COMPONENT/SYSTEM: The infotainment/audio head unit and instrument display, which communicate over the vehicle's MOST (Media Oriented Systems Transport) fiber-optic network via FAKRA connectors. The vehicle is available for inspection on request. FAILURE: While driving, the center display and Carplay screen spontaneously reboot and show "Network Communication Lost." During these events I intermittently lose the rearview backup camera and other display functions and navigation as well as communication. The failures are intermittent, happen more often over bumps, and sometimes at startup, and even during smooth road conditions. SAFETY RISK: Losing the rearview camera is a hazard when backing up, and having the instrument/infotainment display reboot while driving is distracting and removes information the driver relies on constantly. CONFIRMED BY A DEALER: Yes, repeatedly. The manufacturer issued a safety recall (Recall 23-047, "MOST FAKRA service cables"), which a franchised dealer performed in May 2024, replacing MOST network components. About 15 months later, the same defect returned; the dealer found the audio-unit connection was not secured, reseated it, and confirmed the wiring had proper continuity (ruling out rodent or wire damage). The dealer otherwise offered only workarounds - an aftermarket Bluetooth adapter and recording video of the failure - not a durable repair. The issue still persists. INSPECTED BY MANUFACTURER: The manufacturer-directed recall remedy was performed by the dealer; there has been no police or insurance inspection. WARNING MESSAGES: Yes - a "Network Communication Lost" message accompanies each reboot. The problem was present before and after the May 2024 recall and had clearly recurred by mid-2025. KEY POINT: This is a failed safety-recall remedy. Recall 23-047 was performed at about 29,000 miles and the same defect returned by about 36,700 miles - a failure within roughly 7,600 miles. It persists today.

I bought the 2020 equinox May the 9th 2026 the dealership concealed a lot of things going on this vehicle and I complain to them two days after purchase he told me I was stuck with it in the meanwhile as I drive the vehicle I have had repairs done but it's something going on with the acceleration it accelerates on its own sometimes and slow to pick up speed. The steering seems to get away from me at times that makes me feel like I'm going to lose control when driving. I have almost had a couple of accidents since then so what really worries me is the steering problems which I just had a little alignment done today July 15th 2026 and the accelerating from time to time went backing up or pulling off. I do feel like the safety of others can be at risk for should I say my safety is that risk at times so I am extremely cautious when I'm driving. No I have not had really anyone to look at I have been having a car looked at for many issues but that issue I am still trying to get in to see a mechanic that I'm used to dealing with but he's been backed up. No there are no lights of any such but when I purchased it the sensors in the tires and I've had that replaced and I've noticed this situation with the acceleration when I bought the vehicle and the slow speed pick up when driving.

Summary
The water pump failed catastrophically at 63,100 miles, forcing a complete road-side breakdown and damaging the radiator. This is a known manufacturing defect affecting the 3.6L VR6 composite plastic pumps. Due to the failure of the pump, the radiator tank became hot causing a hole to form in the radiator and needing to be replaced. I still own this car and you may inspect it if needed. My safety was at risk as well as others safely was put at risk because this caused car to over heat leaving me broken down on the side of the road. My mechanic at specialized auto repair confirmed and reported this issue to me and they provided the repair. It was not inspected by police, or insurance because it was a break down, not an accident and that's not coved by insurance. I knew that the car was overheating because waring system showed it to be overheating. It was actually more than over heating because of the water pump failure, the radiator melted causing car to dump all the coolant/water creating an over heating emergency. The warning system first appeared at the same time as the pump failure and the car had already overheated due to radiator hole when the waring system alerted me to the car overheating.

I am reporting a catastrophic structural wheel failure involving my 2023 Tesla Model Y, VIN [XXX], with approximately 78,439 miles. On XXX at approximately 9:30pm, while heading home at approximately 35-40 mph, the front passenger-side wheel fractured and separated into two major pieces. The vehicle was being operated normally on a freshly paved road under clear skies. There was no pothole, curb strike, collision, or known impact with any object before the failure. This was not a routine flat tire or cosmetic wheel issue. The structural wheel itself split apart, creating an immediate loss-of-control risk and raising serious concerns about the integrity of the remaining three original wheels. We purchased a replacement wheel, tire, and TPMS sensor out of pocket and had the repair completed by McGriff Best-One Tire & Service, a respected regional tire shop. After examining the failed wheel, McGriff documented that the damage did not appear to have resulted from an impact and appeared consistent with a manufacturing or factory defect. The failed wheel has been retained and preserved for inspection. An earlier McGriff service document mistakenly identified a previously owned Tesla Model 3 that remained in its customer system. McGriff corrected the vehicle information, and the corrected documentation and photographs confirm that the repair and inspection involved the subject 2023 Tesla Model Y. I reported the incident to Tesla and requested escalation to Field Engineering or Product Safety, transportation of the vehicle for inspection, examination of the remaining wheels, and a written engineering report. Tesla’s Birmingham service personnel initially treated the matter as a routine customer-paid wheel replacement. Tesla subsequently dismissed my written safety concerns and closed the service chat without providing a substantive investigation, engineering findings, or assurance regarding the remaining wheels. Moving about 55 miles per hour on right lane of I-95 the Jeep in front of me braked abruptly. I braked hard but as my Sienna was quickly slowing down to stop, the brake pad felt spongy when it was slower and as I pressed even harder, it seemed to have stopped decelerating at that slower speed and that was when my Sienna slammed into the back of the Jeep. Happened today on July 15, 2026 at around 4:24 EST. I will also file this same report to Toyota. My wife and I are at the hospital getting checked, but my Sienna looked undrivable.
