Hood striker fasteners failed allowing hood to separate from striker at highway speed resulting in hood flying open, striking and damaging windshield and blocking view of driver. Photos from incident scene and subsequent dealer inspection agree on fastener failure as the striker was found fully latched into the retainers. Fortunately, driver was in an outer lane and able to navigate to shoulder and stop vehicle without further damage.

Summary

On September 13, 2026, my 2023 Tesla Model Y (13,300 miles) displayed a High Voltage System alert stating the vehicle may not restart and service was required. I brought the vehicle to the Tesla Service Center for diagnosis. The technician found the alert occurred when the brake pedal was applied and identified electromagnetic interference affecting signal integrity in that circuit. On September 17, 2026, a ferrite suppressor was installed to address the interference, and the technician noted the alert cleared. He advised that if the concern returned, replacement of the CHMSL (Center High Mount Stop Lamp) may be required. The same day I picked up the vehicle after this repair, the High Voltage System alert recurred. Both occurrences happened while the vehicle was operating under Full Self-Driving (FSD), not under manual braking. My foot was not on the brake pedal either time. This detail was not accounted for in the original diagnosis, which was based on manual pedal actuation as the trigger. I have raised this discrepancy with Tesla and requested a re-diagnosis based on FSD-commanded brake actuation, including review of vehicle logs for both incident timestamps. My safety concern is that the alert explicitly warns the vehicle may not restart, and the underlying cause has not yet been correctly identified, since the initial repair did not resolve the issue and failed the same day. The vehicle and its diagnostic logs are available for inspection upon request. The problem has been reproduced and confirmed by the Tesla Service Center (two visits, same fault). It has not been inspected by NHTSA, police, or insurance. No warning lamps or symptoms were noted prior to the first occurrence on September 13, 2026.

Summary

Safety complaint description In August 2026, I experienced a potentially dangerous incident involving my 2023 PolestareBay logo 2 AWD Performance while performing a U-turn. During the maneuver, the vehicle unexpectedly stopped or hesitated and did not move forward when I expected it to, leaving me exposed in the roadway and creating a risk of a collision with approaching traffic. I do not know what caused this behavior. I am concerned that it may involve an unexpected interruption of vehicle propulsion, a driver-assistance system or another electronic or mechanical malfunction. I have not received a diagnosis confirming the cause. I have also experienced other concerning vehicle symptoms, including clicking, popping and creaking noises during low-speed turns, a clunk from the front-left area and a tendency for the vehicle to pull slightly to the left despite equalized tire pressures. I do not know whether these symptoms are related to the U-turn incident. I reported these concerns in August 2026 and scheduled the earliest available service appointment at Dyer & Dyer, the authorized PolestareBay logo service location in Georgia, for September 18, 2026. I delivered the vehicle for that appointment on September 18. On September 22, I was informed that the vehicle had not yet been inspected or diagnosed and that the next anticipated opportunity for an initial inspection was October 13, 2026, unless an earlier opening became available. As of September 22, 2026, the cause of the unexpected vehicle behavior has not been identified, and I have not received confirmation that the vehicle is safe to operate. I am submitting this complaint because the unexpected failure to respond during a U-turn created a potentially serious traffic hazard. I am requesting that NHTSA document and evaluate this incident as a possible vehicle safety defect.

Summary

I am filing this formal complaint regarding a severe, recurring, and catastrophic electrical and vehicle control system failure in my 2017 Chevrolet Trax. On multiple occasions while driving at highway speeds (70+ mph), the vehicle experienced simultaneous loss of critical safety and control systems—including power steering, traction control, and instrumentation displays—plunging the vehicle into a severe "limp mode." This sudden and unpredictable loss of control at high speeds presents an extreme safety hazard to myself, my passengers, and other drivers. CHRONOLOGY OF EVENTS & DEFECT PATTERN 1. Initial Intermittent Failures (Past 12 Months): The traction control light began activating intermittently and randomly without cause. A dealership reset temporarily cleared the issue, but it later returned. 2. Sensor System Failures (Approx. March 2026): All four Tire Pressure Monitoring System (TPMS) sensors began failing sequentially until complete loss of tire pressure telemetry across the dashboard. 3. First Major Highway Power Failure (July 2026): While traveling at 70 mph on the highway, the vehicle suffered a complete system collapse: Simultaneous loss of Power Steering (requiring extreme manual steering effort at high speeds) and Traction Control. Check Engine Light illuminated. Instrumental cluster failed completely: digital tachometer and speedometer registered 0 RPM and 0 MPH while actively traveling at speed. Upon pulling off the highway, the vehicle entered a limp state, restricting speed to under 10 mph. After shutting off the engine, the vehicle's dashboard screen went entirely black/unresponsive upon re-attempting ignition. After sitting stationary for an extended period, the vehicle restarted, though the Check Engine Light remained illuminated.
