Summary
On almost all Mustang Mach-E vehicles, a loose or faulty cable connection which runs to the forward facing camera in the rear-view mirror housing causes error messages to suddenly display on the dashboard screen. Generally, the driver will receive a "Front Camera Fault – Service Required" error message, in addition to a "Pre-Collision Assist Not Available" error message, also followed by an error message indicating that lane centering is also no longer functioning. All of this occurs after the vehicle travels over a harsh imperfection/bump in the road pavement, or in my case, every time I drive over railroad track crossings. I have seen so many reports of owners receiving this exact same issue, and everyone says the same thing: when this issue begins to occur, the vehicle will disable all of these safety features for about 30 seconds or less, and then all of the systems will reactivate like normal. Many owners state that eventually these errors become permanent, which causes all of these safety systems in the vehicle to become PERMANENTLY disabled, until the vehicle is brought into a dealership service center. If the vehicle is still within the 3-year/36,000 mi factory warranty, Ford will replace the coaxial cable running to the camera at no charge to the vehicle owner. However, as many owners have stated, if their vehicle is beyond the factory warranty, the vehicle owner usually is forced to pay for the repair out of pocket, which seems to range anywhere between $1,000 and $4,000. The reason I am reporting this matter is because I believe this is a major safety concern. When the front camera fault appears on the driver's display, the vehicle automatically disables many of the safety systems, including the ability for the vehicle to avoid a collision by bringing the vehicle to a stop. Because this is a major safety concern, I believe that NO Mustang Mach-E owner should be forced to pay for this repair out of pocket, and I believe that this should be a recall.

My vehicle was moving slowly in a pickup line for my child. Randomly, the seat belt retractor went off and pulled me back into the seat. Initially, I thought it was a gunshot. Smoke was all in the car. My neck/shoulder has some bruising and cuts from the seat belt. There was a seatbelt light on the dash. It has been there for some time, but the recall's hasn't been issued and it's not realistic to not have a car until FoMoCo issues the fix.

The vehicle has suffered recurring, critical electronic and low-voltage safety system failures that first began under the original factory warranty and have now resulted in a complete central computer (HW4) failure. History of Defect: 1. Feb 2025 (~7,170 miles): Initial failure of the 16V low-voltage network and safety systems. Tesla replaced the VCFRONT body controller and low-voltage battery under warranty. 2. May 2025 (~14,543 miles): Safety equipment failures required replacement of the cabin radar and seat belt buckles under warranty. 3. Sept 2026 (~80,000 miles): Total Hardware 4 (HW4) central computer failure. This resulted in the complete loss of all Active Safety Systems, Forward Collision Warning, Automatic Emergency Braking, Autopilot, backup cameras, and power management features. Safety Risk: The complete failure of the HW4 computer completely disables essential safety functions required for safe vehicle operation on public highways. Tesla has denied warranty coverage for the HW4 replacement despite the issue originating from early low-voltage controller failures during the bumper-to-bumper warranty period. ADDITIONAL CRITICAL SAFETY INCIDENT: During the computer/system failure, I experienced a power steering malfunction accompanied by an on-screen warning. The steering wheel became extremely hard to turn/stiff, making it nearly impossible to steer the vehicle safely.

While driving, the dashboard very quickly flashed the words "Charging System Service Soon" so fast I almost couldn't read it. Normally messages remain on the screen until I acknowledge them, but this was not the case. This happened at least 3 times within 30 minutes. When I got home, I parked and turned off the car. When I needed to turn the car back on shortly after, the vehicle would not turn on. I was able to get it jump started and immediately drove it to the Ford Service Center. The tech notes indicated: "Found SSM for concern. Required some module updates. During update, the PCM bricked due to low power. Through nearly 7 hours of testing, found the soldered connection of the main battery cable from battery to battery junction box is no longer connected and has caused a short circuit through the whole harness. There was a recall that expired in 2024 that was for this exact concern. (22B39) Recommendation is to replace full wiring harness and verify all modules are programmed properly. Pending customer approval.”" This is a safety concern in the sense that the disconnected battery cable could have led to the vehicle losing power while driving. Losing the ability to control the vehicle in this manner is a serious danger to me and other drivers, and I am grateful I was not driving when it lost power due to the disconnection.

Summary
The infotainment touch screen on my 2023 Volkswagen Taos has completely blacked out and failed, leaving my backup camera entirely inoperable. This has created a severe blind-spot hazard where I cannot safely reverse. On top of this failure, the vehicle has a chronic history of being in and out of the dealership service department for unresolved fuel system problems, specifically regarding the fuel injectors and fuel lines under warranty. The dealership has repeatedly failed to permanently fix this fuel defect. Now, the car actively loses engine acceleration and drops into a dangerous slow crawl ('limp mode') at busy intersections when the air conditioning is running, forcing me to turn off the climate control just to move safely. Combined, these recurring powertrain and electrical defects create an immediate, severe safety hazard for myself and my young children while driving.

Today, on [XXX] outside of Sapulpa Oklahoma, while in the inside lane of three southbound lanes, cruise control on at 80mph.
