This investigation was opened based upon allegations made by the New York City Transit Authority (NYCTA) regarding the operation of a subject lift installed on one of itsMCI motorcoaches.On March 6, 2012, an occupied powered wheelchair rolled off an elevated subject lift, injuring the occupant.While looking into this incident, ODI found that NYCTA personnel had failed to utilize the original equipment wheel chair restraint (an interlocked safety belt in this case).Normally, failing to use the belt would mean the lift could not be raised.This is because, to comply with FMVSS 403, S6.4.7.1 and S7.7.2.2, the lift manufacturer's (Ricon) wheel chair retention device incorporates an interlocked belt.With the required interlock, if the belt is not latched, the lift will not operate.However, NYCT failed to use the belt and intentionally disabled the interlock.ODI is unaware of any other incidents of wheelchair roll-away involving the subject lifts. Given these facts, a defect trend related to the subject lifts wheel chair retention system has not been identified and this PE is closed.

ODI has completed an extensive analysis of rear crash fuel tank system integrity data for the subject Jeep Cherokee, Grand Cherokee and Liberty vehicles (SVs).As of June 2013, ODI knew of 56 post-collision fatal fires, 28 non-fatal fires, and 6 fuel leak incidents (totaling 90 incidents, 75 fatalities and 58 injuries) involving the SVs.Based on this analysis, ODI issued a Recall Request Letter (RRL) on June 3, 2013 requesting that Chrysler
recall the Grand Cherokee and Liberty vehicles due to the rate of fatal, non-fatal fire, and fuel leak incidents when compared to peer vehicles (compact and medium SUVs) built during the same time period.In the RRL, ODI described how the location of the aft-mounted fuel tank made the SVs vulnerable to rear impact crash fuel tank failures.During the analysis, besides the high speed/high severity crashes present in both the SV and peer vehicles, ODI observed a significant number of low and moderate speed rear impact crash related fires and leaks, particularly in the Liberty.The data also demonstrated that SVs originally equipped with towing hitch receivers (trailer hitches) appeared to be under-represented in rear-impact related fuel tank failures.In the RRL, ODI requested that Chrysler
provide its response by June 18, 2013, otherwise NHTSA might proceed to an Initial Decision that the SVs contain a safety-related defect.Chrysler
responded to the RRL on June 4, 2013 and vigorously disputed the tentative findings of the RRL.Among other things, Chrysler
argued that the SVs had an overall safety record superior to their peers, met or exceeded all applicable federal motor vehicle safety standards (FMVSS) and only experienced fires in severe high energy rear impacts.Nonetheless, while continuing to maintain that the SVs did not contain a safety-related defect, Chrysler
proposed a recall (13V-252) of approximately 2.5M (1.6M currently registered) model year (MY) 1993 - 1998 Grand Cherokee (ZJ) and MY 2002-2007 Liberty (KJ) vehicles on June 18, 2013.The recall remedy submitted by Chrysler
would employ a hitch receiver assembly consisting of a steel cross-member behind the fuel tank with forward-projecting arms bolted to the frame rails on either side of the fuel tank.For the MY 1999 - 2004 Grand Cherokee, Chrysler
indicated it would conduct a customer satisfaction campaign and inspect vehicles equipped with aftermarket tow hitch receivers, and if necessary, replace any such hitch receivers whose conditionmay increase the risk of fuel system failure in rear crashes.Examination of the available data established that the MY 1999 - 2004 Grand Cherokee did not pose the same magnitude of safety risk as the MY 1993 - 1998 Grand Cherokee and MY 2002-2007 Liberty, particularly in low and moderate speed rear impacts.Because the agency has concluded that the vehicles do not present an unreasonable risk to safety, ODI is closing its investigation of the MY 1999 - 2004 Grand Cherokee.Although it was not within the scope of the Petition initiating this investigation or the Preliminary Evaluation preceding this EA, ODI also examined the performance of the MY 1993 - 2001 Cherokee as part of this investigation.NHTSA's assessment of the available data for the Cherokee did not establish an unreasonable risk in comparison to peer vehicles.ODI does not approve proposed defect remedies.While offering to install hitch receiver assemblies on the MY 1993-1998 Grand Cherokees and 2002-2007 Liberty, Chrysler

9-7x vehicles originally sold or currently registered in the salt belt states of Connecticut, Delaware, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, Wisconsin, and the District of Columbia. The recall remedy requires the inspection and repair or replacement of the master power window switch, which GM refers to as a driver door module (DDM). At the same time, GM implemented a special coverage program to replace failed DDMs for vehicles registered outside of these states. ODI was not satisfied with GM's exclusion of non-salt belt vehicles and continued its investigation. On June 13, 2013, GM submitted a related recall notification (13V-248) to reclassify the initial special coverage program into a safety recall for vehicles that were not previously repaired under special coverage.The Office of Defects Investigation (ODI) opened Engineering Analysis EA12-004 on June 11, 2012 to further investigate allegations of DDM thermal incidents in model year (MY) 2006-2007 Chevrolet
Trailblazer vehicles. Thermal incidents involve cases where fire-related events such as smoke and melt are present. ODI has received 170 reports alleging a thermal event occurred. Complainants stated that the DDM melted and in some cases produced odor and/or smoke. Fourteen of these incidents led to a fire where visible flames were reported or the damage extended significantly beyond the DDM. In addition to the 170 reports listed above, ODI received 63 reports alleging the DDM acted erratically or became inoperative. In response to ODI's Information Request (IR), GM provided 619 unique consumer complaints that related to the alleged problem. Seventy-seven of those reports led to a fire with flame, and 4 reported injuries of smoke inhalation and/or slight burns from contact with hot switches. GM also submitted 20,400 warranty claims for replacement of the DDM, most of which involved inoperative switches. The problem condition is caused by fluids, such as rain water, entering the driver's door module and subsequently causing corrosion that could result in an electrical short in the printed circuit board. A short may cause the power door lock and power window switches to function intermittently or become inoperative. However, the electrical short may also cause overheating that could melt components of the door module, producing odor, smoke or in some cases fire with flame. The circuit board was redesigned and GM implemented the new design into vehicle production on May 14, 2007. All failures cited above occurred in vehicles produced before this build date (i.e., with the original design DDM). GM also reported that all MY 2006-2007 GMT360/370 platform vehicles and MY 2005 SAAB
9-7x vehicles utilized the same DDM and therefore ODI expanded the subject population to include these vehicles.This investigation is closed on the basis of GM's announcement of safety recalls 12V-406 and 13V-248.
