Summary
The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia America, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners' request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI's evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing. There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA's reasons for denial of the petition will be published. On August 8, 2022, the Office of Defects Investigation (ODI) opened this Recall Query (RQ) based on 3 incidents alleging front seat belt pretensioners that may rupture during deployment, on model year (MY) 2020-2022 of various Hyundai and Kia Models, resulting in 3 injuries (note: one incident occurred outside the U.S. and its territories).Between October 2021 and March 2023, Hyundai and Kia have filed the following 11 safety recalls for pretensioner rupture: NHTSA Recall No. 21V-796, 22V-069, 22V-123, 22V-127, 22V-218, 22V-354, 22V-447, 22V-458, 22V-816, 23V-094, and 23V-210.Hyundai/Kia have stated that the abnormal deployment of the pretensioners is due to over-pressurization of the pretensioner pipe that can cause metal fragments to enter the vehicle occupant compartment, which can result in injury to vehicle occupants.The various makes/models covered by the above recalls include approximately 405,900 certain 2020-2022 Hyundai Accent, 2021-2022 Hyundai Elantra and Elantra HEV, 2021-2022 Hyundai Santa Fe HEV, 2022-2023 Hyundai Santa Fe PHEV, 2020-2022 Hyundai Venue, 2021-2023 Genesis G80, 2023 Genesis G80 EV, 2023 Genesis G90, 2023 Genesis GV60 EV, 2022-2023 Genesis GV70, 2021-2023 Genesis GV80, 2021-2022 Kia Sorento HEV, and 2022 Kia Sorento PHEV.In response to ODI's investigation, in December 2022, Hyundai notified ODI that they would be conducting testing to validate subject vehicle seat belt pretensioners. The tests were intended to evaluate pretensioner deployment characteristics based on the unique deployment signature of the subject and peer vehicles. ODI met with representatives from Hyundai on several occasions during the investigation to discuss testing progress and data. As a result of the testing, Hyundai was able to simulate the pretensioner failures. It was determined by Hyundai during testing, the front driver-side and/or passenger-side seat belt pretensioners may explode upon deployment in the event of a crash which can cause metal fragments to enter the vehicle occupant compartment.Hyundai thus notified ODI of their decision to proceed with a safety recall, to expand the scope of the previous safety actions.In view of the recall actions taken by Hyundai and Kia, ODI concerns have been addressed. The agency reserves the right to take additional action if warranted by future circumstances.

On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash.On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. No failures of brake hoses on hybrid vehicles have been reported to ODI or Ford.Ford's actions address ODIs concerns at this time. Hybrid vehicles will be monitored for related failures. On July 22, 2022, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE22-006) to assess alleged failures of the rear-view camera (RVC) in Model Year (MY) 2020-2021 Cadillac XT5, XT6, and GMC Acadia vehicles. The complaints report that the screen for the RVC is black, sometimes with a red triangle and a circle with a line through it. To date, ODI has received a total of 18 complaints and several EWR Field Reports with allegations of the RVC image failing to display when the vehicle is in reverse. The reports have been received over the past 28 months with most occurring with low vehicle mileage.During the investigation, ODI learned incorrect crimping specifications were utilized in the production of the coaxial cable connector. Improper crimping leads to loss of connection between the RVC and dashboard screen that results in failure of the RVC image to display. Failure or malfunction of the RVC image to display results in reduced visibility of the area behind the vehicle. This limited view may lead to an increased risk of incidents resulting in injury and/or property damage while the vehicle is in reverse.The improperly crimped connectors were only present on subject vehicles equipped with the optional RVC surround vision feature (UV2). Vehicles not equipped with the optional RVC surround vision feature were not affected by this failure. On September 28, 2022, General Motors (GM) filed a defect information report (NHTSA number 22V-709) recalling certain MY2020-2021 Cadillac XT5, XT6, and GMC Acadia vehicles equipped with the optional RVC surround vision feature (UV2). GM will replace the coaxial cables with properly crimped connectors.In view of the recall action being taken by GM, ODI is closing this preliminary evaluation. Summary
The Office of Defects Investigation (ODI) received a petition dated March 16, 2022, requesting an investigation regarding allegations of defective intake valves in model year (MY) 2021 Ford Bronco vehicles equipped with the 2.7L Eco-boost ("Nano") engines. The petitioner alleged these vehicles are experiencing loss of motive power at any highway speeds with no-restart as a result of catastrophic engine failures. To date ODI is aware of 42 submitted Vehicle Owner Questionnaires (VOQ's) alleging a loss of motive power. On July 22, 2022, ODI opened a Preliminary Evaluation (PE22-007) to investigate allegations of stalling/loss of motive power on the subject vehicles. Upon receiving Ford's Information Request (IR) response, ODI learned Ford was aware of 110 consumer complaints (including field reports), 125 warranty claims and 136 engine exchanges related to the 2021 Ford Bronco. Ford's response indicated these failures are across the "Nano" engine family, which includes both the 2.7L and 3.0L EcoBoost engine variants with a total estimated population of 708,837 vehicles. Additionally, ODI became aware that the subject engines contained intake valves that were manufactured out of a specific alloy known as "Silchrome Lite", which can become excessively hard and brittle if an over-temperature condition occurs during machining of the component. The overly brittle intake valve can fracture at the keeper grove causing the intake valve to freely contact the piston, resulting in catastrophic engine failure. A design modification was implemented in October 2021, which changed the intake valve material to a different alloy known as "Silchrome 1", that is less susceptible to over-temperature during machine grinding. ODI is upgrading this PE to an Engineering Analysis (EA23-002) to better understand the failure mechanism identified by Ford and to ascertain the complete scope.
