Summary
A recurring fuel-odor and drivability problem began while the vehicle was under manufacturer warranty and was never permanently resolved. In September 2022, a Volkswagen dealer documented code P2421 for an EVAP system vent valve stuck open and performed a software update. In June 2024, another Volkswagen dealer documented a strong fuel smell intermittently entering the passenger compartment but found no leak and made no repair. In April 2025, the vehicle was reported to sputter when accelerating from a stop. In July 2025, fuel odor at startup, poor fuel economy, and slow acceleration were again reported, but no leak was found and the vehicle was declared to be operating normally. In June 2026, the vehicle developed an EPC warning and codes P0316 and P0302, resulting in diagnosis of a cylinder 2 injector problem. In September 2026, another repair facility documented continued gas smell, hesitation, possible misfire, cold-start code P0316, and a damaged intake-pipe O-ring causing an air leak. Fuel odor and drivability concerns continued. On September 22, 2026, Volkswagen Customer CARE advised that the vehicle should not be driven until the fuel-leak concern is resolved and instructed that it be towed to an authorized Volkswagen dealership. My concern is that a potentially safety-related fuel/EVAP and drivability defect was repeatedly reported over several years but was never permanently identified or corrected. I believe whats happening is - The transmission is downshifting too early, and applying a significant and random amount of braking resistance. This braking goes off and on at random. It prevents the driver from braking in a controlled manner, as this random and unpredictable additional braking caused by - i think - the transmission is constantly changing. Therefore sometime my truck will randomly and suddenly slow down, then accelerate. As a driver, I am always trying to match the trucks random braking, and brake calmly. Its impossible to do. This has caused me to nearly rear end people at stop lights, Fly out of my lane and into oncoming traffic while turning, or other people nearly rear end me. It is hard to explain exactly whats happening, but imagine a second person stepping on and off the brakes, at random times, and trying to drive normally with this extra braking. Also, the E brake turns itself on at random times and will remain locked up and catches fire. Thats happened a few times The tailgate also doesnt latch properly and will come open while driving. I have had it at the dealer for service 4 seperate times, but since there are no codes, they have yet to fix anything. I refuse to drive it anymore

Summary
I was at my workplace’s parking garage today and was attempting to park my 2019 Honda HR-V. While I was slowly attempting to park, the vehicle suddenly accelerated without my intention, and the brakes became unresponsive. The vehicle continued to accelerate uncontrollably despite my attempt to stop it. The vehicle crashed through/into the metal barrier cable at the edge of the parking garage. The barrier cable ultimately prevented the vehicle from going over the edge of the garage. If the barrier had not stopped the vehicle, I could have fallen several levels from the parking garage and potentially died. The vehicle eventually came to a stop after making contact with a neighboring parked vehicle. That vehicle was damaged during the incident. I am extremely fortunate that no one else was injured. I also did not sustain any major injuries, although my left knee is currently aching because my seat slid completely forward during the sudden acceleration. The vehicle is currently at a towing facility. Immediately following the incident, I was extremely shaken and was not in a condition to take the vehicle to a Honda dealership or independent mechanic. As of the time of this report, the vehicle has not been inspected by a Honda dealership, mechanic, or independent service center, so the cause of the incident has not yet been determined. Prior to the incident, the vehicle had been operating normally. There were no warning lights or other warnings displayed on the dashboard, and there was no indication of a problem with the vehicle before the incident. The vehicle appeared to be functioning normally as it had on previous days. I am reporting this incident because of the serious safety concern involving the sudden acceleration and apparent loss of braking effectiveness. I believe it is important for NHTSA to have a record of this incident, particularly because the consequences could have been fatal if the parking garage barrier had not stopped the vehicle.

2017 Volvo XC90 manufacturing defect where panoramic sunroof drainage network fails under normal rain, pooling water directly in the floorboard channels over safety module networks and wire harnesses.Water infiltration caused a catastrophic short-circuit of the floorboard-mounted amplifier while driving. This forced the audio system to emit an un-modulated, maximum-volume electronic blast through the cabin, causing severe driver distraction and documented temporary acute acoustic trauma/ear injuries to myself and my young child.Progressive electronic oxidation from ongoing water intrusion has since caused systemic failures of critical safety electronics. This includes spontaneous, unprompted activations of the automated emergency SOS calling network and dangerous operational delays in the backup camera engaging.Dealership service personnel inspected the vehicle and explicitly warned that the water-logged electrical networks are heavily compromised and at imminent risk of a complete blackout while in motion, posing a severe threat of sudden loss of power steering, braking, and propulsion on the highway. Rain is currently actively dripping from the interior ceiling onto passenger cabin components. This platform-wide engineering flaw creates an active fire, electrical, and crash hazard.

Summary
On [XXX], while driving near Mammoth Mountain, California (Highway [XXX]), the vehicle suffered a sudden, severe engine malfunction. The engine began misfiring violently, accompanied by a loud, mechanical clacking noise, and experienced an immediate loss of propulsion power. This mechanical failure left my family—including young children—completely stranded on the side of a mountain highway in a secluded wilderness area with dropping temperatures (39°F) and active local wildlife Because the vehicle could not safely maintain highway speeds or generate sufficient power, it created an immediate and severe traffic safety hazard, forcing us to secure emergency transport for the vehicle. Which took 4hours for someone to come out to the secluded hwy on a holiday weekend. Dealership Diagnostic Findings A formal inspection conducted by a certified General Motors dealership (Michael Hohl Motor Company) confirmed the following: Symptoms: Gas engine diagnostic confirmed "Misfire, and clacking noises coming from passenger side of the motor." Diagnosis: Cylinder Head / Valve / Lifter / Lash Adjuster failure. The dealership documented a collapsed lifter and recommended a complete engine tear-down to inspect lifters and pushrods for wear or bending. The contact owns a 2026 Chevrolet Silverado 1500. While driving at an undisclosed speed, several brake warning lights illuminated on the instrument panel. The contact attempted to depress the brake pedal, and the brakes failed to respond as needed. The contact pumped the brake pedal continuously to bring the vehicle to a stop. The vehicle was then driven in LIMP Mode. The vehicle was taken to a dealer and was diagnosed with a brake control module failure, which was caused by an incorrect software version that had crashed the module.
