The Office of Defects Investigation (ODI) opened PE14-027 based on a report of 13 front brake hose (jounce hose) failures in 11 vehicles in the District of Columbia Metropolitan Police Department (DCMPD) fleet of 46 model year (MY) 2013 Ford
Explorer Police Interceptor (PI) vehicles (VOQ 10621634).The DCMPD fleet provided Ford
with 71 jounce hoses for analysis, including 4 of the 13 failed hoses.Inspection of each of the failed hoses identified a small tear in the outer cover on the body end of the hoses on the outboard side. Ford
?s lab analysis of these hoses identified splits in the inner liners 1.5-2.0mm from the end of the nipple (outside of the crimped ferrule joint).Ford
conducted pressure testing of 10 randomly selected samples of the remaining 67 hoses that had not experienced leaks and 9 of the samples leaked at 6,000 psi.Ford
?s examination of the 9 hoses that leaked in the pressure tests found 4 that failed on the outboard side of the hose (similar to the 4 leaking field parts) and 5 that failed on the body end of the hose on the inboard side.Four additional non-leaking hoses were sent to the hose supplier, Hitachi, for analysis.Broken yarn was observed in scanning electron microscope examinations at both the body side and caliper side hose to ferrule crimps, indicating severe bending stress had occurred at these locations. Hitachi supplied an additional set of non-leaking field return hoses and a brand new set of hoses to an outside laboratory (Element Materials Technology) for analysis.The lab did not identify any manufacturing or material deficiencies in the hoses and concluded that the hose failures were caused by excessive bending stresses.Based on the examinations of the returned jounce hoses, Ford
conducted durability testing on new hoses that were the same design as those found on the subject vehicles.Ford
first determined the worst case conditions for the hoses; jounce bumpers removed, internal rebound stops fully compressed, and shock absorbers attached. With the jounce hose installed, the steering was rotated to the maximum turn angle while the suspension was translated from full jounce to full rebound.This determined the maximum hose length of 13.0in (331mm) and a minimum hose bending radius of 1.6in (40mm).Using these conditions hoses tested on a Suspension Motion Simulator far surpassed Ford
?s performance specifications. Twenty (20) complaints of front brake jounce hose failure were identified on a total of 18 Explorer PI vehicles, including the 13 hose failures in 11 vehicles from the DCMPD fleet that prompted PE14-027.The remaining 7 complaints included 3 from one fleet that reportedly occurred on the banjo block end of the hose assembly and single complaints from 4 other fleets.Only two brake hose
assemblies were used in the subject and peer Explorer vehicles, with the BB53 level hose used until June 20, 2012, and an updated DB53 level hose used thereafter.No pattern of failures was observed by hose assembly, with 15 of failures involving the BB53 hose assembly (including the 13 from DCMPD manufactured in March 2012) and 5 the DB53 hose assembly built after June 20, 2012. No root cause was determined for the reported Explorer PI jounce hose failures, but Ford
identified improper service repair procedures (e.g., hanging the brake caliper from the brake hose
during brake pad replacement) as a possible contributor to higher than expected rate of front brake jounce hose leaks. The Office of Defects Investigation (ODI) opened this investigation based on a Vehicle Owner Questionnaire (VOQ) alleging a failure of the swing-away hitch pivot bolt resulting in a loss of control.This condition could potentially increase the risk of a crash.During the investigation, ODI collected and reviewed subject and peer vehicle information related to the alleged defect.ODI selected model year 2010, 2012, 2013, and 2014 Tracker Marine trailers with swing-away hitches as peer vehicles.ODI sent an Information Request letter to Tracker Marine in order to collect complaint, warranty, parts sales, and design information.Tracker Marine provided all requested information and provided additional clarification upon request.A thorough review of available data revealed no crashes, injuries, or property damage claims.ODI did identify 11 warranty claims on the subject vehicles and 9 warranty claims on peer vehicles that indicated the hitch failed in some way.Tracker Marine provided additional details about each claim. ODI determined of nine claims which indicated the hitch pin or bolt was replaced, no replacements were due to a structural failure of the pin or bolt.Most of the pins were replaced due to damage to the pull ring, which has no effect on the integrity of the hitch.Another nine claims indicated that the pin or pivot tube was cracked.This would result in excessive play and warn the driver that they needed to have the hitch repaired. The remaining 2 cases had insufficient details. All known failures were identified and repaired before a complete separation or loss of control occurred.ODI also considered part sales for the subject component.While part sales did slightly out number warranty claims, it is impossible to correlate each sale to a particular make/model/model year trailer.The subject components are interchangeable and can be purchased in the aftermarket.The owner who submitted the VOQ to ODI also provided the failed pivot bolt to ODI for inspection.The Vehicle Research and Test Center (VRTC) inspected the bolt and suggested that a drilling operation used to create a lubricant channel possibly contributed to the bolt failure.The VRTC analysis indicates there is potential for improvement within Tracker Marine's production of the swing-away hitch.However, the claim rate suggests the issue is not wide spread and can be detected prior to complete failure.This investigation is closed as further use of agency resources does not appear to be warranted. The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. On May 22, 2013 the Office of Defects Investigation (ODI) opened a Preliminary Evaluation investigation (PE13-017) on Sutphen SPH aerial ladder bucket equipped emergency vehicles for cable failures resulting in unintended bucket retraction.This investigation was closed November 19, 2013 when Sutphen submitted a safety recall 13V-565.At the time of the recall, Sutphen believed the failures were a result of poor maintenance.Sutphen conducted a remedy program where they inspected all of the subject vehicles, repaired any damaged aerial ladder components and trained the owners on the proper maintenance of the equipment.Since the closing of PE13-017, ODI has been made aware (through media reports) of three post recall failures that resulted in 6 injuries.These failures occurred following the implementation of the 13V-565 recall remedy on the subject vehicles.On August 6th, 2014 ODI opened this Recall Query (RQ14-003) on the same Sutphen Corporation SPH aerial ladder bucket equipped emergency vehicles based on the three additional aerial ladder bucket failures.On September 4th, 2014 an ODI investigator, Sutphen engineers, a Hall County technical representative and County Commissioner conducted an inspection of a failed subject vehicle in Atlanta, Georgia.The Fire truck and aerial device were impounded and secured in the maintenance shop where all parties were able to conduct an inspection of the failed components.The Hall county representatives extended the aerial ladder bucket for the ODI and Sutphen inspectors to take measurements and pictures.Also, the failed cables were laid out on the garage floor for inspection. On October 18th 2014, Sutphen purchased the subject fire truck from Hall County for additional analysis and later decided to conduct an additional Safety Recall.Sutphen determined the root cause of the unintended retraction of the aerial ladder bucked was a failure of the cables which extend and retract the ladder.Subsequently a field retrofit kit was developed, analyzed, and tested to ensure that in-service aerials could be restored to a safe operating configuration.This is accomplished by increasing both the size and strength of the cables and limiting the maximum hydraulic pressure in the extension system. This new modification kit will update the field units to meet Sutphen's standards and exceed NFPA requirements.This investigation is closed with safety recall 14V-718.

This Preliminary Evaluation was opened after NHTSA became aware of two fires involving a MY 2011 and a MY 2013 Can-Am Spyder RT manufactured by Bombardier Recreational Products (BRP).Both occurred within days of each other in July, 2014.The first report (VOQ 10615679) involved a 2011 Spyder RT, converted for police work and operated by the Morgantown, WV police department burned while being operated on July 20, 2014.The second report (VOQ10617819) involved a 2013 Spyder RT that burned while being operated near the 29 Palms Marine base in the Mojave desert.After a comprehensive review of all information and test data collected during the investigation NHTSA staff met with BRP to discuss its findings.After careful consideration BRP decided to conduct a safety recall (15V095) to address operator burns from elevated vehicle surface temperatures, fuel boiling due to inadequate management of engine compartment heat, and excessive fuel vapor emissions due to the inability of the bikes evaporative emissions system to capture those vapors.
