On January 30th 2014 the Office of Defects Investigation (ODI) opened PE14-002 based on Vehicle Owner's Questionnaire (VOQ) No. 10554304 alleging that the front wheel completely separated on a model year 2008 American LaFrance Eagle aerial-ladder fire truck.During this investigation, ODI sent American LaFrance an Information Request (IR) letter seeking further information.After sending the IR, ODI learned that under direction from its owner Patriarch Partners LLC, American LaFrance had ceased operation and closed its manufacturing facilities.Subsequent communications with Patriarch Partners revealed that the records necessary to respond to ODI's IR could not be obtained.As a result of this information not being available, ODI was not able to conduct a complete investigation or locate the owners of existing vehicles.In an effort to notify owners of the vehicles about the potential safety issue, ODI released a consumer alert.This alert was distributed to the Emergency Responder community via the firstresponder.gov web site.Copies of both the alert and website where it was published can be found in the public file for this investigation.Information will continue to be collected from any operators who contact ODI who will be advised of the risks and how best to prevent future failures.Due to the inability to locate production, owner, and failure or warranty data, ODI is closing this Preliminary Evaluation.A safety-related defect trend has not been identified at this time and further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if additional data becomes available.

dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims.The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness.In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota
, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety-related defect does not exist.For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.

Durango vehicles manufactured between January 5, 2010 and December 11, 2013 (see NHTSA Safety Recall 14V-391), with notifications to begin in August 2014. According to Chrysler
, the fire is caused by an electrical short in the vanity lamp wiring for either one of the sun visors mounted on the vehicle. The sun visors are mounted to the roof of the vehicle through the headliner with three metal screws. It is possible for the sun visor wiring to be pierced by one of these screws either during initial vehicle assembly or later headliner area repairs which may cause an electrical short potentially resulting in fire.ODI opened an investigation involving MY 2011-2013 Jeep
Grand Cherokee and Dodge
Durango vehicles based on consumer reports of headliners catching fire at the front of the passenger's compartment near the sun visors. Complainants reported failure symptoms that ranged from discoloration to open flame burning of the headliner and/or sun visor material. Complainants reported a burning plastic smell and smoke from the headliner, often occurring while the vehicle was in motion. Damage ranged from a small burn spot on the headliner to a near complete burn of the vehicle resulting in a total loss; sunroof damage/glass shattering occurred in some cases. Incidents are more likely to occur when the ignition key is in the on position.However, several vehicles continued to burn after the key was turned off, and key-off fires, while highly unlikely, cannot be fully eliminated as a potential problem. The earliest incidents chronologically were often associated with vehicle assembly or in-plant repair procedures. Chrysler
revised assembly and in-plant repair procedures for the headliner which reduced the number of fields incidents.However, anytime a repair is made to the headliner of the vehicle there could still be a risk of shorting the wiring and creating a hazard.Dealership technicians will inspect the vehicles for damaged wiring caused by an improper service procedure and repair any damage. Also, all vehicles will have a plastic guideway installed on each sun visor that routes the wiring away from the attachment screws preventing the wiring from being shorted. Durango built before December 11, 2013 are affected by the recall; vehicles manufactured after this date had the guideway installed during vehicle production.ODI received a total of 6 complaints as of June 2014, all of which reported a fire and two of which alleged related injuries. All 6 incidents occurred while the vehicle was being driven. In each case the driver smelled smoke and witnessed the headliner burning on either the driver or passenger side. Two complaints indicate smoke inhalation injuries to the driver. In response to ODI's information request letters of 8/28/2013 and 2/26/2014, Chrysler
provided consumer and warranty data which was received by ODI on 10/7/2013 and 3/19/2014. Analysis of the Chrysler
data indicated a total of 91 unique incidents related to the investigation of which about two thirds reported an open flame and smoke, the remaining showed only small burn marks with no visible flame. Three Chrysler
reports indicated injuries consisting of minor burns on the customer?s hands and smoke inhalation.The investigation is closed on the basis of the recall action the manufacturer has undertaken which is sufficient to address NHTSA?s con
