The Office of Defects Investigation opened this Preliminary Evaluation to investigate VOQ complaints alleging a failure of the Occupant Restraint Controller (ORC) module in model year (MY) 2012 Jeep Liberty.The purpose of the investigation was to understand Vehicle Owner Questionnaire (VOQ) complaints stating that the air bag warning light remained illuminated while the vehicle was in operation.In many cases the warning light issue was reportedly corrected by a replacement of the ORC module. In 2013 FCA
issued NHTSA safety recall 13V282 to address ORC module failures caused by an electrical over stress (EOS) condition that damaged an internal resistor.The failure lead to non-deployment of the active head restraints in rear impact crashes of sufficient severity to require deployment.This condition would also result in illumination of the air bag warning light and set a B2208 diagnostic trouble code (DTC).Recall 13V282 involved two different remedies based on the model year and model of the recalled vehicle.Some models were repaired by replacing the ORC with an updated unit.However the subject vehicles of this investigation, which made up the largest population in the recall, received a software update to the Totally Integrated Power Module (TIPM), another electronic device that is involved in powering of the ORC. After the investigation was opened, FCA
began collecting failed subject vehicle ORC modules so they could be analyzed for failure causation.From June 2017 to August 2017 FCA
obtained 13 ORC modules and analyzed 10 of them.FCA
determined 9 contained evidence of the resistor over stress condition.Of that group, 7 had experienced resistor failures prior to the TIPM being re-flashed under the recall action.In these cases, replacement of the ORC module would have been the only effective repair to extinguish the air bag warning lamp.This observation correlates well with VOQ complaints ODI received from consumers who experienced an airbag warning light illuminated when they presented their vehicle in for the original 13V282 repair.Many of these consumers were told the TIPM re-flash did not fix the warning light issue, and that their ORC module needed to be replaced (at the consumer's expense). ODI notes that 8 of the 10 analyzed ORCs displaying the resistor EOS condition also had DTC B1BCF stored.This code is related to the detection of a possible internal fault with a rollover sensor.FCA
conducted additional testing and concluded that the presence of the resistor failure caused the subsequent setting of the this DTC. On October 4, 2017 FCA
decided to conduct a safety recall to replace the subject vehicle ORC modules with a redesigned device.FCA
submitted the defect information report to the agency on October 10, 2017. The Office of Defects Investigation (ODI) is closing this Preliminary Evaluation and upgrading to an Engineering Analysis (EA) 18-001.During the PE, ODI requested and reviewed information provided by Nissan including consumer complaints and other field and technical data related to failures of the hydraulic control unit (HCU), an electro-mechanical brake system component that provides antilock braking and other functionality in the subject model year (MY) 2009 Murano.The subject failure of the HCU, which involves improper operation of an internal hydraulic control valve, can cause extended brake pedal travel and may result in a reduction in overall braking effectiveness.Tier-one supplier Continental
manufactured the HCU for Nissan
.ODI currently recognizes 484 subject vehicle complaints that appear related to the alleged defect.Fourteen crash allegations, with three non-life threatening injuries are under evaluation also.ODI has been monitoring consumer complaints received, and in some cases has obtained suspect HCUs from complainants' vehicles.Recovered HCUs sent to NHTSA's Vehicle Research and Test Center (VRTC) in East Liberty, Ohio are under evaluation, and VRTC has conducted limited vehicle testing also.In a July 28, 2017 submission, Nissan
acknowledged that a failure of the HCU can result in increased brake pedal travel during braking.Nissan
's analysis of returned parts revealed that a zinc coating used on the normally closed valve assemblies inside the HCU may react with certain brake fluid
constituents.The reaction results in the formation of a gelatinous material that can harden with time and prevent the valve from closing after opening in an ABS
event.When this occurs, Nissan
maintains that the hydraulic brake system remains intact and that full braking is available once the pedal travels an additional distance, about 36 mm according to Nissan
.Although still under investigation, Nissan
states it has not determined the condition represents an unreasonable risk to motor vehicle safety, citing low rate and vehicle age as additional factors.In March 2018, Nissan
began conducting a customer satisfaction campaign on certain MY 2009 Murano to service (flush and replace the brake fluid
), and then test and replace the HCU as needed.The campaign is being conducted free of charge to customers.During the EA, ODI will request VRTC continue to evaluate the cause and consequence of HCU failures, including the affects of multi-valve failures.ODI will further study the potential scope of the alleged defect as it understands similar HCUs and brake fluid
is used in other MY Muranos, and potentially in other Nissan
models as well.ODI notes that improved valve coatings were introduced on Nissan
HCUs at some subsequent stage.

The focus of the investigation was to determine whether actions performed by General Motors
adequately addressed any unreasonable risk to motor vehicle safety in the subject vehicles concerning the headlights and the HDM that controls them.Under normal electrical load and environmental conditions, the HDM can overheat inside the Underhood Electrical Center (UEC). This can cause the module to fail and result in a simultaneous loss of both low beam headlights and both daytime running lights (DRLs), often with no warning or prior detectability to the driver. This failure can be intermittent or permanent. No other exterior lights are affected. High beams, turn signals, brake lights, and reverse lights are still operational.General Motors
addressed this issue with 2 recalls (14V755 & 15V519) and a special coverage program (N172091910). In view of the recalls and special coverage program implemented by GM and the vehicle high beams continuing to operate, ODI has tentatively concluded that the actions taken by GM mitigate any unreasonable risk to safety posed by HDM failure in the subject vehicles.Accordingly, the investigation is closed. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take additional action if warranted by new circumstances.The ODI reports cited above are available for review online at NHTSA.gov under the identification numbers in the attached document.
