did not, at that time, recall its Rear Facing Infant (RFI) seats that used the same buckles.On April 2, 2014, Evenflo
submitted a DIR to recall approximately 1.3 million convertible and booster car seats
produced with QT1 and QT3 buckles. Like Graco

disputed that a safety risk was present in the buckles and claimed that in an emergency situation the entire car seat
could be removed from the vehicle, with the child inside.On October 14, 2014, the Agency contacted Evenflo
, and advised that the Agency was prepared to send a Recall Request Letter if the company did not recall the RFI. As stated to Evenflo
, it was the Agency's position that the car seats
should be recalled because the same QT1 buckles had already been recalled by Graco
in its RFI; the complaint data was sufficient to warrant a recall; and, there was no evidence that any differences in the configuration of Evenflo
RFI compared to Graco
's RFI had an impact on the functioning of the buckle. The Agency had previously described to Evenflo
its serious concerns about the inherent safety risk present when a child in an RFI needed to be quickly removed from a vehicle due to an emergency situation and the buckle was difficult to or would not open.On October 20, 2014, Evenflo
submitted a DIR recalling 202,346 RFI car seats
produced with the QT1 buckles. In its chronology, Evenflo
states that the company had received 10 consumer complaints of difficulty opening the buckle. However, at the time the DIR was submitted the Agency was aware of 71 unique complaints submitted to Evenflo
and there were two additional complaints made directly to the Agency. The DIR also stated that Evenflo
would conduct a Customer Awareness Campaign (CAC) to replace the QT3 buckles used on the non-recalled RFI models. Registered owners of both the recalled seats and the seats covered by the CAC will receive notification letters informing them of the recall and CAC action. Evenflo
's two recalls and CAC action covers all Evenflo
car seats
produced with QT1 and QT3 buckles. Evenflo
's DIR states that it voluntarily recalled its convertible and booster seats in April 2014. Under the National Traffic and Motor Vehicle Safety Act, a manufacturer of an item of replacement equipment is, in fact, obligated to provide notice once the manufacturer learns that the item of equipment contains a defect related to motor vehicle safety. On April 3, 2014 in Howard, Wisconsin, a model year 2014 Freightliner Business Class M2 truck manufactured by Daimler Trucks
North America LLC (DNTA) experienced a catastrophic CNG fuel container burst when it was struck by an unsecured forklift being transported inside the truck's cargo compartment.A contractor working for the National Center for Statistics and Analysis (NCSA) Special Crash Investigations (SCI) Program conducted a field examination of the incident vehicle and the incident scene (SCI Case Number: IN14016). The Office of Defects Investigation (ODI) subsequently sent an information request letter to DTNA and received two responses dated January 23, 2015 and June 24, 2015, respectively.ODI continues to evaluate the CNG fuel system integrity on the subject vehicles and other medium- & heavy-duty CNG vehicles.ODI also continues to investigate CNG fuel system integrity related incidents as they arise. ODI has engaged industry on this subject at various meetings, seminars and conferences, and through involvement in committees hosted by industry and standards development organizations including the Natural Gas Vehicles for America (NGVAmerica) national trade association and the American National Standards Institute (ANSI)/ CSA Group.This Preliminary Evaluation has been upgraded to an Engineering Analysis (EA16-001) so that ODI may gather additional information about CNG fuel system integrity from additional vehicle manufacturers, fuel system integrators and component suppliers and continue to engage with stakeholders to better understand the industry state of the art and the hazard potential.

On July 7, 2014 the Office of Defects Investigation (ODI) opened a preliminary evaluation based on five complaints (VOQs) alleging incidents ofinterference between dislodged exhaust manifold heat shields and the lower steering shaft/knuckle, including one alleging the interference resulted in a crash on a highway entrance ramp. Since opening this investigation, ODI has received four additional owner complaints.One indicated that the shield was removed before binding could occur.The other three occurred at low speed and produced some momentary steering binding that was overcome by the driver when additional steering effort was applied.Once the shield was repaired or removed, the steering returned to normal.The single incident ODI identified as a possible crash (VOQ No. 10578715) related to this condition could not be verified and the description of the crash in the complaint was not supported by the police accident report.No additional crash allegations have been identified related to the alleged defect condition.Information provided by Ford in its September 10, 2014 response to ODI's information request letter identified 10 additional incidents resulting in a low rate of 1.6 incidents per 100,000 vehicles sold in vehicles ranging from 7 to 11 years in service.Six of these incidents were reported by a single source, a state police fleet that corrected the problems in March 2011 and has not reported any additional failures since.Ford
also reported 5 warranty claims, but these incidents occurred at very low mileages and appeared to be caused by loose or missing heat shield attachment hardware rather than the corrosion condition that was the cause of the more recent incidents and the focus of this investigation.This investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist.
