crashes, NHTSA attempted to collect as much information about each incident as possible.This included insurance documents, photographs, police accident reports, vehicle weights, maintenance history, and the failed tires.In the case of two of the crashes, not enough information could be obtained to determine the exact cause of the tire failure and crash.In the case of the third crash, the tire was collected and analyzed.This analysis found that the tire failed as a result of impact damage and not overloading.In October 2015, BBC initiated a customer service campaign (S15YF) where owners would be notified that BBC would replace the front wheels and tires on the subject vehicles.This action was taken to address owner concerns that the ride comfort in the vehicle was bad and that tire pressure was difficult to maintain at the required 130psi.The new wheels and tires installed by BBC increased the load capacity of the tire, allowing owners to carry the maximum load of the coach at a lower tire pressure.At the time of the closing of this investigation, NHTSA does not have the necessary evidence to establish the existence of an unreasonable risk to motor vehicle safety in the subject vehicles.NHTSA is not aware of any front axle failures in the subject vehicles for any reason. The single tire failure crash where sufficient evidence had been preserved resulted from the vehicle striking an object damaging the tire.The action taken by BBC to increase the load capacity of the tires on the subject vehicles addresses the safety concerns prompting this investigation.This investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency may take further action if warranted by the circumstances.

Summary

In a letter dated September 15, 2015, a consumer submitted a petition to the National Highway Traffic Safety Administration (NHTSA) alleging that software defects caused an accident experienced by his wife in their model year 2010 LexuseBay HS250H vehicle as she was attempting to park the vehicle. The petition requests that NHTSA have ToyotaeBay correct software defects in their electronic throttle control software and then issue a national recall of all effected [sic] vehicles and have ToyotaeBay replace the old faulty code with the new safer code. NHTSA has reviewed the material cited by the petitioner.The results of this review and our evaluation of the petition are set forth in the DP15-007 Petition Analysis Report, published in its entirety in the Federal Register and included in the repository for this investigation, available at www.safercar.gov. The Agency performed a thorough technical review of the concerns raised in the petition.The petitioner believes that the information from the Event Data Recorder (EDR), which records certain pieces of information for five seconds prior to a crash, shows that for the last 0.8 seconds prior to hitting the brick building the accelerator pedal was wide open at 100% at the same time the brake pedal was being depressed. Our analysis of data recovered from the EDR and from the engine management HybrideBay Control Unit (HCU), together with vehicle inspection information contained in the petition, indicate that:1) Sometime during the last 0.512 seconds of the event time frame the accelerator pedal was fully applied; 2) At the airbag trigger point (EDR t = 0s) immediately following the vehicle's impact with the building, the stop lamp switch status was ON, indicating that the brake had been applied at least enough to illuminate the brake lights; 3) At no time during the event were the brake and the accelerator pedals depressed simultaneously (based on information from the HCU); and 4) The braking system on the vehicle was fully functional and was capable of stopping a vehicle at full throttle if meaningfully applied in sufficient time. After a thorough assessment of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to safety implicated by the petitioner's allegations, it is unlikely that an order concerning the notification and remedy of a safety-related defect would result from any proceeding initiated bygranting the petition.After full consideration of the potential for finding a safety related defect in the vehicle, and in view of NHTSA's enforcement priorities and its previous investigations into this issue, the petition is denied.
