What is the reason for this notice? Proterra has decided that a defect that relates to motor vehicle safety exists in certain 35’ Catalyst E2 (2017 – 2019), 40’ Catalyst E2 (2017 – 2019), 35’ Catalyst RR (2020 – 2021), 40’ Catalyst RR (2019 – 2020), 35’ ZX5 (2020 – 2022), and 40’ ZX5 (2019 – 2022) transit buses. Why is your vehicle being recalled? On certain vehicles, a lack of plexus adhesive between the torque limiter plate and bus body may cause bolts holding the steering gear box in place to become loose. This condition can make it difficult to steer the vehicle and, over time, if the steering gear box were to move out of position, the driver could lose the ability to steer the vehicle, increasing the risk of a crash. We apologize for the inconvenience. We are committed to your safety, the safety of your customers, and your continued satisfaction with our products. IMPORTANT • Your Proterra vehicles identified by VIN above are involved in Safety Recall 22V-114 because bolts holding the steering gear box in place may become loose. • Proterra will provide a remedy free of charge as soon as parts are available. In the interim, you should complete the enclosed steering gearbox torque stripe and re-torque fleet check. • Contact Proterra’s customer service department at 864- 438-0000 or ServiceParts@Proterra.com if you have any questions. What should you do? Parts for the recall remedy are not yet available, and in the interim, we request that you complete an inspection of the steering gearbox bolts’ torque stripe and re-torque the bolts, as described in the attached fleet check bulletin. You should submit a warranty claim for the labor associated with this fleet check, which is estimated to be one hour per bus. You should also monitor the vehicles included in this campaign. If your bus operators hear noise when turning the steering wheel or have difficulty turning the steering wheel, please stop driving the vehicle and arrange for the vehicle be towed for service. What will Proterra do? We will notify you again once the remedy parts are available and will provide a service bulletin and repair kit for completing the remedy at no cost to you. What if you have already addressed this issue in your vehicles? The fleet check must not be conducted on buses that have had Loctite previously applied on the steering gearbox bolts. If Service Campaign SC-22-003 has previously been performed, do not proceed with this fleet check. Instead, the forthcoming repair kit and service bulletin will remedy those buses. STEERING GEARBOX TORQUE STRIPE AND RE-TORQUE FLEET CHECK Retrofit Description: This fleet check is to verify that the Steering Gear mounting bolts have maintained securement. Loss of securement may indicate that the plexus adhesive layer may be insufficient between the torque plate and the chassis. If the vehicle fails this inspection, then further invasive testing can be done to evaluate the adhesive layer. This procedure verifies the Steering Gearbox securement bolt torque. Complete the Proterra approved Lockout/Tagout procedure to make the bus safe for work. 2. Using four Wheel Lifts, raise the bus into the air. Using four Jack Stands, support the bus for safety. 3. Access the Steering Gearbox underneath the bus. 4. Using the following illustrations and photographs as a guide, inspect the Steering Gearbox Securement Bolts. The following photograph shows an example of a good unbroken torque stripe. 6. The following photograph shows an example of a bad torque stripe. Notice the torque stripes are broken due to rotation of the bolt. 7. The following photograph shows an example of a good torque stripe. Although it is broken, the bolt has rotated clockwise for a small portion of one turn. This can occur when the bolts have been re-torqued prior to shipment during a factory inspection. 8. Using a Calibrated Torque Wrench with a 1-5/16 Inch Ratchet/Socket, set the torque wrench to 450 ft-lb. Slowly tighten the bolt. If the bolt rotates before the wrench clicks, please report that bolt failed the re-torqueing check on the inspection form linked above. 10. Using a 1-5/16 Inch Ratchet/Socket, remove that bolt (018758-031) and add Loctite 248 to the threads. The Loctite 248 should fill the threads on the bolt where it engages the fastener. 11. Using a Calibrated Torque Wrench with a 1-5/16 Inch Socket, torque the re-installed bolt to 150ft-lb. Then test the re-application of the torque, remove, and reinstall the other three (3) remaining bolts in the same manner in the sequence shown in the illustration on the right below. 12. Using a Calibrated Torque Wrench with a 1-5/16 Inch Socket, torque each of the four bolts to 300 Foot Pounds. Cross-torque the bolts by following the sequence shown above. 7. Using a Calibrated Torque Wrench with a 1-5/16 Inch Socket, torque each of the four bolts to 500 Foot Pounds. Cross-torque the bolts by following the sequence shown above. 8. Using Alcohol and Shop Towels, remove any Orange Torque Stripe Paint from previous torqueing operations. 9. If the Torque Stripes are in good condition and none of the bolts failed the re-torque test no further inspection is required and this check is complete. The bus may be lowered and returned to service. 11. If bad/broken torque stripes are identified or any of the bolts failed the re-torque test, contact Proterra Customer Service to receive additional instructions after filling out the inspection form linked above. If a Proterra team member performs the inspection the Work Order should be escalated to the Proterra back office in addition to submitting the form. 12. If bad/broken torque stripes were found or any of the bolts failed the re-torque test, the bus may be lowered and Lockout/Tagout devices removed but the bus should not be returned to service until additional inspections are performed.

Proterra investigated and determined the root cause of this failure was the same as the root cause of two 2020 incidents of loose steering gearbox bolts. The 2020 incidents led Proterra to declare a voluntary safety recall (20V-664) in October 2020 on 169 vehicles. The vehicle in the December 2021 report was not one of the 169 vehicles included in the 20V-664 campaign. In January 2022, Proterra’s Quality Compliance Steering Committee met and determined further investigation of this issue was warranted. On February 21, 2022, the Committee reviewed the investigation findings, including the supplier’s production records related to the amount of plexus adhesive applied to the torque limiter plate. The Committee decided to conduct a voluntary safety recall to address this issue. As of February 21, Proterra knows of no crashes, injuries, or fatalities associated with this issue. These locking caps and bolts should be installed during the next routine scheduled service for each vehicle. Updated 6,000-mile preventative maintenance guidelines will include work instructions to determine whether there is a gap between the steering gearbox and the bus body due to inadequate application of plexus adhesive. Any vehicle exhibiting such gap will be remedied by removing the torque plate and installing a new torque plate with sufficient plexus adhesive. All vehicles within the recall population are covered by Proterra’s warranty, therefore owners who have had this issue remedied prior to this campaign have not incurred costs for that remedy. How Remedy Component Differs from Recalled Component : The remedy components are “locking caps” and new bolts capable of accepting the caps. There are no caps on the steering gearbox bolts of any vehicle in the recall population. For the potential additional remedy associated with the 6,000-mile inspection, the remedy component is a new torque limiter plate with sufficient plexus adhesive coverage. Updated 6,000-mile preventative maintenance guidelines will include work instructions to determine whether there is a gap between the steering gearbox and the bus body due to inadequate application of plexus adhesive. Any vehicle exhibiting such gap will be remedied by removing the torque plate and installing a new torque plate with sufficient plexus adhesive. How Remedy Component Differs from Recalled Component : The remedy components are “locking caps” and new bolts capable of accepting the caps. There are no caps on the steering gearbox bolts of any vehicle in the recall population. For the potential additional remedy associated with the 6,000-mile inspection, the remedy component is a new torque limiter plate with sufficient plexus adhesive coverage. The recalled component is a torque limiter plate that lacks sufficient plexus adhesive. Identify How/When Recall Condition was Corrected in Production : The recall condition was corrected at the supplier’s production facilities on February 18, 2022 via the supplier’s adherence to work instructions requiring 100% plexus adhesive coverage on the torque limiter plate.
