Chronology :

-2016-12-24 First Case reported

-From 2016-12-24 to 2020-12-08 nine (9) more cases were reported through field reports

-2021-04-23 First analysis on alternator front bearing failure. No evidence was found to consider this failure as a safety related issue.

-2021-05-18 NHTSA-ODI opened a preliminary evaluation (PE21-011) related to alternator front bearing failure and AFSS activation.

-2021-07-23 Prevost sent the package of information required by ODI

-2022-01- ODI came back to Prevost with a new set of questions

-2022-02-28 Prevost sent a package to answer the ODI

-2022-03-09 NHTSA and Prevost met to discuss ODI findings. ODI defined that even with the low percentage of failure on alternators because of front bearing failure, there could be a safety related defect related to front bearing failure

-2022-03-15 Internal inquiry was started to analyze root cause. A link between the introduction of new belts and an increase of thermal events related to alternator’ front bearing failure was identified. This new information triggered a new new case to present to Volvo Product Safety Committee (PSC)

-2022-03-30 Case presented to PSC. PSC decided there is a safety related defect and a recall is required

-2022-03-31 Preliminary discussion with NHTSA to expose the findings

-2022-04-06 Prevost submitted the vehicle defect report to NHTSA

1 Associated Investigation

Alternator Front Bearing Failure
NHTSA ID: PE21011
Dated opened: May 13, 2021

NHTSA’s Office of Defects Investigation (ODI) is opening a Preliminary Evaluation after receiving a tip from a fleet safety manager concerning alternator front bearing failures that cause elevated temperatures within the engine compartment, resulting in the fire suppression system (FSS) activating, immediately shutting the engine down, and rendering the bus inoperable.

Review of Early Warning Reporting Data identified 9 field reports for Model Year (MY)2015 through MY2019 H3 and X3 Prevost buses, describing alternator failures in any of 3 alternators equipped on the bus. In some cases, a minor fire occurred damaging nearby electrical harnesses.

An immediate shut down of the bus can increase the risk of a crash or injury of passengers when transferring to another bus.

ODI is opening this investigation to evaluate the cause, consequence, scope and frequency of the alleged defect in the subject Prevost buses.
