Summary
Clek, Inc. (Clek) is recalling certain Olli booster seats, model number OL12U2 manufactured between January 20, 2020 and May 10, 2022, and Ozzi booster seats, model number OZ11U2, manufactured between March 1, 2020 and May 10, 2022. The installation diagram displaying the proper way to install the booster seat with a continuous-loop lap/shoulder belt is missing. As such, these child seats fails to comply with the requirements of Federal Motor Vehicle Safety Standard number 213, “Child Restraint Systems.”
Remedy
Clek will mail owners a new label, free of charge. Owner notification letters are expected to be mailed, beginning July 25, 2022. Owner’s may contact Clek’s customer service by email at olliozzilabel@clekinc.com or by telephone at 1-866-656-2462.

Clek, Inc. (Clek) has decided that a non-compliance which relates to motor vehicle safety exists in certain Clek Olli and Clek Ozzi Backless Booster Car Seats manufactured between January 20, 2020 and May 10, 2022. As our records show that you have registered as an owner of a Clek Olli or Ozzi Backless Booster Car Seat within the affected production date range, we are sending you this recall notice to advise you of a repair kit being made available to correct the issue.

Description of the non-compliance:
Clek has determined that an illustration showing the vehicle belt routing diagram was not properly displayed on the booster car seats warning label.
Remedy of the non-compliance:
Included with this letter is a label that shows the required vehicle belt routing illustration.
Please follow the instructions below to affix this label to your booster car seat.
Step 1 – Clean the area where the label is to be applied. See Figure 3
Step 2 – Remove the label backing exposing the adhesive.
Step 3 – Apply the label to the side of the plastic armrest.
Step 4 – Slide your thumb or finger over the label applying pressure to ensure the label is properly affixed to the plastic

Chronology :
On May 18, 2022, Clek received a letter from NHTSA ODI dated 5/18/22 advising of a potential noncompliance on the Olli booster car seat during testing of the car seat as part of the FY2021 compliance test program.
On May 19, 2022, Clek opened an internal investigation into the reported potential noncompliance. As the Olli booster car seat is very similar to the Ozzi booster car seat, Clek also reviewed the Ozzi car seat for the potential noncompliance.
On May 31, 2022, Clek confirmed that certain production of the Olli and Ozzi booster car seats do not include a required installation diagram and determined that the affected booster seats do not conform to Section S5.5.2 (l)(1) of FMVSS 213.
Initial 573 submitted on June 7th with the intention to petition pursuant to 49 CFR 556.
On June 16, NHTSA recommends that Clek conduct a noncompliance recall in lieu of submitting its petition for inconsequential compliance due to the fact that the required label was missing. Clek takes NHTSA’s recommendation under consideration.
On June 30, Clek decided to conduct the recall.

Description of the Safety Risk : The safety risk is low as all other labels are present, including a label showing NOT to install with a lap-only belt, the phrase “belt here” is molded into the seat to inform a caregiver of the correct installation and information pertaining to the correct installation, including the lap/shoulder belt routing over the occupant that is shown in the missing illustration, is provided in the instruction manual. In addition, the lap/shoulder belt routing is consistent with the lap/shoulder belt routing for the adult caregiver. However, if the booster car seat is not properly installed with the lap/shoulder belt, the child may not be properly secured by the vehicle belt, increasing the risk of injury in a crash.
Description of the Cause : The label where the required illustration was located had inadvertently been removed from the side of the seat cushion following a change in the sewing supplier. Identification of Any Warning
