After a preliminary review of consumer complaints and other information related to alleged defects, NHTSA obtains information from the manufacturer(including data on complaints, crashes, injuries, warranty claims, modifications, and part sales) and determines whether further analysis is warranted.  If warranted, the investigator will conduct a more detailed and complete analysis of the character and scope of the alleged defect.

Summary Description: Inadvertent Air Bag Deployment


Summary

Based on analysis of ChryslereBay's response, and including ODI's consumer complaints, ODI has identified 87 reports of air bag inadvertent deployment incidents resulting in 50 alleged injuries.The injuries consists of burn/cuts and bruises to the upper body region.Forty-two (42) of the 87 incidents involved the driver frontal air bag, and the remaining 45 incidents involved both the driver and passenger frontal air bags inadvertently deploying without a crash.These incidents occurred both at vehicle startup or while the vehicle is being operated on the roadway.Some owners noted that the air bag warning light had illuminated, or had intermittently illuminated, and that this occurred just prior to the air bag deployment, while others stated they did not observe any illumination of the air bag light. Inspection of the Occupant Restraint Control (ORC) module conducted by ChryslereBay indicates that an internal electronic circuit (an ASIC chip) which controls air bag deployment fails due to an electrical over stress (EOS) condition possibly caused by a transient voltage spike.ChryslereBay is in the process of identifying potential root causes of the component failure and/or a source for the electrical over stress.In addition, ChryslereBay noted that there have been no reports of inadvertent air bag deployment in vehicles manufactured after a March 19, 2003 production date.However, ChryslereBay had not identified a component or other change that would explain why the inadvertent deployments ceased. Summary

In a letter dated March 2, 2012 ChryslereBay Group LLC (ChryslereBay) submitted a Defect Information Report (NHTSA recall No. 12V-085) to the NHTSA identifying a safety defect regarding rear suspension lower control arm corrosion in approximately 210,000 model year (MY) 2004 and 2005 JeepeBay Liberty vehicles, originally sold or currently registered in a Salt-belt state (Connecticut, Delaware, Iowa, Illinois, Indiana, Kentucky, Massachusetts, Maine, Maryland, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, Wisconsin and the District of Columbia).According to ChryslereBay, excessive corrosion could result in fracture of a rear suspension lower control arm which may result in a loss vehicle control.In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to ChryslereBay, ODI identified 54 non-duplicative reports alleging failure of one or both rear lower control arms due to excessive corrosion.Thirteen of these complaints allege that the failure resulted in some loss of vehicle control.For purposes of this analysis, loss of control events involve alleged complete or partial departure from the intended lane of travel.An additional 11 complaints reported difficulty maintaining control of the vehicle when the rear lower control arm failed.All of the complainant vehicles were either originally sold or registered at the time of the incident in a Salt-belt state.ODI did not identify any reports (complaints or warranty claims) related to rear lower control failure due to excessive corrosion in the approximately 138,000 MY 2004 and 2005 Liberty vehicles that are not included in this recall (i.e., not sold or currently registered in a Salt-belt state).In ChryslereBay?s Defect Information Report, ChryslereBay stated that during the investigation a field survey of MY 2004 and 2005 JeepeBay Liberty vehicles was conducted.In total, ChryslereBay is aware of 83 incidents alleging rear control arm fracture (including complaints and field study results).Based on ChryslereBay?s decision to conduct a safety recall this investigation is closed. Summary Description: Service Jack Failure


Summary

The subject vehicles (SV) are all automatic transmission, front wheel drive vehicles with hand actuated mechanical park brakes. The investigation focused on allegations that the service jack failed and caused the vehicles to fall. The subject scissors-type jack appears to fail at the slotted hinge joints where the jack screw is located (bends at mid-point). The failures occur while the jack is in use, and vehicle movement (forward or backward) is typically reported coincident with the failure. FordeBay reported that most MY 2003 Windstar and all MY 2006-2007 Freestar/Monterey also use the subject jack, an additional 223,246 vehicles.NHTSA has received six consumer complaints (VOQs) involving three injury incidents and a fatality.FordeBay reported 26 complaints for the SVs involving an additional 4 injuries. In the fatal incident, the van fell on a person working on the engine under the otherwise unsupported vehicle; ODI was unable to determine whether the vehicle was adequately chocked or the park brake was properly set. Reported injuries include a broken arm, head and hand injuries, arm and rib bruises, and muscle strains. NHTSA has received just one new report since opening this investigation. The complaint trend in ODI and FordeBay data is flat if not declining.FordeBay provides instructions and warnings in the owner's manual, on a placard placed with the jack, and on the jack itself. These instruct consumers to put the vehicle gear selector in Park, properly set the parking brake, chock the opposite wheel, use the jack only for changing a tire, loosen the lug nuts prior to jacking, and to not get under a vehicle supported by the jack only (NHTSA also recommends that jacks only be used for tire changing).In the VOQs, roughly half the consumers were using the jack for something other than tire changing, half had not chocked a wheel, and half had not properly set the parking brake. ODI notes that since the front wheels are drive wheels (with an open center differential) the Park gear cannot provide vehicle stability (longitudinally) when a front wheel is raised. All NHTSA complaints and most of the complaints to FordeBay involved failures while a front wheel was raised.In response to ODI's inquiry, FordeBay conducted testing on a 2004 Freestar. FordeBay's tests showed that when a front tire was raised the service jack held without failing under various scenarios except when a rocking force was applied and the parking brake was not properly set. Based on its testing and its analysis of field data, FordeBay concluded that the type of jack failure evidenced in this investigation (bending at mid-point) was likely caused by vehicle movement due to an improperly secured vehicle, or other misuse of the jack. FordeBay notes that rates in this investigation were similar to those in another investigation (EA91-049) which ODI closed. ODI also analyzed jack data for MY 2003 and newer vehicles to compare to the SV population. To assist with this analysis, ODI requested data from the jack manufacturer (actually its successor company) who provided limited data but did identify other models using the same design jack. ODI was unable to identify a significant difference in the incidence of jack failures (including those involving bending failures) between the SVs, other vehicles which used the same jack, or other vehicles where the jack type was unknown or different.A safety-related defect has not been identified at this time and further use of agency resources does not appear to be warranted.Accordingly, investigation PE11-033 is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist and ODI reserves

Summary

This investigation was prompted by Preliminary Evaluation (PE) 11-017, and was based on reports from consumers alleging that both low beam headlights stopped illuminating simultaneously.Some complainants also reported that the combination switch (switch) that controls the headlights and turn signals, and/or its electrical wiring harness connector were damaged by overheating.ODI's PE information request letter asked HondaeBay for certain data on model years (MY) 2002-2006 CR-V, 2003-2008 Pilot, 2003-2008 Element, 2001-2005 Civic, and 2000-2001 S 2000 vehicles.Analysis of complaint and HondaeBay data show that a complete low beam headlight circuit failure occurs in models with 2-bulb headlight systems (i.e., a single bulb on each side for high and low beam).Models with 2-bulb systems include MY 2002-2004 CR-V, 2003-2005 Pilot, 2001-2003 Civic, and 2003-2008 Element.Models with 4-bulb headlight systems (i.e., two bulbs on each side, one for high and one for low beam) have different electrical circuits for the headlights and do not experience the same failure mechanism as the 2-bulb system.MY 2000-2001 S 2000 models with 2-bulb high intensity discharge headlights also have different electrical circuits for the headlights and do not experience the same failure mechanism.HondaeBay introduced the 4-bulb headlight system in CR-V in MY 2005, in Pilot in MY 2006, and in Civic in MY 2004.On March 29, 2012 HondaeBay submitted a defect information report (DIR) to recall 554,428 vehicles, including MY 2002-2004 CR-V and MY 2003 Pilot.In the DIR, HondaeBay stated that tension from the combination switch wiring harness to the electrical terminal for the low beam headlight circuit could cause a small amount of motion as the combination switch was operated.This motion, over time, can wear the surface of the terminal creating copper oxide, increasing electrical resistance, which may cause the terminal to melt within the harness connector.ODI continued its investigation, and on August 31, 2012 HondaeBay submitted another DIR to recall an additional 820,789 vehicles.This report included the MY 2002 Civic vehicles which were not subject to a prior safety recall (04V-086) for headlights, all of MY 2003 Civic, and MY 2004-2005 Pilot.HondaeBay's DIR advised that operation of the turn signal switch lever at high speed and/or with heavy force can cause movement between the combination switch body and the sub-harness wiring connector.This can cause wear and continued movement can create copper oxide, increasing electrical resistance, and potentially causing the terminal to melt within the harness connector. ODI received a complaint regarding a corroded battery post located behind a service panel near the driver's seat on a 2006 Thomas Built C2 school bus that resulted in a fire on the vehicle.It was determined that like buses built prior to a December 26, 2005 design change which added a flange to the lower edge of the panel were susceptible to water intrusion.The design change was meant to address concerns of loose panels but consequently prevented water ingress and the subject corrosion. Daimler TruckseBay North America (DTNA) submitted a recall notice on October 18, 2011 involving 3,300 of the estimated 6,139 vehicles subject to this investigation.In subsequent communication with ODI, DTNA explained that nearly two thirds of 4,773 MY2007 vehicles were built with the December 26 design change and therefore do not contain the safety related defect.DTNA's announcement of a safety recall on this matter, and subsequent explanation of the scope of vehicles involved, addresses ODI's concerns.The 573 letter was received prior to the due date of the IR response, therefore DTNA has been relieved from having to submit an IR response.DTNA will inspect, clean, repair and protect power cables as needed.The subject vehicles have been recalled and this investigation is closed.The ODI report cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification (ODI) number: 10395455
