Summary
The Body Control Module of my 2018 Accord has failed. At 70,000 miles on my vehicle the door locks began to be unresponsive. At 100,000 Miles all door locks were unresponsive to the door control and fob control. The only way to secure my vehicle is to manually lock all doors and use the key within my fob to unlock the drivers door and then manually unlock all other doors. Ed Martin Honda in Indianapolis inspected the vehicle and determined the the Body Control Module would need to be replaced to make the Door locks work. There were known issues with the Body Control Module as there was an active recall for the software of the system. I completed the recalls software update at the time it was issued but this seems to be a continued problem.

On June 24th, 2026, I received a warning for "Supplemental Restraint System Problem". The airbag Dash indicator illuminated on the dash. The issue is intermittent and the warning has appeared on five separate occasions since. The message had went away when the vehicle has been turned off and has not appear every time the car is running. It does appear mid-travel on occasion. The issue is consistent with the passenger airbag sensor failure description recall that is associated with other VIN Numbers for the 2028 Accord but my VIN is not listed in the recall.

The forward collision warning is excessively sensitive to vehicles and pedestrians. I've had the alert go off recently for something so simple as pulling into a parking spot. It goes off frequently just driving past pedestrians and throughout what I consider to be normal driving where I am not coming close to a collision with a vehicle. It is very unsettling and startling when the alert goes off indicating that I am coming close to a collision with a vehicle or a pedestrian. I had a 2023 Silverado 1500 LT/2.7 Turbomax engine and it was not this sensitive. I fear that this may actually cause an accident at some point or if the emergency braking were to activate with another vehicle behind me.

This is the second high-voltage traction battery failure on this vehicle. The original HV battery failed and was replaced by Nissan under warranty at an authorized dealer on XXX, at approximately 59,000 miles. The Nissan-manufactured replacement pack has now failed after approximately 16 months in service. On XXX, at ~74000 miles, the vehicle was diagnosed at Johnson City Nissan ([XXX]) with an internal short circuit within the high-voltage battery pack The error codes were P31E7-00 0B and P0AA6-1A 0B. Dealer service personnel identified the condition as an internal short in the pack. Due to the fire risk associated with an internal short in a lithium-ion traction battery, I declined to take possession of the vehicle, which remains at the dealership. An internal short can lead to thermal runaway without warning, whether the vehicle is being driven, charging, or parked. The failure of a factory replacement battery pack in 16 months, following the failure of the original pack, indicates a potential manufacturing or design defect in Nissan's service replacement HV battery packs for this platform. I am reporting this so NHTSA can evaluate whether other Leaf replacement packs exhibit similar internal short failures. At 112,000 miles, the engine in my 2024 Honda CR-V Sport Hybrid (2.0L Atkinson-cycle engine) suffered a catastrophic head gasket failure, causing severe coolant intrusion into the cylinders. All scheduled maintenance intervals in the owner's manual were followed and documented. The dealer and corporate (Case #16444920) have refused assistance. This is a widely documented mechanical defect where constant thermal cycling (the gas engine repeatedly turning on and off during hybrid operation) stretches the factory head bolts and destroys the gasket seal. This defect can lead to unexpected, total engine failure while driving at highway speeds, posing a severe safety risk to the occupants.

Summary
On April 8, 2026, my 2020 Cadillac XT5 experienced a sudden and complete engine failure while I was driving. Prior to the failure, there were no warning lights, no overheating indications, no abnormal engine noises, and no performance issues that would have alerted me to a developing problem. Without warning, the engine failed and the vehicle immediately lost power, creating a serious safety hazard. The vehicle became inoperable and had to be towed. Fortunately, I was not traveling on a freeway or in heavy traffic at the time of the failure. Had this occurred at highway speeds or while crossing an intersection, the sudden loss of propulsion could have significantly increased the risk of a crash. This unexpected loss of power increased my risk a potential highway hazard. The vehicle was subsequently inspected and diagnosed by an authorized Cadillac dealership. The dealership determined that the engine suffered an internal mechanical failure, including rod bearing failure and metal contamination throughout the engine. The dealership advised that the engine required complete replacement. The vehicle has been properly maintained and serviced. There was no owner misuse, neglect, overheating event, or other known condition that would explain such a catastrophic failure. The engine failed unexpectedly and without any prior warning to the driver. I am submitting this complaint because a sudden engine failure resulting in immediate loss of power presents a significant safety concern. I believe NHTSA should investigate whether similar failures have occurred in other 2020 Cadillac XT5 vehicles and whether a manufacturing or design defect may exist that could place other drivers at risk.
