Summary
The Office of Defects Investigation (ODI) has received 221 complaints and several field reports alleging an issue with the Auto Start/Stop feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the Auto Start/Stop function engaged. Some of the complainants allege that a jump was required for the vehicle to restart. The subject vehicles are equipped with a 3.5L engine coupled with a 9-speed automatic transmission. To help improve fuel efficiency, the Pilot Touring and Elite vehicles are equipped with Auto Start/Stop capability. When the system is enabled by the driver and certain operating conditions are met, the Auto Start/Stop system will automatically shut off the engine when the vehicle comes to a complete stop. The engine is automatically restarted when the driver releases the brake pedal after a stop.ODI met with Honda on various occasions on this issue. Honda indicated that per the information provided from NHTSA complaints and TREAD (Transportation Recall Enhancement, Accountability and Documentation) reports, they have found a correlation with customers' allegations for the Auto Start/Stop failure to restart the vehicle when the system is activated. On April 10, 2022, the Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE) after receiving 5 consumer complaints alleging the Power Sliding Doors (PSD) auto-reverse function failed to work properly in the model year (MY) 2022 Kia Carnival minivan vehicles.At that time, ODI was aware of 3 complaints alleging injuries to consumer?s arm, hand or shoulder caused by the closing PSD.Additionally, consumers alleged being trapped against the closing door while it was traversing in the closing direction.The initial complaints also alleged excessive force is needed to reverse the direction of the closing door.At this time, ODI is aware of 26 complaints and 11 injuries on the MY 2022 Carnival vehicles.In its August 2022 responses to ODI's Information Request on the PE, Kia provided 36 customer complaints, 21 field reports, and 5 related warranty claims.Kia also provided ODI with 21 reports of alleged injuries, however, Kia stated that it was able to confirm only 9 injuries.Typically, these injuries are attributed to fingers and arms.The Kia Carnival automatic stop and reversal system is designed to automatically reverse the direction of the closing PSD if it meets resistance from an object or person.It relies on resistance sensing technology which requires a minimum input force or pressure to trigger activation.The primary sensing components used to regulate the auto-reverse system activation are the anti-pinch sensor and the PSD motor torque systems.Following several discussions with ODI during this investigation, on March 31, 2023, Kia filed a safety recall (NHTSA No. 23V-236) of 51,568 MY 2022 and 2023 Kia Carnival vehicles (produced through February 22, 2023) to reduce the risk of injury from the closing PSD.The remedy will be to reprogram the PSD control module with an updated software that provides two warning chimes when the door begins to close or open (to alert the vehicle users the door is about to move).Also, the updated software will change the door speed to move more slowly as the door approaches its latching point.In view of the recall action being taken by Kia, ODI is closing this PE.The agency reserves the right to take additional action if warranted by future circumstances.The ODI complaints cited above can be viewed at www.NHTSA.gov under the following ODI identification numbers: 11423165, 11442133, 11447955, 11448290, 11450964, 11458125, 11461462, 11466619, 11466623, 11468938, 11469661, 11471224, 11471232, 11472272, 11487649, 11487671, 11489378, 11489577, 11491053, 11498214, 11501337, 11507176, 11507880, 11509037, 11512506, and 11516428.

The Office of Defects Investigation (ODI) opened Recall Query RQ22-001 to evaluate the scope and remedy of Recall 22E-016. Recall 22E-016 addressed a defect causing the Automated Driving System (ADS) to shut down on a vehicle that was being operated by Pony.ai in a driverless mode. This condition resulted in a single vehicle crash on October 28, 2021. In the submitted recall report, Pony.ai identified the defect as a coding error. They also identified three (3) ADS units effected by the defect. Additionally, they stated that the relevant ADS code was revised to correct the error on the evening of the crash. Pony.ai uploaded the software to the affected units the following day. ODI sought more detailed information surrounding the failure and the failures of redundant ADS safety systems leading to the crash, and steps that Pony.ai has taken to prevent similar failures in the future. In their response, Pony.ai provided additional details regarding the origin and nature of the software error in the primary ADS unit which led to the incident and how Pony.ai became aware of the error. Pony.ai also provided details regarding errors which occurred in the installation of the redundant system installed in the incident vehicle, which led to the failure of that system. After thoroughly assessing the material submitted by Pony.ai, ODI believes that the company has properly identified both the error and the affected vehicles. The software upload completed immediately following the incident described above appears to have resolved the issue. Additionally, Pony.ai identified several process and procedural changes which they have either already undertaken, or plan to undertake, to prevent the recurrence of similar errors in the future. ODI is closing this Recall Query. NHTSA reserves the right to take additional action if warranted by new circumstances.

Summary
On March 29, 2021, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE 21-009) after the agency received 58 reports alleging engine stalling in 2013-2016MY Audi vehicle models A8, S6, S7, S8 & RS7 equipped with the 4.0L, twin-turbocharged engine. Complainants reported the vehicles would stall with no warning while driving at highway or city traffic speeds. Several of the complainants alleged an inability to restart. The complaints alleged the major contributing factor to the engine stalling was the failure of the turbochargers. The complaints cited the turbocharger failure was a result of oil starvation due to a clogged oil strainer in the turbocharger oil supply line.The location of the oil strainer in the turbocharger oil supply line is in the inner valley of the engine. It is not easily accessible, making servicing the component difficult. The oil strainer removes particles and carbon deposits from the oil before lubricating the turbochargers. Over time, if the debris is not removed from the oil strainer, it becomes clogged and prevents oil from flowing to the turbochargers. This causes the turbine bushings to run dry and overheat resulting in turbine shaft failure due to thermal stress.On April 04, 2022, PE21-009 was upgraded to an Engineering Analysis (EA22-001) to further assess the scope, frequency, and potential safety related consequences of the alleged defect. Volkswagen filed recall 22V-178 to replace the 30?m mesh oil strainer with a 90?m mesh oil strainer in the subject vehicles. VW introduced the larger mesh oil strainer into production in March of 2017 effecting the 2017-2018MY vehicles.ODI reviewed information from VW for vehicles that had the larger mesh installed in production. The data received showed, since the new oil strainer size of 90?m went into production for the 2017-2018MY, there has not been any reported incidents of turbocharger failures due to oil starvation or other failures that could be related to the larger screen mesh. In view of the recall being conducted by VW and lack of any detrimental side effect to the turbocharger caused by the larger mesh, ODI is closing this Engineering Analysis (EA).
