On XXX, at approximately 1:00 p.m., my 2019 Buick
Encore unexpectedly caught fire while I was operating it in [XXX]. Prior to the fire, I experienced abnormal symptoms, including the vehicle failing to restart, a hissing sound, smoke coming from the engine compartment, and then visible flames. The smoke changed from black to white and then green before the fire intensified. I safely exited the vehicle before the fire spread. The exact component that failed is unknown. Based on my observations, the fire appeared to originate from the engine compartment or underneath the front of the vehicle. The vehicle was declared a total loss. This incident placed my safety at serious risk because the fire occurred without warning while I was using the vehicle. Had I not exited when I did, the outcome could have been much worse. The incident also created a hazard to nearby motorists and anyone in the surrounding area. The problem has not been reproduced because the vehicle was destroyed by the fire. The vehicle was inspected by General Motors
after the incident as part of a product allegation investigation. General Motors
later informed me that it found “no merit” to my product allegation but refused to disclose the basis for its conclusion, stating that its investigation materials are confidential and proprietary. As a result, I still have not been provided with an explanation for what caused my vehicle to catch fire. I am submitting this complaint because I believe an unexpected vehicle fire is a significant safety concern that should be documented and evaluated. My 2025 Kia
Sportage PHEV, brand-new with ~750 Miles, was in a low-speed collision with a parked vehicle when the Forward Collision-Avoidance Assist (FCA
) system failed. I was pulling out of a parking spot from a complete stop. My foot was not on the accelerator; the vehicle moved forward under idle power as I prepared to brake. A parked vehicle was directly ahead. The FCA gave no audible warning, no visual warning, and applied no automatic braking. A collision occurred. This system is designed to detect obstacles ahead and warn or brake to prevent a forward collision. It did neither, despite a large, solid, stationary vehicle directly ahead, in clear conditions, with an unobstructed sensor. In normal use it routinely alerts on nearby cars and objects, so it is active and sensitive to exactly this hazard. Its failure to detect a full-size vehicle directly ahead indicates a malfunction of a critical safety system. No warning lamps appeared before the failure. Only after the collision did the vehicle display a "Check Forward Safety System" warning. This is not the only concern. On April 3, 2026, over two months before this collision, I noted in writing to the selling dealer that Lane Keeping Assist would sometimes turn off while driving, the dash indicator shifting from green to white to grey to black. I was told this was normal, so I did not pursue it. Intermittent loss of an active safety system is itself a concern. Together these may indicate broader driver-assistance problems warranting investigation. The system is available for inspection. The vehicle was delivered to the dealer June 27, 2026 and remains there. Diagnostics are not complete, so the malfunction has not been confirmed and no findings produced. The contact owns a 2014 Hyundai
Santa Fe Sport. The contact stated that while driving at undisclosed speeds, the vehicle started to lose power while depressing the accelerator pedal, with black smoke coming from the engine compartment. The oil, check engine, and battery warning lights were displayed on the instrument panel. The contact pulled over and discovered black smoke coming from under the hood. Due to the failure, the vehicle was towed to an independent mechanic, and the engine was replaced. Two weeks after the replacement, the starter failed to operate as needed, and the vehicle was towed back to the independent mechanic, where the starter was replaced. Upon investigation, the mechanic discovered NHTSA Campaign Number: 17V226000 (ENGINE AND ENGINE COOLING); and associated the vehicle with the recall. The contact was referred to the NHTSA Hotline to report the failure. The vehicle remained with the independent mechanic and was not repaired. The manufacturer was not notified of the failure. The failure mileage was 120,577.

I wrote Audi
to request escalation of an unresolved issue involving my 2018 Audi
Q5, related to Safety Recall 90S9 (NHTSA Campaign 21V947000) for the gateway control module. Summary of the issue: My vehicle recently experienced a complete gateway control module failure due to water intrusion. I lost steering and brake assist warnings while driving, and the vehicle would not restart after being pulled over safely. It was towed first to the nearest Audi mechanic who diagnosed the issue as Recall 90S9. We then contacted Audi Memphis for inspection by them since it was a recall issues and paid a 2nd towing charge. Audi
Memphis (owned by Gossett Motor Cars) said they could not do anything until they looked at the car. I have since obtained a copy of the original repair order from when Recall 90S9 was performed on my vehicle. The vehicle at the time of recall repair was owned by my Mother. The recall requires two remedies: (1) installation of a protective cover over the gateway control module, and (2) sealing of an underbody seam on vehicles produced before September 2021. The repair order shows a technician comment reading "INSF" next to the seam sealant line item, which indicates the sealant application was noted as insufficient at the time of the original recall service. Despite this documentation, and despite towing my vehicle in specifically for a 2nd inspection, Audi Memphis has stated they have no findings and no liability for the current failure. Given that: • My vehicle is included in the affected range for Recall 90S9, • The original repair order appears to document an incomplete or insufficient sealant application, and • Audi
's own 90S9 technical bulletin states that damages resulting from improper repair or failure to follow the work instructions are the dealer's responsibility, I am requesting that this case be escalated for review. The current gateway control module repair be repaired at no cost to me. I currently have no working vehicle.

The contact owns a 2022 Fleetwood Frontier RV. The contact stated that while driving at an undisclosed speed, the passenger’s side window located next to the entry door was loosened and exposed. The window was a single-pane, frameless model measuring approximately 24X12 inches, which opened from the bottom by a cranking mechanism. Although the window was cranked shut, the wind pressure caused the window to shake violently, almost ripping the window from the frame of the RV. The entry door was not properly fitted to the body of the RV. The contact stated that the cargo bay doors located at the rear of the RV were loosened and unstable. Upon inspection, the contact discovered that the rear bay doors were constructed out of particle board and had only one screw holding the door in place. The contact stated that the doors were exposed to the elements, completely open to the engine compartment, and showed signs of water intrusion. Additionally, the contact stated that none of the exterior doors or door hinges were properly sealed. The dealer was notified of the failure. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 36,000.
