Fuel Tubes May Crack and Leak Fuel

Summary

Cummins, Inc. (Cummins) is recalling certain X12 Diesel Engines, with part numbers D0S3005BX03 and D0S3009BX03. The fuel tubes between the fuel rail and the injectors for cylinders four, five, and six may fatigue and crack, which can result in a high pressure fuel leak.

 

Remedy

On engines with 75 miles or less, dealers will install vibration isolators to the fuel tubes. Engines with more than 75 miles will receive new fuel tubes with vibration isolators. Repairs will be performed free of charge. Owner notification letters are expected to be mailed in January 2022. Owners may contact Cummins customer service at 1-800-286-6467. Cummins’ number for this recall is C2532.

Dear Cummins OEM Customer:

We are writing to inform you that Cummins Inc. has determined that a safety-related defect exists in certain X12 engines that were manufactured between April 26, 2016 and November 1, 2021. Cummins has initiated a recall of the subject engines. Our records indicate that some of these engines have been shipped to you.

 

A copy of Cummins’ initial Part 573 Defect Notification report is enclosed. This notification was submitted to the National Highway Traffic Safety Administration (NHTSA) on December 1, 2021. As more fully described in the Defect Notification, the fuel tubes between the fuel rail and the injector for cylinders 4, 5 and 6 may be susceptible to cracking, resulting in a high pressure fuel leak. In the presence of an ignition source, this condition may increase the risk of a fire.

 

Enclosed is a list of Engine Serial Numbers for affected engines that our records indicate were shipped to your plants. For engines installed in vehicles that have left your control, we encourage you to make a safety defect determination in accordance with 49 USC 30118 et seq. and, as appropriate, to submit a Defect Notification to NHTSA in accordance with 49 CFR Part 573.

 

Cummins will conduct the recall, including notifying owners pursuant to 49 CFR Part 577, performing the remedy, and submitting quarterly status reports to NHTSA. Cummins is requesting that you provide the associated vehicle model identification, VINs and owner contact information to facilitate owner notification. Please work with your Cummins account team leader to ensure that the requested information is provided to Cummins as soon as possible.

 

For engines/vehicles that are still in your inventory, please contact the Customer Quality Assurance representative at the Jamestown Engine Plant to arrange for repairs. Please note that Federal law prohibits delivery of these engines, or vehicles equipped with these engines, under a sale or lease until this condition has been remedied.

 

Cummins will conduct a safety campaign to remedy the issue by replacing certain fuel tubes with new fuel tubes with the addition of a vibration isolator.

 

Cummins apologizes for this inconvenience, but we are initiating this action in the interest of our customers’ safety and satisfaction with our products. Thank you for your assistance.

This notice is sent to you in accordance with the requirements of the National Traffic and Motor Vehicle Safety Act.  Cummins Inc. has decided that a defect which relates to motor vehicle safety exists in certain Cummins X12 Engines that were manufactured between April 26, 2016 and November 1, 2021 and for installation in mixer, truck, coach, emergency vehicle, refuse truck, recreational vehicle and crane applications.

 

This notice applies to the engines(s)/vehicles(s) listed on the enclosed table.

 

WHAT IS THE PROBLEM?

The fuel tubes between the fuel rail and the injectors for cylinders 4, 5 and 6 may be susceptible to cracking, resulting in a high pressure fuel leak.  A high pressure fuel leak in the presence of an ignition source may increase the risk of a fire.

 

WHAT ARE WE DOING ABOUT THE PROBLEM? 

Cummins is currently developing the permanent remedy for this issue.  You will receive a second notification to inform you when the final remedy is available and to provide instructions to schedule the recall for your engine.

 

WHAT SHOULD YOU DO?

In the meantime, Cummins advises you to periodically check the injector fuel tubes for signs of leaking and to seek repairs if leaks are identified.  If a leak occurs, the operator may smell diesel fuel or observe a mist.

Description of Change

Vibration isolators have been added to the high pressure injector supply lines on the cylinder numbers 4, 5, and 6 only.

The vibration isolators are supplied as part of a kit that also contains the high pressure injector supply line.

Vibration isolators will be offered for sale individually, however, the high pressure injector supply lines will only be sold in the corresponding kit.

 

Reason for Change

The addition of vibration isolators will reduce the dynamic stress on the high pressure injector supply lines.

 

Service Instructions

If existing high pressure injector supply line is replaced, use the new kit containing the line and the vibration isolator. See corresponding Service Manual. Reference Procedure 006-051 in Section 6.

New vibration isolator must be installed with a new high pressure injector supply line.

If an existing vibration isolator has been lost or damaged, replace with a new vibration isolator.

See corresponding Service Manual. Reference Procedure 006-051 in Section 6.

The high pressure injector supply line and the vibration isolator are not assembled inside the kit. Verify the vibration isolators are installed. See corresponding Service Manual. Reference Procedure 006-051 in Section 6.

Attention

 

Purpose

The purpose of this memo is to notify repair locations that Cummins Inc. is initiating interim end user notification mailings regarding a safety defect on certain X12 engines.

 

Discussion

Cummins Inc. has decided that a defect which relates to safety exists in certain X12 CM2350 X119B engines that were manufactured between April 26, 2016 and October 11, 2021 and certain X12 CM2450 X137B engines that were manufactured between August 27, 2019 and November 1, 2021. The fuel tubes between the fuel rail and the injector for cylinders 4, 5 and 6 may be susceptible to cracking, resulting in a high-pressure fuel leak. A high-pressure fuel leak in the presence of an ignition source may increase the risk of a fire. Cummins has notified affected Original Equipment Manufacturers to which these engines have been shipped and has filed a Defect Notification with the U.S. National Highway Traffic Safety Administration for applicable engines. NHTSA has assigned this recall number 21E-099. The Safety Campaign is being finalized; it is currently expected to be published on QuickServe Online on or about February 17, 2022 when the parts are expected to be available. Campaign Number C2532 will apply to this Safety Campaign.

 

Action

Until the Safety Campaign is available, for affected units that develop a leak, replace the fuel line per service procedure in the associated service manual and refer to TSB210271 for information regarding vibration isolator installation. For units that are not leaking, distributors and dealers must wait until the appropriate Safety Campaigns are published on QSOL to schedule any repairs. Cummins expects to begin notifying owners on January 28, 2022 with interim letters informing them that the safety defect exists and that they will receive another notice when the repair is available.  Cummins is advising customers to check for leaks in accordance with the daily maintenance schedule outlined in the appropriate Engine Owner’s Manual and seek repairs if leaks are identified.  If a leak occurs, the operator may see or smell diesel fuel. A second notification will be sent to end users to inform them when the final remedy is available and provide instructions to schedule the recall for their engine. The second notifications are expected to mail during 1st Quarter of 2022. Customers and repair locations with questions may contact Cummins Care at 1-800-CUMMINS (1-800-286-6467).

Description

This Safety Campaign is being issued to address an issue on certain X12 CM2350 X119B and X12 CM2450 X137B units in which the fuel tubes between the fuel rail and the injector for cylinders 4, 5 and 6 may be susceptible to cracking, resulting in a high pressure fuel leak. A high pressure fuel leak in the presence of an ignition source may increase the risk of a fire.

Cummins has reported this issue to the U.S. National Highway Traffic Safety Administration (NHTSA), which has assigned it Recall Number 21E-099.

Note : This field action provides retroactive coverage for repairs made in anticipation of and prior to the release of this publication.

 

Action

In order to qualify for repair under this field action, an engine:

  1. will be covered regardless of coverage status, and
  2. must show as OPEN on QuickServe® Online for this field action.

Note : The ESN list is attached for reference.

After verifying that the engine meets the above requirements, perform the following actions:

1. Replace the fuel injector supply lines 4, 5, and 6 with new fuel lines and vibration isolators. Refer to Attachment B for detailed instructions to complete the repair.

2. If vibration isolators are already installed on injector supply lines 4, 5, and 6, no further action is needed. File a claim for admin ONLY and close the eligibility of the engine for C2532.

3. File one claim for the listed parts and required labor associated with this repair.

 

Material Disposition

Materials removed as a result of this field action must be scrapped.

X12 Fuel Injector Supply Line Campaign Procedure

The purpose of this campaign is to replace Injector Supply Lines and add Vibration Isolators on the Fuel Lines for cylinders 4, 5, and 6.

 

Verification

  • Locate Injector Supply Lines on cylinders 4, 5, and 6.
  • Check to see if there are Vibration Isolators installed on the Injector Supply Lines. See Figure 1 below.
  • If there are Vibration Isolators installed on Injector Supply Line 4, 5, and 6 then no further action is needed.
  • If there are NO Vibration Isolators installed on Injector Supply Lines 4, 5, and 6 then proceed with the campaign instructions below.

Service Instructions

Follow the steps below to remove the Fuel Injector Supply Lines for cylinders 4, 5, and 6 and replace with new Fuel Injector Supply Lines with Vibration Isolators supplied in the parts kit.

1. Before starting the removal and installation of the Injector Supply Lines, loosen the pump to rail line at the rail to vent the fuel pressure. Keep your hand clear of the line when loosening the fuel rail nut. See Figure 2.

2. Tighten the fuel rail nut back to 31 Nm (23 ft‐lbs).

 

3. Disconnect the batteries. See equipment manufacturer service information.

4. Clean the fittings and area around the connection points for Injector Supply Lines 4, 5, and 6 using electrical contact cleaner.

5. Remove Injector Supply Lines 4, 5, and 6.

6. Cover the openings with clean care caps.

7. Break the capscrews for the fuel rail loose and then tighten them back up just enough so the fuel rail does not move. DO NOT fully torque the fuel rail capscrews back down.

8. Install a Vibration Isolator on each of the Fuel Injector Supply Lines 4, 5, and 6.

a. Make sure the Vibration Isolators are installed firmly on the straight section on the upper half of the Fuel Injector Supply Lines.

b. Make sure there is a gap between the Vibration Isolator on Fuel Injector Supply Line #5 and the Crankcase Breather Tube. See figure 3.

i. If there is contact between the Vibration Isolator and the Crankcase Breather tube, then you either need to try to position the Crankcase Breather Tube better or you can slide the Vibration Isolator up the fuel line farther. The Vibration Isolator must NOT be positioned past the bend in the fuel line. See Figure 4 below to see where the Vibration Isolators can be installed.

ii. Some configurations may have other interferences as well, see figure 5. The Vibration Isolator must not come in contact with any other component.

Figure 4: Visual reference for where the Fuel Injector Supply Line the Vibration Isolators can be installed. The Vibration Isolators MUST be installed between the bends of the fuel line or between the red lines in the example above.

 

Figure 5: Some configurations may have a Compressor Air Inlet Tube going above line #5. Make sure the Vibration Isolator does not come into contact with this tube or any other component.

 

9. Using clean engine oil, lubricate the three Fuel Injector Line grommets in the rocker cover before fitting the Fuel Injector Lines.

10. Install the Fuel Injector Supply Lines one at a time following the steps below.

a. Hand tighten the Fuel Injector Line Nut at the Injector. Make sure the lines are centered within the nut.

b. Hand tighten fuel line nut at the rail. Make sure the line is centered within the nut.

c. Torque the injector end of the Fuel Injector Line to 31 Nm (23 ft‐lb).

d. Torque the Fuel Rail end of the Fuel Injector Line to 31 Nm (23 ft‐lb).

11. Repeat steps a – d for the other two Fuel Injector Supply Lines.

12. Torque the capscrews for the rail to 46 Nm (36 ft‐lb).

13. Connect the batteries. See equipment manufacturer service information.

14. It is not necessary to vent air from the high‐pressure fuel system before starting the engine. Cranking the engine will prime the fuel system.

15. Operate the engine. Check for leaks.

16. Destroy the 3 Fuel Injector Supply Lines that were removed from the engine.
