On August 31, 2020, ODI upgraded PE18-014 to Engineering Analysis (EA)20-001 to continue its investigation of brake vacuum pump failures in model year (MY) 2012-2013 Range Rover Evoque vehicles and peer Jaguar Land Rover (JLR) products equipped with 2.0L GTDI engines fitted with Magna vacuum pumps. This investigative activity has focused on the cause, scope, and effect of inadequate vacuum supply at the vehicles brake booster with its attendant increases of brake pedal effort and diminished braking performance. Following a detailed analysis of data collected, the alleged defect is progressive and reasonably detectable with adequate power braking assist available prior to complete pump failure; occurs at marginal failure rates, particularly where the vehicle age is considered; and the complaint trend dissipated to one occurrence per year over the last three calendar years.Based on the analysis conducted, this investigation has been closed. On August 31, 2020, the Office of Defects Investigations (ODI) opened Engineering Analysis (EA) EA20002, as an upgrade of Recall Query (RQ) RQ19003. This investigation, related to recall 14V-266, was opened to investigate instances of detachment of the flexible steel seat belt anchor cable attached to the outboard side of the front seats in vehicles manufactured by General Motors (GM). The subject vehicles included model year (MY) 2009 through 2014 GMC Acadia, Buick Enclave, and Chevrolet Traverse SUVs as well as MY 2009 through 2010 Saturn Outlook SUVs. These vehicles were previously repaired for this defect under recall 14V-266. The subject vehicle population is approximately 1.3 million vehicles. Special Coverage N192219480, issued in June 2019, addressed the subject defect in vehicles that previously received the recall remedy repair for subject recall 14V-266.The Special Coverage allows for component replacement for 12 years or 180,000 miles from the initial sale date, whichever comes first. Special Coverage N192219480 instructed service technicians to inspect the driver's seat belt anchor cable for any damage to the protective sleeve and if damage was observed, the cable was replaced with a new sleeve as well as a new closeout piece that relocated the cable to mitigate the subject condition. In view of the Special Coverage conducted by General Motors, the high detectability of the subject failure, and the low rate of occurrence, this Engineering Analysis is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. Summary
On January 9, 2009, Daimler Trucks North America (DTNA) issued Recall 09V-024 on model Year (MY) 2007-2009 Cascadia Trucks for water intrusion into the Signal-detect and Activation (SAM) Chassis and Cab modules resulting in potential electrical shorts that can affect lamp and lighting (tail, stop, side-marker lamps, trailer lighting) functions and possibly result in a fire hazard inside the truck's cabin. Both modules have direct 12-volt power via the Powernet Distribution Box (PNDB). The recall remedy involves installing a new plastic drip shield between the SAM cab module and the bulkhead, to prevent water intrusion from reaching the module.During the review of customer complaints (VOQ) and field reports (submitted by DTNA through Early Warning Reporting) related to SAM cab electrical failures, the Office of Defects Investigation (ODI) found multiple reports and four (4) VOQ on model year 2007-present trucks alleging electrical failures and thermal events related to water intrusion affecting the SAM cab module. Some reports described repeated electrical failures, including fires with the ignition on or off. Numerous reports indicate water had entered the cab through the windshield seals. Two complainants alleged multiple repairs of the windshield seal did not prevent water intrusion into the SAM cab module.In July 2019, DTNA issued a technical service bulletin (TSB) 54-312 instructing technicians to replace the drip shield and the secondary cover on the SAM Cab module with a new one-piece drip shield.DTNA claims the replacement was a product enhancement and would be a good modification when replacing any SAM Cab module. Upon review of the TSB, ODI became concerned the new replacement shield was intended to correct a problem with the shield installed as a remedy for recall 09V-024 and all new production trucks since the recall was announced. Summary
The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE) on August 10th, 2020 to investigate 26 reports of stalling due to clogged fuel filters on model year (MY) 2018 - 2020 Honda African Twin (CRF 1000 - 1100) motorcycles.Many of the owners allege the subject vehicle would suddenly stall while riding. Although the stalling speeds varied, most consumers allege the stalling occurred at highway speeds. This stalling condition is caused by clogged fuel filters. The subject vehicle fuel filters become contaminated with zinc-oxide (corrosion). The corrosion is coming from the inside of the fuel tank, along the end seam of a joint weld. The corrosion clogs the fuel filter, restricts fuel flow to the engine, and subsequently causes a sudden engine stall.During complainant interviews, ODI learned the bikes can sometimes be restarted after the initial stall. However, several consumers described this restart as causing a false sense of security.After restarting the bike and attempting to accelerate/merge into traffic, the bike quickly stalls again. This time the rider is placed in the middle of traffic, potentially causing a crash or risk of injury.On October 9, 2020, ODI received Honda?s Information Request (IR) response and learned that Honda was aware of 36 complaints, 87 field reports and 83 warranty claims of stalling related to clogged fuel filters. There were no claims of crashes or property damage associated with the alleged defect on the subject motorcycles. Additionally, Honda initiated three different modifications or changes to the fuel tank during production. On March 11, 2020, Honda released a customer satisfaction campaign to remove the zinc oxide that can contaminate the fuel pump filter, causing reduction of fuel flow. The part numbers for the fuel tank did not change after the implementation of the zinc oxide removal process. The process improvement was a running-change and no parts were withdrawn from production. On October 5, 2020 Honda added a new maximum contamination specification. This new specification was initiated to limit the contamination from the tank supplier. The tank supplier improved the cleaning process to meet the low contamination limit.Discussions and analysis of the data between ODI and Honda resulted in the manufacturer recalling certain 2018-2019 Africa Twin Adventure Sports (CRF1000L) and 2020 Africa Twin Adventure Sports (CRF1100L) motorcycles.Contaminants inside the fuel tank may cause the fuel filter to clog, resulting in an engine stall, and increasing the risk of a crash.ODI is closing this PE with Honda?s voluntary safety recall 20V-797. With the recall action taken by Honda, this investigation is closed, as further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other models or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.

Summary
On August 6th, 2020, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE20-012) to investigate alleged thermal events, and/or vehicle fires originating at the charge hub in 2014 Chrysler Town and Country minivans manufactured by Fiat Chrysler Automobiles (FCA). While the investigation was focused on the 2014 Chrysler Town and Country vehicles, the scope of the investigation was expanded to include the 2013-2020 Chrysler Town and Country, and Dodge Grand Caravan vehicles (the RT Platform).During the investigation ODI sent two separate Information Request letters to the manufacturer. One on August 17th, 2020 and one on March 31st, 2021.ODI identified a subject 2014 Chrysler Town and Country van on a vehicle salvage website that appeared to have experienced a thermal event where the charge hub is located. With the help of the NHTSA's Vehicle Research and Test Center (VRTC), ODI purchased the vehicle from the salvage company to inspect the damage and determine if more information about the failure could be obtained. A joint inspection with the manufacturer was conducted. Following this comprehensive inspection, a cause of the fire could not be determined.FCA initiated a parts collection initiative to obtain parts from the field for testing. The test results from the FCA field parts collection were inconclusive as they could not identify a common cause for the alleged thermal events. FCA looked at multiple liquid solutions that could have entered the charge hub circuit and caused it to short out, potentially leading to a fire. FCA also performed testing where the charge hub circuit was shorted out using a wire to observe if a flame or fire was produced. FCA provided their assessment to ODI concluding that none of the testing methods performed lead to a flame or fire being propagated.Given the low rate of thermal incidents at the subject vehicle charge hub, no reports of serious injury, zero reports of crash or death, and a lack of common failure mode, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if warranted by the circumstances.
