Circuit Breaker May Not Function Properly

Summary

Cummins Inc. (Cummins) is recalling certain 2019-2021 Power Generation C70D2RE and C100D2RE trailer-mounted generator sets. Certain Molded Case Circuit Breakers (MCCBs) may have been manufactured improperly, which can result in inadequate circuit protection.

 

Remedy

Cummins will inspect and replace the circuit breakers, as necessary, free of charge. Owner notification letters were mailed January 14, 2022. Owners may contact Cummins customer service at 1-800-286-6467. Cummins number for this recall is C4776.

PURPOSE:

The purpose of this memo is to notify repair locations that Cummins Inc. is initiating end-user notification mailings regarding a safety defect on certain population of PowerPact L 250-600A Molded Case Circuit Breakers (MCCBs) as reported by supplier, Schneider Electric via their Product Safety Notice, PRB-233066.

 

DISCUSSION:

Schneider Electric notified Cummins that a population of PowerPact L 250-600A MCCBs may contain a safety issue requiring urgent attention due to “a nonconforming component in its mechanism that may result in the circuit breaker not operating as intended under certain conditions. Should this condition occur, a potential for PERSONAL INJURY AND/OR PROPERTY DAMAGE may exist”.

Cummins is actively working to identify the serial numbers of the generator sets and transfer switches potentially affected by this issue to launch two Safety Campaigns in the near future (Campaign 4775 – TSB210252 for stationary application and Campaign 4776 – TSB210251 for mobile application). Until that time, Cummins is requesting end-users to apply the warning label provided by Schneider Electric on the unit’s control panel.

The Safety Campaigns are expected to be published on QuickServe Online in January 2022.

Cummins will contact end-users by mail when the Campaigns are published. In the meantime, Cummins is not recommending that end-users take the units out of operation. Cummins is recommending that units be remedied under the Safety Campaign as soon as the remedy is available.

 

ACTION:

Distributors and dealers are requested to look for the new Safety Campaigns going to be published in January on QSOL. Cummins expects to begin notifying owners in January 2022 with letters informing them about the safety defect and instructing them to apply the warning label pending publication of the Safety Campaigns.

Issue

Schneider Electric has notified Cummins that a population of PowerPact L 250-600A Molded Case Circuit Breakers (MCCB) may contain a safety issue requiring urgent attention due to “a nonconforming component in its mechanism that may result in the circuit breaker not operating as intended under certain conditions. Should this condition occur, a potential for PERSONAL INJURY AND/OR PROPERTY DAMAGE may exist”. The purpose of this service bulletin is to cascade information from the Schneider Electric Product Safety Notice, PRB-233066, to:

(1) increase visibility to the Schneider Electric Notice of Potential Unsafe Condition, and

(2) allow for proactive inspection to determine whether a particular unit may be impacted by this issueas part of the Safety Warranty Campaign #4776 launched by Cummins.

 


Schneider Electric is committed to delivering high quality products to our customers. According to our records, your company has recently purchased (refer to Appendix 3) PowerPact L 250 – 600A, ComPact NSX 400 – 630A (please refer to Appendix 2 for details on identifying the impacted product). These products have either been purchased for incorporation into a finished offer such as a Low Voltage Distribution equipment or Generator Set for example (LV switchboard Builder or OEM) or have been supplied directly to an installation.

As part of our ongoing quality monitoring process, we bring to your urgent attention a safety issue that has exhibited itself on some of the PowerPact L 250 – 600A, ComPact NSX 400 – 630A  Molded Case Circuit Breakers (“MCCB”).

The potentially affected MCCB may contain a nonconforming component that may result in the circuit breaker not operating as intended under certain conditions. Should this condition occur, a potential for PERSONAL INJURY AND/OR PROPERTY DAMAGE may exist. Hence Schneider Electric has decided to carry out a full investigation and remedial action.

Corrective actions have been implemented at Schneider Electric to prevent this issue being repeated on shipment of any new MCCB.

The purpose of this letter is to inform you about the potential risk and ask you to provide the proper identification and information. This information will be used to identify and complete necessary remedial work.

We urge you to comply with the requests in this document and follow the containment actions described in Appendix 1.

Chronology :

20 Oct 2021 – Cummins received a written “Product Safety Notice/Notice of Potential Unsafe Condition” from Schneider Electric regarding the subject circuit breakers.

21 Oct – 3 Nov 2021 – Cummins investigated the issue, including conducting a search of warranty claims. Cummins identified 11 warranty claims for replacement of defective circuit breakers which may possibly be due to this condition.

9 Nov2021 – Cummins decided to implement the safety action initiated by Schneider Electric by conducting a safety recall of the generator sets that may have been built with the subject circuit breakers. To date, Cummins has received no reports of accidents or injuries due to this condition.

16 Nov2021 – Cummins filed its initial defect notification with NHTSA.

17 Nov2021 – Cummins has subsequently determined that the production end date for the C100D2RE units in the recall populations was October 13, 2021.

16 May2022 – Cummins completed a Product Safety Hazard Analysis to evaluate the potential risks that this condition may present in Cummins generator sets, with consideration given to application-specific design features.

2 Jun2022 – After further investigation and analysis, Cummins concluded that Cummins generator controls contain features that will provide short-circuit protection should the condition in the MCCB occur, thus mitigating the potential for personal injury and/or property damage. Nonetheless, by way of this Cummins safety recall, Cummins will continue to implement the Product Safety Notice issued by the supplier of the circuit breakers installed on the generator sets.
