The Office of Defects Investigation (ODI) was in contact with a fleet that experienced multiple incidents of driver and passenger seatbelt buckle assemblies failing to unlatch.The seatbelts, manufactured by Taizhou TIANDA Rope and Belt Co, are original equipment on certain Stallion 900 motorcoaches.Preliminary evaluation suggested that two significantly different buckle assembly designs exist for a single part number 67RB1-04021.At the time of this investigation, failures appear to be limited to the earlier design. Shortly after opening this investigation, Taizhou Tianda Rope & Belt Co. ODI opened PE15-002 based on field reports which reported multiple incidents of air loss in the subject steel wheels.Several of the field reports indicated failures early in the vehicles's service life when used on the steer axle.Accuride identified the subject wheels of the field reports as model 50408 which has two hand holes and an acrylic e-coat finish .Multiple hand hole options and finishes were available from Accuride.Many purchasers selected the most economical finish option which was pretreat + e-coat.The more durable pretreat + e-coat + powder coat came at a premium price however proved to be significantly more durable.To create a more competitive and longer lasting product, Accuride has created an acrylic e-coat with an epoxy e-coat paint option and minimized its availability of pretreat + e-coat only option.A thorough review of the Information Request Response (IRR) identified two separate process conditions which contributed to some wheels loosing air pressure.A part failure in one of the welding machines was the root cause of pin holes in the butt weld. The other process issue involved the surface preparation for paint.Corrosion was evident on the tire side of the rim prior to the first tire change.Further study of the condition revealed that oxidation started below the paint.This indicated that the surface with not thoroughly cleaned and/or protected prior to paint being applied.ODI considered wheel position in its analysis of failures.The steer axle typically experiences more stress and carries a heavier load per wheel/tire than experienced in a dual wheel configuration found on the drive or trailer axles.However, wheel position is rarely identified in the failure reports.This made it impossible to definitively assign responsibility to any one wheel position.Furthermore, of the reports that did include position it was evident that multiple positions experienced failures.ODI reviewed warranty and complaint data as well as surveyed fleets.Although the data did indicate there were events where a slow loss of air occurred as a result of the process failures identified by Accuride, the data did not suggest there was an unreasonable risk to motor vehicle safety.While slow air loss in a tire is undesirable, these wheels are regularly inspected and as a result there are no known events involving a crash, rapid air loss or structural failure of the rim (caused by the alleged defect) after five plus years of exposure.In light of the low risk to motor vehicle safety, further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists.The agency reserves the right to take further action if warranted by the circumstances.

's DIR, a misrouted A/C combo line can be pinched and cut or thermally degraded by the exhaust manifold heat shield, potentially resulting in loss of air conditioning, smoke and/or fire from ignition of leaked A/C refrigerant oil onto a hot exhaust manifold.The recalled vehicles will be inspected for misrouted A/C combo lines and those found to be misrouted will be replaced.FCA
estimates that one percent of the recalled population may be affected by the A/C combo line routing defect.ODI?s analysis of warranty data provided by FCA
in response to the information request letter for PE15-003 identified a spike in claims related to the A/C combo line leakage in vehicles equipped with 2.4L engines built after October 1, 2014.FCA
implemented its final process corrective action for the hose routing concern on June 18, 2015.The 113,034 MY 2015 Jeep
Cherokee vehicles equipped with 3.2L engines have different A/C line routing configurations and are not affected by the A/C combo line routing defect.FCA
provided a total of 220 warranty claims potentially related to the A/C combo line in vehicles equipped with 2.4L engines.Ninety-five percent of the claims (210) involved the 74,574 Jeep
Cherokee 2.4L vehicles built during the recall period.Only 10 A/C combo line claims and no related fires were identified in the 28,250 Jeep
Cherokee 2.4L vehicles that were built before or after the period with the hose routing concern.Analysis of engine compartment fires reported to ODI and FCA
identified a total of 10 fires in MY 2015 Jeep
Cherokee vehicles.Nine (9) of the fires involved vehicles equipped with 2.4L engines and 8 of these involved vehicles built during the production range subject to Recall 15V-676.These include 5 fires that FCA
's investigations determined were most likely caused by the A/C combo line and 3 fires of unknown cause originating in the area of the exhaust manifold heat shield, resulting in fire incident rates in the recall population of 6.6 per 100,000 vehicles for the A/C combo line condition and 10.6 per 100,000 vehicles for fire incidents for which the A/C combo line could not be excluded as the cause.The other 2 engine compartment fires involved a low-mileage fire of unknown cause in a vehicle equipped with a 3.2L engine and a minor self-extinguished fire that FCA

The Office of Defects Investigation (ODI) opened this RQ to investigate allegations that the scope and remedy of safety recall 13V-535 were inadequate. Recall 13V-535 was limited to a relatively small population of Ambulance Package vehicles and ODI received complaints of pickup trucks and Fire Engine Prep Package vehicles being affected by the same defect. Ford manufactured approximately 555,985 subject vehicles between February 22, 2010 and January 30, 2015 that may be affected by defective EGT sensors and Powertrain Control Module (PCM) software calibration. The subject vehicles are equipped with four (4) EGT sensors to monitor engine operating conditions. An EGT sensor fault may produce false exhaust over-temperature signals. If these signals are produced, the Powertrain Control Module (PCM) implements a Failure Management Effects Mode (FMEM) and the driver receives a Stop Safely Now (SSN) system warning on the instrument cluster message center. The FMEM triggers a 70% reduction in available engine power over a period of no more than 45 seconds, during which it may not be possible to accelerate or maintain vehicle speed. The PCM then commands the engine to shut down when vehicle speed reaches approximately 1 mph. There is no immediate restart capability. Depending on the PCM software calibration, ignition key position and ambient temperature conditions, the engine may not restart until the end of a 10-60 minute cool down period.Ford
and its component supplier(s) made numerous changes to the PCM software calibration and the design, material and manufacture of the EGT sensors over the period of subject vehicle production. This continuous product improvement affected vehicle performance regarding the alleged defect. Ford
acknowledges the defect condition in the subject vehicles but argues that an unreasonable risk to motor vehicle safety is limited to Ambulance Package and Fire Engine Prep Package vehicles where commanded shutdown could impact emergency response and patient care. Ford
also argues that the defect trend is in decline. Ford
also initiated a new safety recall (15V-175) to address the 13V-535 remedy concerns in the Ambulance Package vehicles and Fire Engine Prep Package vehicles. The 15V-175 remedy reprograms the PCM with a new software calibration that stops a single EGT sensor fault to cause the vehicle to enter the FMEM shutdown mode.Ford
also initiated an EPA emissions recall (14E03) for all subject vehicles that reprograms the PCM with the software calibration employed in safety recall 15V-175. Finally, Ford
initiated a Customer Satisfaction Program (15M01) providing extended warranty coverage of 8 years or 80,000 miles for the the EGT sensors on certain MY2011-2012 subject vehicles.Due to the EPA emissions recall, that had a 66.6% completion rate in Ford
's December 2015 progress report to the EPA, the defect trend is in decline. ODI is not aware of any crashes, deaths or injuries related to the alleged defect.ODI believes the alleged defect poses a safety risk but notes that the EPA emissions recall is addressing the safety problem.Compelling issuance of a duplicate recall would not represent a wise use of agency resources.Accordingly, this Recall Query is closed.This action is not a finding that no defect exists and NHTSA will take further action if such action is warranted.
