Improperly Tightened Axle U-Bolts

Summary
Keystone RV Company (Keystone) is recalling certain 2021-2022 Crossroads Redwood, Dutchmen Voltage, and Yukon trailers. The U-bolts on the axles may have been improperly tightened, causing the bolts to loosen and the axle to slide.
Remedy
Dealers will tighten the axle spring U-bolts, free of charge. Owner notification letters were mailed October 27, 2022. Owners may contact Keystone customer service at 1-866-425-4369. Keystone’s number for this recall is 22-436. This recall expands NHTSA recall number 21V-015.

SAFETY ADVISORY # 22-436
Keystone is conducting a voluntary RECALL notification campaign in accordance with the National Highway Transportation and Safety Act. It has been decided that vehicles in the recall population have a Dexter axle using a spring pack with U-bolts that may not be properly torqued. Under certain conditions during travel, if the improperly torqued U-bolts come loose there is an increased risk of the axle sliding back on the springs, which could result in poor vehicle handling and/or vehicle crash.

Note – Check serial number on Key Express to verify the campaign is open. Some vehicles were repaired before they left Keystone.
When performing warranty or recall service, please make certain that appropriate Personal Protective Equipment (PPE) is used.
The remedy is to torque the axle U-bolt nuts in a cross pattern to 110 ft lbs.
REPAIR INSTRUCTIONS
Step 1
Locate the trailer on a level, flat, hard surface. Chock the wheels.
Step 2
Follow the Dexter instructions below.
Step 3
Proceed to warranty reimbursement.
U-Bolt Fastener Torque on D80 Axle Models (RE-TORQUE)
Chronology :
6/13/2022 – During a claim review of 8k axles it was determined that a claim of axles had shifted similar to Keystone recall 21-394, 21V-015. Claim review to determine number of total failures.
6/16/2022 – Four cases were sent to Dexter to review failures of spring packs. Dexter opened investigation into failures.
8/18/2022 – Meeting with Dexter to review a total of 9 failures dating between 1/7/2021 and 8/18/2022, zero personal injuries or deaths, and review investigation. No new information determined into failure mode of spring pack. No determination could be made as failed axles had yet to be returned from dealers to review failed parts. Failed parts to be obtained and tested.
9/14/2022 – Meeting with Dexter to review investigation. No new information determined into the failure mode of the spring pack. Investigation is ongoing.
9/15/2022 – Keystone Safety Committee reviewed the information and concluded that the data presented indicated a unanimous vote for a voluntary recall. Investigation called for to determine affected unit range.
9/23/2022 – Range determined.

Claim review to determine number of total failures. 6/16/2022 – Four cases were sent to Dexter to review failures of spring packs. Dexter opened investigation into failures. 8/18/2022 – Meeting with Dexter to review a total of 9 failures dating between 1/7/2021 and 8/18/2022, zero personal injuries or deaths, and review investigation. No new information determined into failure mode of spring pack. No determination could be made as failed axles had yet to be returned from dealers to review failed parts. Failed parts to be obtained and tested. 9/14/2022 – Meeting with Dexter to review investigation. No new information determined into the failure mode of the spring pack. Investigation is ongoing. 9/15/2022 – Keystone Safety Committee reviewed the information and concluded that the data presented indicated a unanimous vote for a voluntary recall. Investigation called for to determine affected unit range. 9/23/2022 – Range determined.
