The contact owned a 2020 Volvo XC60 II. The contact stated while reversing, the rear view camera displayed a black screen. The contact stated that the accelerator pedal was depressed, however the vehicle failed to accelerate as intended. The vehicle's speed independently accelerated. The contact's vehicle crashed into a parked vehicle. The contact stated that there were two additional occupants in the vehicle. The contact sustained pain in the right knee, no medical treatment was provided. The passenger, who was seated in the rear seat, sustained neck and upper back injuries. The passenger received medical treatment at the local medical center. The vehicle was driven to the residence. The failure was intermittent. The contact stated that the vehicle was previously taken to the dealer, however the failure was not duplicated. The contact later received a recall notification, NHTSA Campaign Number: 25V908000 (Back Over Prevention). The manufacturer was not made aware of the failure. The failure mileage was 40,000

Summary
The vehicle is a 2020 Honda Odyssey. A recurring and dangerous safety failure occurs involving the Auto Idle-Stop system. With the system enabled and the vehicle at a complete stop, the engine shuts off as designed. However, upon depressing the brake pedal to resume driving, the engine severely struggles to restart and completely fails. Simultaneously, the vehicle immediately and automatically shifts itself into PARK, and the entire dashboard display illuminates with all available warning icons and messages. This malfunction creates an immediate, high-risk safety hazard. When the incident occurs, I am trapped, locked in PARK, in live traffic lanes, sometimes at busy intersections or during left-hand turns. It is impossible to shift back into drive or quickly restart the engine. I am forced to remain stationary in traffic for multiple minutes until the system allows me to cycle the ignition completely OFF and then back ON to attempt a full restart and shift back into Drive. This leaves my vehicle, my passengers, and other drivers vulnerable to rear-end collisions.

I have had issues with my door locks one by one for the last year until now all of them have failed and no longer work. I have mentioned this issue multiple times at Bob Tomes Ford in McKinney Texas the last few times and got prices for the repair but it is a bit pricey. Recently I have just found out that Ford had campaign 20M04 to address these bad door lock actuator latches but the car has to be under 10 years old and under 150,000 miles. This service I am currently at the service department finally mentioned it to me and advised that I am out of the free repair window as I am now just over the 10 years and mileage and it will now be $2,400 dollars to repair this. I never received any kind of mailing announcing this recall campaign or was never told about it the numerous times I have been into the dealer in the past to bring my vehicle in for basic maintenance like I am usually told about other recalls. My very last trip here I was under the covered time period and nothing was said to me after I mentioned the door locks starting to go bad. I feel Ford and this dealership Bob Tomes Ford is deliberatly not telling customers about this so they do not have to repair it until the consumer will have to pay for the fix. This is incredible deceitful on Ford and the dealerships part and I feel that something should be done about this

The car started having the ACC, LKAS, Collisions Mitigation, Auto High Bam, Road Departure mitigation system warnings. Took it to the dealer for another repair and was told that these lights were due to the front facing camera and was quoted over $1k for repair. I’m convinced it’s electrical and not necessarily what the Honda narrative is. This model has had several issues around electrical components. Please open an investigation into this car around this issue.

On occasion the car will not accelerate from a dead stop like it should. I think it’s turbo lag. There have been a few times I thought I was going be broadsided pulling out into traffic when normal acceleration would have been safe but with a dead pedal for a few seconds it was terrifying. I’ve brought this up to two different dealers and they say without being able to experience it there’s nothing they can do. Someone going to die because of this.

Summary
- Rod #4 failure of unknown cause, although there is speculation that current failure may be related to a major repair in oil system a year ago. - My safety along with my unborn child’s, husband’s and brother in law’s were all at risk. Another driver had to take evasive action to avoid hitting us in an active fast lane on I-70 going between 65-70 mph. There was no shoulder available to pull off on until a half mile up the road. - Yes the problem has been confirmed from the dealership partially. They did not identify a cause for the sudden failure. However stated it could not be determined if this failure is or is not related to a previous major engine repair made a year ago. - The engine has been inspected by the dealership and warranty company. - There were no warning signals prior to either engine failures. The first one being a year ago for head gasket failure that allowed oil to mix with coolant. With the most recent failure the engine ticked briefly after braking to merge for construction, the oil pressure light followed by engine light turned on 5 seconds before engine completely lost power.
