6.2 liter engine failure @ just over 100,000 miles. Prior to 100k miles I had taken to the dealership for sporadic loss of oil. They confirmed that the oil had been upgraded previously as instructed by GM so must be another source of the leak. Engine locked up while driving down the road. Had to have the vehicle towed and the mechanical said the motor is shot, and related to lower camshaft bearing issue. The only warning signs were the low oil light which happened around the mid 90k miles, so I would monitor that. When the engine failed there was no low oil light on, it just started shaking/vibrating and then kicked out. I think hte mileage read right around 103,000 approx. when it died.

On the evening ofXXX, at approximately 6:00 PM, I was driving this Avis rental vehicle on a highway in [XXX] at normal highway speed, below the posted limit, with my foot on the accelerator. The forward collision warning and automatic emergency braking system activated and applied the brakes hard. There was no vehicle, pedestrian, obstacle, or hazard ahead of me, and no vehicle behind me or beside me. The roadway was clear and dry. The sun was low in the sky, and the vehicle was crossing a metal expansion joint on an overpass at the moment of activation. I believe the low sun angle, the expansion joint, or both caused a false object detection by the forward-facing camera or radar. The service brakes themselves functioned correctly. The failure is in the forward sensing and decision logic, which commanded a braking event with no object present. The deceleration was severe and unexpected. I sustained sharp pain in my neck and shoulder. Had any vehicle been following me, this would have caused a rear-end collision at highway speed. There were no warning lamps, messages, or other symptoms before the event, and none afterward. I had driven the vehicle for seven days with no prior indication of a fault. I exited the highway immediately and drove back to the rental location on local streets at low speed because I no longer trusted the vehicle. I returned it the following morning when the Avis office opened, more than a day early, and reported the defect. Being a rental vehicle it was not available to me for inspection. The rental company has advised that it was taken out of fleet for inspection after my return, that there are no open recalls on it, and that it will not release internal service records to me. I do not know the outcome of that inspection. To my knowledge the fault has not been reproduced or confirmed by a dealer, independent service center, or manufacturer representative. The contact's daughter owns a 2019 Hyundai Veloster N. The contact stated that the brake pads needed to be replaced and that the rear calipers were leaking brake fluid. The local dealer was contacted but the seals for the repair were not available. The seals were purchased from O’Reilly’s Auto Parts. The contact stated that the left-side cable had popped off, and the vehicle rolled back while in first gear. There was low compression in the engine. The contact associated the failure with NHTSA Campaign Number: 24V528000 (Fuel System, Gasoline). The contact stated that a compression test was performed. The vehicle was taken to the local dealer, where a leak-down test was performed for one minute, but the result was not provided. The dealer notified the contact of an extended warranty coverage. The dealer stated that the pump could not be replaced. The fuel pump was previously replaced. The contact stated that the crankcase was saturated with raw fuel and the vehicle was unsafe to drive. The dealer needed DTC: P0088 to be populated. The dealer requested that the vehicle be driven for 3,000 more miles before being taken back to the dealer. While receiving an oil change there was an abnormal fuel odor coming from the vehicle. The contact stated that the crank case was saturated with raw fuel. The contact was concerned about a potential fire. The manufacturer was contacted by phone. The failure mileage was approximately 114,941.

Summary
Can smell gas fumes after filling gas tank. Had independent shop analyze problem and codes indicated a cracked fuel pump module. This is a serious hazard because fuel can leak onto exhaust or heat shield and cause a fire. GM currently has a Special Coverage Program #17469 for 2014-2017 corvette. My 2019 corvette has the exact same issue and should be covered under Program #17469. My corvette has only 23000 miles.

There is a malfunction in the AWD system that gives a code for low voltage. I tracked down the issue to a factory defect where the connector to the AWD module doesn’t have a ground wire. Ford has acknowledged the issue under “Ford Customer Satisfaction Program 21B19” but it has since expired. (Expired after the vehicle was purchased) As a result of the fault, It disables AWD, hill start assist, forward collision assist.

Summary
I am writing to formally report a dangerous safety defect involving my 2015 Ford F-150 XLT SuperCrew Cab. While operating the vehicle, I experienced a sudden and total loss of control. Without warning, the gas pedal lost all tension, the truck abruptly downshifted and went into limp mode, and both the check engine and wrench warning lights illuminated on the dashboard. This loss of control while driving created an immediate and severe safety hazard. My investigation into this failure reveals a widespread pattern of similar incidents involving the transmission molded leadframe. It appears that the failure of this component frequently causes an output shaft speed sensor malfunction, leading to the exact dangerous loss of power I experienced Given the high frequency of these reports from other owners. This loss of control while driving created an immediate and severe safety hazard. Upon experiencing these symptoms, I promptly scheduled an appointment at my local Ford dealership. Despite the service department confirming that the transmission lead frame is failing, Ford refused to perform the repair citing that my specific trouble codes are not currently approved under their policy. This is unacceptable, as these symptoms are identical to those addressed in recall 26S28 and Ford has previously acknowledged that these specific codes are directlv related to this component's failure in other mode years. My investigation into this failure reveals a widespread pattern of similar incidents involving the transmission molded leadframe Given that my vehicle exhibits all the symptoms described in the existing recall, it is clear that this is a systemic manufacturing defect that Ford is failing to adequately address. I request that the NHTSA investigate this issue and Ford's restrictive repair criteria, as this defect poses a significant risk to myself and the public on the road.
