Summary
Oshkosh Corporation (Oshkosh) is recalling certain 2019-2022 S-Series vehicles. The u-bolts securing the front axle may fail.
Remedy
Oshkosh will provide new u-bolts, free of charge. Owner notification letters were mailed February 24, 2022. Owners may contact Oshkosh customer service at 1-888-686-7278.

Purpose:
A concern affecting safety exists in some Oshkosh S-Series Front Discharge Mixers. Vehicles have been identified that are equipped with front axle U-bolts that were manufactured incorrectly and do not meet the required material specifications. The defect may cause the front axle U-bolts to loosen or fall off, resulting in the front axle shifting or the axle becoming dislocated from the vehicle. This bulletin provides instruction on replacing the front axle U-bolts.

Chronology:
In late July 2021, Oshkosh was made aware of a customer report of noise coming from the front end of a model year2020 vehicle. The vehicle was relatively new and had been in service for only approximately one year. The report only included a general observation that the driver had noted a popping noise during turning maneuvers but could not locate the source of the noise.
The customer was advised to have the vehicle inspected and in July 2021, Oshkosh technicians inspected the vehicle and found visible deformation to the U-bolts in the front axle. The U-bolts were replaced, but thereafter the customer again reported noises from the vehicle in the area of the front axle.
Following the customer’s second report, Oshkosh’s investigation began to focus on the properties of the U-bolts to understand whether they may have some influence on the noise being reported by the customer. In August 2021, Oshkosh’s certified materials laboratory began an analysis of these Ubolts. Oshkosh also collected samples of U-bolts from other production lots. Testing was conducted through early Fall 2021 to evaluate the material properties of the U-bolts.
After testing multiple samples of U-bolts, and throughout various part lots, it was determined that certain sets of the bolts did not meet the engineering specifications for material hardness. Thereafter, Oshkosh contacted the manufacturer of the subject U-bolts to understand what may have led certain sample parts to not meet the material specifications. The supplier’s investigation confirmed that these samples did not meet the specifications for the U-bolts.
In the Fall, 2021, Oshkosh began to evaluate the potential impact of a U-bolt that did not meet the engineering specifications for hardness when installed at the front axle. This analysis consisted of engineering simulations and in-vehicle testing as well as an evaluation of whether the U-bolts that did not meet material hardness meet specification for clamping load. Oshkosh also worked with the supplier to identify the affected production lots and to correlate those with vehicle production. This evaluation and analysis lasted through the end of 2021.
January 5, 2022- Oshkosh determined that this issue was a safety issue and would go to the field with a Safety Recall to fix the affected population of vehicles.
To date, Oshkosh has only received the single customer report. Oshkosh has not received reports of any incidents involving the front axle becoming loose, shifting out of place or detaching from the vehicle in the field, but out of abundance of caution is going to the field to correct this issue.

This bulletin provides instruction on replacing the front axle U-bolts. Affected Vehicles: The affected vehicle population is Oshkosh S-Series Front Discharge Mixers produced in 2018-2021. Units Affected: VIN 788727 – 814106 Please refer to the attached VIN list of affected vehicles. Time Completion: Complete the enclosed procedure for each affected vehicle. The time of accomplishment may vary due to a number of factors; however, the estimated time for replacement is 1.5 hours. Required Action: Review and verify your ownership of all affected serial numbers of Oshkosh vehicles. Oshkosh records indicate that you are the owner of one or more affected vehicles, a Serial Number List is enclosed. Please be reminded that it is a violation of Federal law for you to sell or lease the vehicles covered by this notification until this recall has been performed on these vehicles. Substantial civil penalties apply to violations of this law. Reimbursement of Costs: Affected vehicles shall be repaired using the customer’s own qualified service technicians – at no cost to the owner. The repair should take approximately 1.5 hours. Parts/Labor Reimbursement: In order to receive credit, claims are to be submitted through the normal warranty claim system. Claims must be received within 30 days of the repair. Battery Disconnect Switch The vehicle’s BATTERY DISCONNECT switch is located on the left-hand side of the battery box. Use the battery disconnect switch when performing any maintenance so the vehicle cannot accidentally be started. a. Turn the battery disconnect switch (Figure A, Item 1) counterclockwise so the hole on the switch aligns with the hole in the bracket (Figure A, Item 2). b. Install the safety lockout device ring (Figure B, Item 1) through the holes on the battery disconnect switch and the bracket. c. Install a padlock (Figure B, Item 2) onto the safety lockout device ring, lock it, and put the key in your pocket. If more than one person is working on the vehicle, each person must install his or her own padlock. FMVSS 1 : NR FMVSS 2 : NR Description of the Safety Risk : Bolts that do not meet the material specifications and where the material is not sufficiently hard, have the potential to loosen or fail over time and may allow the front axle to shift or come loose and in an extreme case if the condition is not corrected, the front axle could detach from the vehicle. This could increase the risk of a crash. Description of the Cause : Incorrectly manufactured bolts that did not meet material specifications as noted on the Engineering drawing. Identification of Any Warning that can Occur : Noise coming from the area of the front axle may indicate the axle shifting and loose bolts. See miscellaneous document Description of Remedy : Description of Remedy Program : Oshkosh will send new u-bolts that meet material specifications and that will secure the front axle properly. This will be done at no cost to the customer. How Remedy Component Differs from Recalled Component : The correct hardware will meet material properties as originally designed. Identify How/When Recall Condition was Corrected in Production : Production will be installing corrected U-bolts. This change was made in production on November 30th, 2021. A new Oshkosh PN was created to aid this cut-in. Recall Schedule : Description of Recall Schedule : Notifications to customers will be made via a customer letter. The 577 customer letter will be mailed pending NHTSA approval. Oshkosh does not distribute vehicles through dealers.
