Summary
Altec Industries, Inc. (Altec) is recalling certain 2022 DC & DM Series Digger Derrick vehicles. During production, an incorrect bolt may have been used to attach the derrick rotation bearing.
Remedy
Altec will inspect and if an incorrect bolt is found, replace the bolts affected bearing race, free of charge. Owner notification letters are expected to be mailed March 10, 2023. Owners may contact Altec’s customer service at 1-877-462-5832. Altec’s number for this recall is CSN 3045.

For US owners, this notice is sent to you in accordance with the National Traffic and Motor Vehicle Safety Act. For Canadian owners, this notice is sent to you in accordance with the requirements of the Motor Vehicle Safety Act. This is to inform you that your vehicle may contain a defect that could affect the safety of a person.
Altec Industries, Inc. has decided that a defect which relates to motor vehicle safety exists in certain DC45/47 and DM45/47B derricks built from September 2022 to November 2022. One or more of the cap screws installed in the rotation bearing inner and/or outer race may be longer than specified. This can cause the cap screw(s) to bottom out in the mounting hole(s), preventing the required preload from being achieved. While there have been no reports of cap screw failures to date, insufficient preload could cause rotation bearing cap screws to fail prematurely. Failure of rotation bearing cap screws could cause uncontrolled boom movement. Death or serious injury could result from uncontrolled boom movement.
Refer to the included notice for the items covered under the Altec Warranty Policy. If you had this repair performed before you received this notice, you may be eligible to receive reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall. All work will be performed at no charge to the customer when presented for repair.

Rotation Bearing Cap Screw Inspection
Units Affected: Certain DC45/47 and DM45/47B derricks built from September 2022 to November 2022. Verify your unit is affected by reviewing the attached list or accessing Altec Connect.
Background: Altec has learned that one or more of the cap screws installed in the rotation bearing inner and/or outer race on certain units may be longer than specified. This can cause the cap screw(s) to bottom out in the mounting hole(s), preventing the required preload from being achieved. While there have been no reports of cap screw failures to date, insufficient preload could cause rotation bearing cap screws to fail prematurely.
WARNING
Death or serious injury could result from uncontrolled boom movement. Failure of rotation bearing cap screws could cause uncontrolled boom movement.
Customer Action: Inspect the rotation bearing cap screws using the Inspection Procedure on page 2, or contact Altec to perform this inspection. If the inspection shows that repair is required, order and install the specified cap screw replacement kit(s), or schedule the installation of the kit(s) by Altec. Complete the inspection no later than the next preventive maintenance interval or 30 days from the receipt of this notice, whichever comes first. Subsequent damage due to failure to perform the required action(s) in the time period allowed will not be covered by warranty.
Requirements: The inspection is estimated to take 30 minutes and 1 person to complete. The repair is estimated to take 3 hours and 1 person to complete for the inner race and 2 hours and 1 person to complete for the outer race.
Completion and Warranty: The inspection and repair are covered under the Altec Warranty Policy and can be performed by Altec, the customer, or the customer’s warranty provider. Altec will perform the work for free at an Altec facility. If the customer or the customer’s warranty provider performs the work, a warranty claim must be submitted to be reimbursed for the cost of the parts and/or labor. Altec will allow up to $90 for the labor to perform the inspection and up to $450 for the labor to perform the repair. Customers are responsible for the travel costs of an Altec Mobile Service technician if the technician performs the work at the owner’s location.

Inspection Procedure: A flashlight may be required for this inspection. Read and understand all steps of the instructions before beginning the procedure.
- Position the unit on a level surface, apply the parking brake, and turn off the engine. Remove the key from the ignition, and secure it following your employer’s vehicle lockout/tagout procedure. Chock the wheels.
- Visually inspect all the cap screw heads in both rotation bearing races to check for an H501 head marking below the Altec logo (refer to Figure 1).
- Review the inspection results from step 2.
- If the H501 head marking is not found on any cap screws, proceed to step 4.
- If the H501 head marking is found on one or more cap screws, proceed to step 5.
- No repair is required. Perform steps a through d shown below.
- Put the unit back into service.
- Complete the Inspection Sheet at the end of this notice, and return it to Altec.
- If the inspection was performed by Altec, mark the CSN as complete on the Service Request.
- Do not complete the remaining step in this document.
- Repair is required. Perform steps a through f shown below.
- Take the unit out of service until the repair has been completed.
- Determine the kit part number(s) required as shown below, based on which race contained cap screw(s) with the H501 head marking. If cap screw(s) with this head marking are found in both rotation bearing races, both kits are required.

- Arrange for the installation of the required kit(s) using either of the methods shown below.
- Contact Altec Service to schedule the installation of the kit(s).

- Put the unit back into service.

Chronology :
On November 23, 2022, a production associate notified Quality Assurance that some bolts 3.5 inches long were mixed into the box of bolts 3 inches long. An investigation was opened that determined the affected lots of bolts, and production inspected all units in WIP containment. The number of units potentially involved was determined and a defect was declared. There were no warranty claims or field reports identified. There have been no accidents, injuries or deaths related to this issue.

Description of the Defect : A bolt half of an inch longer than called out by the design of the threaded joint could have been used to attach the derrick rotation bearing.

Description of the Safety Risk : If an incorrect bolt was installed, the bolt could bottom out in the blind hole, not allowing the required preload to be achieved. As a result, the adjacent bolts could be overloaded during operation and fail, which could cause separation of the rotation bearing, increasing the risk of death or serious injury to the worker or those nearby.

On November 23, 2022, a production associate notified Quality Assurance that some bolts 3.5 inches long were mixed into the box of bolts 3 inches long. An investigation was opened that determined the affected lots of bolts, and production inspected all units in WIP containment. The number of units potentially involved was determined and a defect was declared. There were no warranty claims or field reports identified. There have been no accidents, injuries or deaths related to this issue. Description of Remedy :
Description of Remedy Program : Altec will issue CSN 3045 to owners of the affected units requiring owners
to inspect the lot number on the bolt head. The 3 and 3.5 inch long bolts have different lot numbers that can be used to determine if an incorrect bolt was installed. The inspection and any required replacement will be conducted by Altec free of charge when presented for repair. The Altec Warranty Policy covers the recall remedy including cases requiring reimbursement for the cost of obtaining a pre-notification remedy of the problem associated with this recall. How Remedy Component Differs from Recalled Component :
Identify How/When Recall Condition was Corrected in Production : The remedy component is a bolt of the same lot that meets the length required for the bolted joint.
