In a January 28, 2015 letter to NHTSA, Nissan identified a safety defect in the steering column assemblies used in certain model year (MY) 2008 and 2009 Infiniti EX35, Infiniti FX35/45 and Nissan GT-R vehicles equipped with power tilt/telescope steering columns supplied by Fuji Kiko Co., Ltd (NHTSA Recall No. 15V-054, Nissan R1501/R1502).The recall covers a total of 16,973 vehicles, including 15,647 MY 2008 Infiniti EX35 vehicles manufactured from June 29, 2007 (SOP) to April 25, 2008; 1,100 MY 2009 Infiniti FX35/45 vehicles manufactured from October 31, 2007 (SOP) to April 16, 2008; and 226 MY 2009 Nissan GT-R vehicles manufactured from March 14, 2007 (SOP) to April 25, 2008.The affected vehicles may have been manufactured with a steering column outer tube that is out of specification (out of round), which may result in stress at the upper bearing.According to Nissan, in rare instances, the condition caused the bearing retainer to fracture creating excessive play in the steering wheel.In this condition, if excessive axial force is applied to the steering wheel, the column may separate and cause a loss of steering control.Nissan's remedy instructs dealers to replace the steering shaft on the Infiniti EX35 and FX35/45 vehicles and to replace the entire steering column assembly on the Nissan GT-R vehicles.ODI's analysis of complaint and warranty data related to the alleged defect in the subject vehicles identified a total of 5 incidents in which steering column separation appears to have occurred, including one alleging that a crash resulted.Sixteen (16) additional incidents of loose steering were identified in Nissan complaints (1) and warranty claims (15).The 5 separation incidents identified by ODI involved vehicles built from November 7, 2007 to February 5, 2008, including 3 vehicles built from February 2 to February 5, 2008.The 16 incidents of loose steering involved vehicles built from January 19, 2008 to April 3, 2008, including 9 vehicles built from February 1 to February 11, 2008.None of the incidents involved the approximately 1,800 MY 2008 EX35 vehicles that are not included in the recall, which either were not equipped with the Fuji Kiko power adjustable steering columns (the manual adjustable steering columns were made by a different supplier) or were built after April 25, 2008.This investigation is closed based on Nissan's safety recall.The VOQs associated with this investigation are: 10350255 and 10605536.

Honda timely paid the civil penalty in full on January 20, 2015. Honda also agreed to develop written procedures for comprehensive early warning reporting in compliance with 49 C.F.R. First produced on January 16, 2011, Honda's CBR250R entry-level motorcycle (the subject vehicle) remained in production until being replaced by the MY 2015 CBR300R.In May, 2011, Honda received its first complaint (a field report) concerning subject vehicle engine stalling while decelerating.Honda began a root cause assessment and, in September, 2012, concluded the stalling was due to improperly adjusted engine exhaust and intake valves during engine assembly.Production changes were made to address this issue.The data indicate that subject vehicle engines built with insufficient valve clearances were being corrected during the 600-mile service per the owner's manual maintenance schedule which includes a valve clearance check and adjust.All of the stalling events involved subject vehicles that had not yet had that service completed and most occurred shortly after purchase.ODI's review of Honda's data shows a sharp decline in stalling reports:MY2011 - 48 reports / 43 received prior to October, 2012; MY2012 - 17 reports / 13 received prior to February, 2013; and MY2013 - 5 reports /4 received prior to November, 2013.With the sharp decrease in complaints after February, 2013, no defect trend has been identified, therefore, this investigation is closed. RMD opened this audit query (AQ) investigation in order to better understand and evaluate Chrysler's process(es) for timely and efficient execution of its safety recall campaigns, including its oversight and management of its parts division Mopar.Prior to opening this investigation, RMD had raised concerns with the company related to its execution of two safety recalls, 13V-528 and 13V-529, both of which involved safety defects that could cause the involved vehicles, FCA?s heavier Dodge Ram trucks, to lose steering control suddenly and without warning.Specifically, RMD inquired about the availability of parts for recall repairs, by email and phone on January 15, 2014 and February 19, 2014, due to the volume of complaints it was receiving from owners alleging parts scarcity and parts restrictions. FCA steadfastly denied it had placed parts restrictions on its dealers to manage issues of parts scarcity.FCA shared material showing it had uncommitted stock of parts available for its dealers. Nevertheless, consumer complaints of lack of parts continued.RMD made independent inquiries tosome dealers tobetter understand the conflicting claims between owners alleging lack of ability to repair their defective vehicles due to lack of parts, and FCA?s information showing availability of parts.Dealers informed and provided documentation to RMD on February 21, 2014 and February 25, 2014 showing FCA had, in fact, put into place parts restrictions and, further, had in some instances canceled dealerships? back orders for parts.In addition, during the course of its dialogue with dealers, RMD discovered that FCA had notified their dealerships on February 27, 2014 to return the recall remedy parts over quality concerns, and had therefore effectively canceled its safety recall program without notification to NHTSA. RMD issued a request for information (IR) on October 21, 2014.The company failed to respond to a number of the questions RMD asked that required it to produce specifics surrounding the repairs and parts ordering process with the recalls, including a question about whether a dealer order for recall parts could be canceled and under what circumstances.
