Summary

The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE) based on twenty-two (22) Vehicle Owner Questionnaires (VOQs) for soft rear brakes caused by air seeping into the rear brake lines of model year (MY) 2012-2020 DucatieBay Monster, Multistrada, SuperSport, Panigale, Scrambler and Diavel motorcycles.The consumers were alleging that the rear brakes would not properly function unless the rear brake pedal was applied several times or that there was a total loss of rear braking during normal applications, increasing the risk of a crash. In a March 2020 meeting, DucatieBay indicated that they were aware of certain models in which air may enter the motorcycles' rear braking system through the brake lines, due to exposure to heat from the exhaust system. Although this condition compromises rear braking performance, DucatieBay stated that they did not consider this to be a safety defect because the front and rear brake systems are hydraulically independent from each other, and a motorcycle's front brake system performs most of the stopping. Additionally, they believed that routine brake fluideBay replacement would resolve consumers' concerns, however complainants and dealership service technicians reported that, in some cases, after a dealer serviced the rear brake system, the rear brake pedal would feel soft and spongy again (indicating compromised brake performance) less than a year later--well before the recommended two-year brake fluideBay service interval. As a precaution, DucatieBay issued a series of technical service bulletins (TSBs) to replace the brake lines with redesigned ones, rerouting them away from the exhaust system.ODI's position is that all of the brakes on a motorcycle are considered necessary for safety. Analysis of the data identified that neither the entire model line nor the entire model year range of motorcycles covered by the investigation were affected, but rather certain sub-models or narrower model year ranges of motorcycles incorporated brake lines with a higher exposure to the heat from the exhaust system. Because of the unique routing of each model's brake line, some motorcycles are more susceptible to the condition than others. As these specific models and model years were most likely to exhibit an increased risk of a crash from the lengthened stopping distance, DucatieBay converted the previously issued TSBs into a safety recall.After receiving a total of 94 VOQs, ODI is closing this PE with DucatieBay's safety recall 21V-315. With the recall action taken by DucatieBay, this investigation is closed. However, the closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other models or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.

Summary

The Office of Defects Investigation (ODI) has completed its review of a defect petition received on November 20, 2020 requesting an investigation into the failure of reversing lights to illuminate in certain Model Year (MY) 2013 PorscheeBay 911 Carrera vehicle with manual transmissions. The reversing lights fail to illuminate when the subject vehicles are shifted into reverse.The reversing lights provide a warning to other drivers and pedestrians when the vehicle is reversing. The reversing lights also provide some illumination for the driver when reversing at night. Failure of the reversing lights can cause an increased hazard to the vehicle, its occupants, surrounding vehicles and nearby pedestrians. As of April 14, 2021, NHTSA has identified only one consumer complaint alleging a failure of the reversing lights in NHTSA's database. This complaint was received from the petitioner. Neither NHTSA nor PorscheeBay have received any allegations of crashes or injuries associated with the failure of the reversing lights to illuminate.PorscheeBay determined that the reversing lights on the MY 2012-2015 PorscheeBay Carrera vehicles were becoming inoperative due to the failure of the transmission side actuation mechanism. PorscheeBay determined the weld between the shift rod and the stopper plate was below manufacturing specifications in some vehicles due to production weld process variations. PorscheeBay modified production weld specifications to improve the strength and consistency of the weld joint between the shift rod and stopper plate. In January 2021, PorscheeBay initiated a Service Action (228/20) on MY 2012-2015 PorscheeBay Carrera vehicles with active notification of potentially affected customers to remedy the reversing light failure in the affected vehicles. Warranty coverage on the affected components has been extended to 15 years from the new vehicle delivery date with unlimited mileage. PorscheeBay will replace the transmission in the affected vehicles with a new transmission with the modified weld specifications for the shift rod - stopper plate joint. NHTSA believes there is a high likelihood that many affected vehicles will be remedied with this action.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA's possession, information submitted by PorscheeBay in response to an Information Request letter and the potential risks to safety implicated by the petitioner's allegation, as well as Service Action (228/20) initiated by PorscheeBay offering extended warranty coverage, NHTSA does not believe that a formal investigation is warranted. After full consideration of appropriate factors, the petition is denied. Summary Description: Trolley bus circuit breaker fires


Summary

In a July 7, 2020 letter, Los Angeles World Airports (LAWA) informed the National Highway Traffic Safety Administration (NHTSA) of three separate fire incidents resulting in the total loss of eight of its Spirit of America trolley buses. These buses were manufactured by ARBOC Specialty Vehicles, LLC (Arboc) on incomplete chassis from FreightlinereBay Custom Chassis Corporation. Considering the safety concerns and substantial losses, LAWA grounded the remaining buses in its fleet. OnJanuary 15, 2021, NHTSA opened an investigation to determine if a defect in the subject vehicles exists and presents an unreasonable risk to vehicle occupants and nearby people or property. On February 24, 2021, the Office of Defects Investigation (ODI) sent an Information Request (IR) letter to Arboc. On April 8, 2021, ODI received an IR response from Arboc that included information on vehicle production, consumer complaints, field reports, warranty claims, manufacturer actions, vehicle changes and other data related to the alleged defect. On June 29, 2021, Arboc sent NHTSA a copy of an investigation report/update (Investigations into LAWA Thermal Events) done by a third-party investigation firm (hired by the chassis manufacturer) that included an assessment of the alleged defect/fires. ODI also engaged in discussions with the manufacturer about the alleged defect on multiple occasions during this investigation. Upon analysis of available data, ODI has not determined the existence of a safety defect. The third-party investigation report points to the 30-amp recessed circuit breaker as the hottest point in the vehicle during the December 21, 2019 fire (first fire) and suggests that it could have been a root cause for that fire. However, that report is inconclusive.Based on available information, the causation of the two subsequent fires appears unrelated to the circuit breaker. The second fire (February 2020) appears to have been caused by technician error during repair of fleet vehicles by Arboc pursuant to TSB AR 875. Information provided by Arboc from the third-party investigation indicates that the third fire (June 3, 2020) was likely caused by the intentional or negligent acts of individuals who gained unauthorized access to LAWA property and the trolleys themselves. NHTSA is not aware of fires other than the three described above in the LAWA fleet. There are other fleets with the same make and model trolley buses. Arboc has also previously taken several actions related to the subject vehicles. On June 9, 2020, Arboc issued TSB AR875 to secure the 30-amp recessed circuit breaker. On June 17, 2020, Arboc issued Recall 20V-335 to address misrouted power cables that may short circuit, increasing the risk of a fire. Since June 3, 2020, there have been no reports of vehicle fires and no reports of crash, injury, or death. Based on the available information, NHTSA has closed this investigation without finding a safety-related defect. The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The Agency reserves the right to take further action if warranted by the circumstances.

Summary

In January 2021, the Office of Defects Investigation (ODI) opened a Recall Query (RQ21-001) to assess allegations of post-recall remedy uncommanded tailgate openings in model year (MY) 2017-2019 F-250/350/450 FordeBay trucks. Besides the recall remedy launched in January 2020, FordeBay also implemented a production update in November 2019. At the time of the RQ opening, ODI had received eleven (11) Vehicle Owner Questionnaires (VOQs). ODI sent an Information Request (IR) to FordeBay in February 2021. In numerous meetings after receiving their IR response, FordeBay highlighted the drop in claims since the recall and production update and communicated how they were unable to verify the owners' complaints or replicate the concern. Despite this, FordeBay began developing ways to prevent uncommanded tailgate openings from various human, mechanical or electrical causes. Based on testing, FordeBay issued a Customer Satisfaction program, 21M05-S1/S2/S3, to further address the potential of unintended tailgate openings. This work was applied to vehicles being produced as of December 20, 2021. The program began for already delivered vehicles on July 22, 2022. The covered models include 2017-2022F-250, 350, 450, 550, 650 and 750 Super Duty and 2020-2022 F-600 Super Duty trucks. ODI has received 39 VOQs between February 24, 2020, and December 15, 2023, alleging uncommanded tailgate openings. In a detailed analysis, none of the vehicles in the VOQs were found to have received the Customer Satisfaction update. Since ODI has not received any complaints of post-Customer Satisfaction program tailgate openings, this office is closing this Recall Query. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances. On January 8, 2021, after ongoing review with MercedeseBay Benz, the National Highway Traffic Safety Administration (NHTSA) Office of Defects Investigation (ODI) opened PE21-001 to assess allegations of front roof panel detachments while driving at highway speeds in certain 2015 Mercedes-BenzeBay (MercedeseBay) C-Class vehicles.The subject front roof panel is a stationary panel composed of polycarbonate and is attached to the vehicle via adhesive at the time of manufacture. The panel is found on subject vehicles equipped with panoramic sunroofs. Over time, the subject panels may exhibit cosmetic degradation leading to replacement of the panel, a procedure which entails: removal of the old panel and adhesive, site preparation, and installation of a replacement panel with new adhesive.The consumer complaints that prompted the opening of PE21-001 all described detachments of front roof panels that had been replaced after the vehicle was put into service.MercedeseBay' internal investigation identified a broader scope of affected vehicles (2014-2020 CLA, GLA, C, S, GLC, and GLE class) and found the following potential service procedure- related causes of front roof panel detachments:- Improper surface pretreatments such as the use of the wrong primer or cleaner or scoring of the surface; or - Use of the incorrect adhesive.These conditions lead to a front roof panel bond that, even if sound at the time of repair, cannot maintain its strength over time and environmental exposure.MercedeseBay updated its service and parts ordering systems to reinforce the proper use of specified kits and procedures via webinars, shop foreman meetings, and explicit reference to the risks incurred by diverging from the repair procedures. MercedeseBay is also furnishing parts ordering assistance, and will hold dealerships accountable for improper repairs.ODI's analysis of the ten complaints it has received from the public (VOQ's) and MBUSA's complaint data submitted in response to the information request letter for PE21-001 identified a total of 57 non-duplicative reports alleging incidents in which the front roof panel detached from the vehicle while in motion. Included within the 57 reports are incidents where consumers state the panel detached ...and flew off into traffic.On March 19, 2021, MercedeseBay notified NHTSA that it would recall subject vehicles with front roof panels known to have been replaced after the original sale of the vehicle. In total, the recall (designated as Recall 21V197) covers 15,099 vehicles that were ever registered in the United States.
