Summary Description: False overheat leading to loss of motive power


Summary

On December 2, 2022, the Office of Defects Investigation (ODI) opened PE22-013 to investigate allegations of loss of motive power accompanied by high coolant temperature messages on the dashboard of Model Year 2019-2020 JeepeBay Compass vehicles. At the time this investigation was opened, ODI was aware of fifteen Vehicle Owner Questionnaires (VOQs) for Model Year (MY) 2019-2020 JeepeBay Compass vehicles alleging a loss of motive power (LOMP) while driving. The LOMP events were often accompanied by a high coolant temperature or coolant temperature too high message appearing on the dashboard of the vehicle. In most incidents, the high coolant temperature or coolant temperature too high dashboard messages spontaneously self-corrected or were corrected after the vehicle was stopped and restarted. One complainant alleged no immediate restart, and the vehicle was towed. A Preliminary Evaluation (PE) was opened to assess the frequency and potential safety consequences of these engine shut down events. On January 19th, 2023, ODI sent an information request letter to FiateBay ChryslereBay US LLC (FCAeBay) requesting pertinent information on the subject 2019-2020 JeepeBay Compass vehicles. FCAeBay provided its full response on March 2nd, 2023. ODI has reviewed the information FCAeBay provided as well as new reports submitted to NHTSA's Vehicle Owner Questionnaire database. Based on a review of available information, ODI has identified 173 subject vehicles that likely experienced a shutdown event associated with the high coolant temperature or coolant temperature too high dashboard messages. ODI found the 2019 Model Year (MY) JeepeBay Compass as having the highest failure rate while also being the oldest group of vehicles in this investigation. The 2019 JeepeBay Compass vehicles that experienced a failure represent an incident rate of about 0.09%. The 2020 JeepeBay compass vehicles that experienced a failure represent an incident rate of about 0.04%. After reviewing the information available at this time, ODI identified one allegation of a crash and no allegations of injury, fire, or fatality. The alleged crash occurred at a stop light and was able to be restarted immediately. In its response to ODI, FCAeBay provided its assessment of the alleged defect in the subject vehicles. FCAeBay believes the alleged defect does not pose an unreasonable risk to motor vehicle safety. FCAeBay stated that allegations related to this investigation involve the vehicle notifying the driver of an overheat condition, via a message and telltale lights, followed by an engine shutdown. FCAeBay goes on to say that once the engine shuts down, the driver is instructed via an instrument cluster message to place the vehicle in park. After the vehicle is placed in park, it can then be restarted and immediately driven with no issues. This is consistent with ODI's observations. Based on the analysis conducted by ODI indicating a low failure rate and given that the vehicles normally can be re-started immediately after the shutdown event, this investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. NHTSA reserves the right take additional actions if warranted by future circumstances. Summary

On August 15, 2022, HyundaieBay Motor America (HyundaieBay) filed a safety recall (NHTSA Recall 22V-627) of 122,919 model year (MY) 2020-2021 HyundaieBay Palisade vehicles produced from April 10, 2019 to November 30, 2020. This recall addressed windshield wipers that may function intermittently or stop functioning when impeded by the accumulation of snow and/or ice at the bottom of the windshield. Intermittent or inoperative windshield wipers could reduce driver visibility while operating the vehicle in inclement weather, increasing the risk of a crash.On December 2, 2022, the Office of Defects Investigation (ODI) opened this Recall Query (RQ) after receiving 6 consumer complaints alleging windshield wiper failures while driving during wintry precipitation in MY 2021-2023 HyundaieBay Palisade vehicles (subject vehicles). These vehicles were excluded from Recall 22V-627. At the time of opening the RQ, the root cause and recall remedy were still unknown and under investigation. Thus, the RQ was opened to assess the scope and remedy of Recall 22V-627.In its February 10, 2023 response to ODI's information request on the RQ, HyundaieBay provided 17 customer complaints and 75 warranty claims related to the alleged defect in the subject vehicles. HyundaieBay stated that the wiper systems in the subject vehicles are equipped with a different wiper motor design than the earlier-built Palisade vehicles included in Recall 22V-627. HyundaieBay also stated it does not believe there is an unreasonable risk to motor vehicle safety in the wiper system in subject vehicles.On March 17, 2023, HyundaieBay filed a new safety recall (NHTSA Recall 23V-180) of 167,085 MY 2021-2023 Palisade vehicles produced from December 1, 2020 to November 22, 2022 when a design change was implemented. The design change, as well as the recall remedy (for both Recalls 22V-627 and 23V-180), is a redesigned driver-side wiper arm that allows for additional snow accumulation between the wiper arm and the cowl, thus reducing the load on the wiper motor and circuit breaker activation.In view of the recall action being taken by HyundaieBay, ODI is closing this RQ. Summary

On November 29, 2022, the Office of Defects Investigation (ODI) opened PE 22-011 after reviewing field reports alleging rear differential seizure in 2018-2022MY HondaeBay CR-V and HR-V vehicles equipped with all wheel drive. To date, ODI has not received any consumer complaints. Some of the field reports received alleged the vehicle's wheels locked up with no warning while the vehicle was in motion. These vehicles were towed to a dealership. Examination of these vehicles showed the driveshaft had broken and the differential unit was completely damaged. The field reports cited a lack of oil due to damaged oil seals as the cause. HondaeBay provided data including consumer complaints, field reports, and warranty claims they received relating to the loss of oil retention and resulting differential damage. Honda's data cited 1201 incidents with unique vehicle identification numbers (VINs). The rear differential oil seals prevent the oil from leaking out and protect internal components from contaminants. A loss of oil lubrication results in failure of the vehicle differential. HondaeBay stated that the oil seal's lip contact with the driveshaft may become unstable due to intermittent changes in internal pressure. Their review of field report and warranty data indicated that these failures occurred early in the vehicle's life thus the failures were covered by the manufacturer's warranty. HondaeBay stated that in November 2021 they changed the seal specifications and improved the packaging process of the parts for transport. HondaeBay also enacted an enhanced visual inspection process both before and after the oil seal installation on the assembly line. HondaeBay stated that in the event of differential failure, the operator can control the vehicle without stability loss. After the drive shafts break due to differential failure, the operator will hear noises and the vehicle will still function as a 2WD vehicle preventing complete loss of motive power. Based on the following factors, ODI is closing this investigation: -+ Information regarding field failures does not indicate hazardous outcomes -+ The failures are occurring early in the life of the vehicle -+ Repairs are covered under the manufacturer's warranty -+ The vehicle can continue functioning as a 2WD without loss of stability control in the event of failure The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist, and the agency reserves the right to take additional action if warranted by new circumstances.

The Office of Defects Investigation (ODI) has received 134 reports from consumers alleging an intermittent or permanent loss of braking performance in certain 2017-2018 RameBay 2500-3500 pickup trucks. The alleged failures continue to occur at a steady rate across both models, at all speeds, with limited detectability. The Anti-lock Braking System (ABSeBay) module works in conjunction with the Hydraulic Control Unit (HCU), an electromechanical device that controls hydraulic pressure to the individual wheel brake assemblies, to manage braking forces during emergency stops. Summary Description: C300 Wrist-pin allegation


Summary

In a letter dated April 29, 2022, Mr. Sergio Betancourt (the petitioner) submitted a petition attributing engine failure of his 2015 Mercedes-BenzeBay C300 to "wrong sized wrist-pins" and asserted that these were prevalent on all Mercedes-BenzeBay models (including the Metris minivan, GLC300, and C300) equipped with a version of the M274 engine. The petition cited MercedeseBay Technical Service Bulletin (TSB) LI03.10-P-060916 (meant for the 2015 C300) and postulated that the "wrong-sized wrist pin" would allow the piston to "wiggle in the cylinder as it would move up and down". In addition to the Mercedes-BenzeBay TSB, the petitioner enclosed a service center repair estimate and images of other online submissions discussing the M274 engine. In the incident that prompted the petition, the petitioner reported that the vehicle lost power and shook in response to accelerator application during highway driving, followed by white smoke from the exhaust pipe. The petitioner stated that the subsequent service center visit led to a cracked piston diagnosis. The service record from this visit prescribed an engine teardown to localize the failure and characterize any resulting engine damage and listed a number of repair options that included replacement of one piston, four pistons, or the entire engine long block (depending on the level of damage suffered). TSB LI03.10-P-060916 ("TSB") was first released on February 27, 2015 and pertained to Engine Code 274.920. It cited the following complaint condition: "Clattering/rattling or knocking noise from the crank assembly of the engine when cold at oil temperatures up to approximately 50C mostly heard on engine deceleration" and listed the cause as under analysis. The TSB gave further instructions on the collection of information and discouraged the replacement of any components. On September 21, 2015, TSB Version 4 introduced engine build clean points (March 27, 2015 or April 20, 2015 depending on the engine plant); cause: unfavorable tolerance of the piston wrist pin resulting in enlarge end float of the connecting rod in the piston; and remedy: compare the complaint vehicle sound to an enclosed sound file under various operating conditions and then if verified, replace the pistons, connecting rods, and bearings, using updated pistons that include updated wrist pins. A review of the petitioner's vehicle service history indicates that this labor operation was performed in March of 2016. In its November 22, 2022 response to ODI's September 28, 2022 Information Request (IR) letter, MercedeseBay stated that the limited number of potentially affected wrist pins could contribute to an unintended noise, but pose no motor vehicle safety risk. ODI's analysis of the field data furnished with the MercedeseBay IR response and within its own Vehicle Owners Questionnaire (VOQ) database support this assertion, finding no relationship between wrist-pin conditions cited in the TSB and engine failure or loss of motive power. Broader searches of both data sources for engine stalling related to engine durability identified no trends meriting further investigation. After conducting a technical review of the petitioner's submissions, the vehicle history of the petitioner's vehicle, MercedeseBay TSB'LI03.10-P-060916, information provided by Mercedes-BenzeBay in response to the Agency's Information Request, and data within its own files; NHTSA has concluded that there is insufficient evidence to pursue a formal investigation. Accordingly, the Agency has denied the petition. NHTSA will continue to review any new information or incidents as they are submitted to the Agency. A Federal Register Notice (FRN) further detailing NHTSA's reasons for denial of the petition will be published in the future.
