The Office of Defects Investigation (ODI) received a complaint alleging brake fluid loss caused by a ruptured brake line that had chafed against a wire harness in the engine compartment of the complainant?s vehicle.It was determined that the chafing was caused by contact between the convoluted tubing covering the fuel supply line and the metal brake lines.This issue only affects units with a Caterpillar
(CAT) C7 engine and a fuel water separator mounted in a specific location on the frame rail.Not all units with CAT engines were equipped with fuel water separators, but all units with CAT engines and a fuel water separator have them mounted in the same location.Units with other engines have a different fuel supply line routing and are not affected by this defect.Daimler Trucks

ODI opened a Preliminary Evaluation (PE11-007) based on one fleet report alleging it had experienced multiple wheel-end fires involving the subject vehicles.The fleet believed the fires were due to a power spring fracturing and puncturing a brake chamber diaphragm resulting in a loss of compressed air to the brakes.The loss of air was believed to cause the brakes to drag and overheat.ODI upgraded PE11-007 to this engineering analysis to further evaluate the cause of the wheel fires and assess the scope, frequency, and safety consequences of the alleged defect.After a comprehensive review of the data related to brake chamber failures or replacements on the subject vehicles, ODI did not identify the existence of a safety related defect trend.Of the two vehicle fires initially reported, only one was subjected to a fire investigation.The fire investigator in that case could not identify the component responsible for the vehicle fire.Driver statements reviewed from one fleet which experienced two brake overheating events indicated that the vehicles had not been operating at normal air system pressure levels prior to the events.In addition, the failed or replaced chambers had been in service on average nearly 50 months.ODI reviewed 477 warranty claims and fleet reports of brake chamber failure or replacement.From the data review, three (3) brake overheating incidents involving a subject size 24/30 failed brake chamber were identified.In reviewing the time in service for the overheating incidents, it was found that vehicles had an average of 50 months in service.This time in service represents a typical service life wear out.The injuries reported in the opening resume for this investigation are no longer being considered as having resulted from the alleged defect in the subject brake chambers.In the case of the smoke inhalation originally reported, the fire was not determined to have been caused by a failed brake chamber.In the case of the injuries that resulted while replacing a subject brake chamber, it was found that these chambers have a limited service life and are subject to wear out.As a result, brake chambers are replaced frequently and standard safety precautions should be followed during this maintenance procedure that would prevent any chamber being replaced from experiencing an uncontrolled release of the compressed spring.DTNA has informed NHTSA that it will take the following action to further reduce the likelihood of subject component failures.DTNA will offer owners of subject vehicles, equipped with the original size 211/212 chambers, a free replacement of the chambers.The chambers will be replaced with the more robust 003/004 sealed chambers.In light of the low rate of wheel end fires resulting from a premature failure of the power spring in the subject vehicles, and DTNA's actions to reduce the likelihood of a failed brake chamber, further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency reserves the right to take further action if warranted by the circumstances.For additional information, see the attached closing report.

Braun Vista 2 and Century 2 wheelchair lifts may allow unintended wheelchair roll-away to occur when the outer barrier system fails to latch.This failure mode is alleged to have allowed an occupied, powered wheelchair, weighing 417.3 lbs (the lift is rated to 800 lb.) to roll off an elevated lift resulting in serious injuries to the chair occupant.To address outer rollstop latching issues going forward, Braun modified the subject lifts latching mechanism...a modification introduced into production on 8-24-10.To correct those lifts already in use, Braun issued two service bulletins (36218 and 36359 in April 2010 and June 2010, respectively) and one modified latch kit.Braun has now decided to recall the subject lifts covered by the two bulletins.With that safety-related recall (12E-002) this PE is closed.

The subject vehicle (SV) population includes MY 2005-2006 vehicles built between March 2004 and December 2005 manufactured with a TRW supplied headlight switch assembly (HSA).The HSA was the primary focus of the investigation; earlier and later vehicles utilized different HSAs.Consumers complain that the headlights in the subject vehicles do not illuminate when the HSA is turned on, or that the headlights extinguish while on (subject failure).No other driving lights are affected.The front marker, tail, and instrument lamps operate.Also, the high beams can be illuminated by holding the turn signal stalk in the up position.As reported by consumers, turning the HSA off and on typically corrects the failure.Initially the failures occur infrequently but they become progressively more frequent with age.Chrysler
has indicated its intention to offer of an extended lifetime warranty on the HSA, so that consumers with this problem can have their HSA replaced at no charge when they begin experiencing the subject failure.Commencing sometime in early 2013, Chrysler
will send letters to SV owners notifying them of the lifetime warranty.The SV headlight system design is essentially the same as that described in EA 05-009, which involved headlight anomalies in earlier model Chrysler
minivans; refer to it to for additional information.The subject failure is caused by friction between the plastic cam and metal surfaces of the electrical contacts in the HSA, which results in the production of plastic particles (debris).Analysis shows that debris can collect on the electrical contact that controls the headlights, resulting in an open circuit and preventing the headlights from turning on.Subsequent operation of the HSA may temporarily displace the debris, at which time the headlights operate correctly.Lubricant was applied to the plastic cam beginning in December 2005, reducing debris production.Consequently, subsequent failure levels were lower.The report counts shown above include only those that involve the subject failure.Reports involving other failure modes which were identified have not been included either because they were not safety related, or if they were safety related, because there was no defect trend.The single crash event listed in the Failure Report Summary Table above involved a deer strike allegedly due to loss of headlights in February 2006.No injuries were reported in this crash.An April 2004 non-crash incident (?Other? above) involved a momentary loss of headlights.The consumer drove into a ditch, and one minor injury was alleged. The subject vehicles experienced a 2.2% warranty claim rate.While standing alone this would appear to be significant, a portion of these warranty claims likely involve other problems with the HSA unrelated to the subject failure.In view of the on road experience of drivers in rapidly restoring headlight illumination by turning the headlight switch off and on again, the rate and particularly the recent trend in problems with the switch, and Chrysler

On August 29, 2011 ODI received a Vehicle Owner's Questionnaire (VOQ) from a Los Angeles school district indicating a 2003 Thomas Built Saf-T-Liner HDX compressed natural gas (CNG) school bus in their fleet had insufficient clearance between the CNG tank and mounting bracket resulting in damage to the outer tank wrapping.The Mathematical Analysis Division of NHTSA calculated that a sample size of thirty (30) buses would give a high confidence (90%) estimation of whether similar buses were experiencing tank abrasion.In the course of this investigation nearly 70 buses (over 300 tanks) were inspected; no tank other than the subject of the VOQ was found to have insufficient clearance.Busses manufactured before and after the subject vehicle were inspected.As a result of this investigation, Daimler Trucks North America (DTNA) developed an improved inspection procedure that addresses the concerns identified in the Los Angeles complaint.In early February 2012 DTNA will mail an owner letter along with the new inspection procedure.See the investigative file for copies of DTNA's bulletin and owner letter.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency reserves the right to take further action if warranted by circumstances.The ODI report cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification (ODI) number:10423400
