Prevost Car (US) Inc. (Prevost) is recalling certain 2021 X3-45 commuter buses. One bolt that secures the rear bench seat frame to the body of the vehicle was not installed during the manufacturing process. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 210, “Seat Belt Assembly Anchorages.”
Remedy
Dealers will install the missing bolt, free of charge. Owner notification letters are expected to be mailed on August 22, 2022. Owners may contact Prevost’s customer service at 1-866-870-2046.

DESCRIPTION
On vehicles affected by this recall, one bolt that secures the rear bench seat frame to the vehicle’s body was not installed during the manufacturing process because a hole required for the installation of the bolt was missing on the lower back panel.
Therefore, all affected vehicles must have this hole drilled and the required bolt installed following the procedure below.

2022-06-09 Issue detected in production at the Plattsburg plant by Prevost’s Quality Department.
2022-06-13 Retrofit R22028 was launched to inspect and add a bolt, if required, on the rear bench assembly.
The retrofit concerns vehicles in production and in service.
2022-06-29 Case was presented on Volvo Product Safety Working Group (PSWG) to evaluate if there is a potential safety related issue.
2022-07-07 Case was presented to Volvo Product Safety Committee (PSC). It was concluded that there was a safety related defect and that a recall is needed.
2022-07-11 Safety Recall report submitted to authorities.
No warranty claims related to this issue. No other field reports, incidents, accidents, injury, or fatality were reported.

Prior to working on the vehicle, set the ignition switch to the OFF position and trip the main circuit breakers equipped with a trip button. On Commuter type vehicles, set the battery master switch (master cut-out) to the OFF position. Lockout & Tag out (LOTO) must be performed during set-up, maintenance or repair activities. Refer to your local procedure for detailed information regarding the control of hazardous energy. 1. Remove the seat base from the 2nd curb side rear bench seat (red circle Figure1). • Unscrew the two screws retaining the seat base to the frame (arrows Figure1). • Lift to remove the base and expose the frame structure. Drill the missing 1/2in hole in the plywood base using the slot as a guide (Figure3). • Using a cordless drill (or equivalent) and a 1/2 in drill bit, drill through the carpet and the plywood inside the slot to expose the welded nut behind it. NOTE The use of a drill stopper installed at 3/4 in from the drill tip is recommended. IMPORTANT: Compare with the two top bolts and drill the hole at the same position (left, center or right) in the slot (right position is shown in Figure3). • Remove wood chips and frayed carpet from the hole as necessary. Install the missing bolt and hardware (Figure4). • Install the supplied 5002067 Bolt, the 500487 lock washer and the 500897 flat washer. • Torque the bolt to 27 lb-ft. • Add a strip of yellow Torque Seal indicator to the bolt head. • Reinstall the seat base (Torque to 27 lb-ft) to complete the procedure. How Remedy Component Differs from Recalled Component : The issue concerns an inadequately installed component, and not a defective component. After the remedy, the rear bench seats will be adequately installed in the vehicle by the addition of a bolt. This additional bolt distinguishes the recalled component’s installation. The concerned vehicles are all MY2021 Prevost X3-45 Commuter buses manufactured during the indicated production dates. The recalled products are limited to these specific vehicles since they were the only ones equipped with the defective rear bench seat assembly. A total of 30 MY2021 Prevost X3-45 Commuter vehicles are concerned by the recall. Production Dates : DEC 02, 2020 – JAN 26, 2021 VIN Range 1 : Begin : NR End : NR Not sequential Description of Noncompliance : Description of the Noncompliance : On certain vehicles, one bolt that secures the rear bench seats’ frame to the vehicle’s body was not installed during the manufacturing process. One bolt is therefore missing in the rear passenger bench assembly. FMVSS 1 : 210 – Seat belt assembly anchorages FMVSS 2 : NR Description of the Safety Risk : If the vehicle is involved in a crash, the rear seats could separate from the vehicle’s body. In this event, the passengers would be inadequately restrained, thus increasing the risk of injury. Prevost has not received any reports of death or injury associated with this defect. Description of the Cause : The final Prevost drawings of the wood panel used in the rear bench assembly were not distributed to the panel supplier. Consequently, the panels were missing a hole required for the installation of a bolt, and the bolt was not installed. Identification of Any Warning that can Occur : There is no warning with this issue. The missing bolt is hidden under the seat’s cushion. Involved Components : Component Name 1 : USSC GT TRANSIT Component Description : Set of passenger seats Component Part Number : 867942 Supplier Identification : Component Manufacturer Name : NR Address : NR NR Country : NR Chronology : 2022-06-09 Issue detected in production at the Plattsburg plant by Prevost’s Quality Department. 2022-06-13 Retrofit R22028 was launched to inspect and add a bolt, if required, on the rear bench assembly. The retrofit concerns vehicles in production and in service. 2022-06-29 Case was presented on Volvo Product Safety Working Group (PSWG) to evaluate if there is a potential safety related issue. 2022-07-07 Case was presented to Volvo Product Safety Committee (PSC). It was concluded that there was a safety related defect and that a recall is needed. 2022-07-11 Safety Recall report submitted to authorities. No warranty claims related to this issue. Description of Remedy : Description of Remedy Program : potentially affected vehicles. Letters will be sent to customers to inform them on how to proceed. Prevost will execute this correction free of charge (parts and labor) for the customers. If a customer has already inspected and corrected this defect before the safety recall has been launched, this customer will be reimbursed according to Prevost reimbursement plan. How Remedy Component Differs from Recalled Component : The issue concerns an inadequately installed component, and not a defective component. After the remedy, the rear bench seats will be adequately installed in the vehicle by the addition of a bolt. This additional bolt distinguishes the recalled component’s installation. Identify How/When Recall Condition was Corrected in Production : The recall condition was corrected in production on 2022-06-13 when the retrofit was launched. All vehicles in production were corrected prior to being delivered.
