In a letter dated April 18, 2012, Specialty Manufacturing, Inc. (SMI) notified the NHTSA that certain ProLo bus roof escape hatches, installed as original equipment on various school buses and transit buses, may fail to conform to Federal Motor Vehicle Safety Standards (FMVSS).These hatches, shipped between April 1, 2011, and March 25, 2012, experience increased friction between the rear plate assemblies and the nylon rear pop-up support, resulting in an opening force that exceeds FMVSS No. 217, Bus Emergency Exits and Window Retention and Release.In the event of an emergency, a noncompliant hatch may not provide a means of readily accessible egress, increasing the risk of injury to the vehicle occupants.NHTSA assigned recall number 12E-017 to this matter.On July 12, 2012, RMD mailed information request (IR) letters to five (5) manufacturers who had not yet conducted safety recalls for this issue.Collins Bus responded that they did not incorporate these escape hatches into any new buses but instead sold them to dealerships.Collins noted that they would notify the dealers of SMI's recall.The other four (4) manufacturers filed Part 573 Reports and conducted safety recalls for their affected vehicles.The following recalls were filed as a result of this EQ:Turtle Top:12V-330 Elkhart Coach:12V-348 Girardin Minibus, Inc:12V-362 Glaval Bus:12V-384As RMD received adequate responses from all the companies it contacted, this Equipment Query is closed.

The Office of Defects Investigation (ODI) received Early Warning Reporting (EWR) data from the manufacturer that indicated some number of subject vehicles had allegedly experienced a fracture of one or more support brackets resulting in an unsecured air brake reservoir.This could result in a loss of air pressure necessary for proper service brake application and a potential road hazard if the reservoir separates from the vehicle in operation.By letter dated August 15, 2012, Daimler Trucks North America LLC (DTNA) has notified ODI that it has initiated a safety recall (identified by NTHSA Recall No. 12V-405 and DTNA Recall No. FL-631) to address a safety defect in approximately 1,663 model year (MY) 2009 through 2012 Freightliner 114SD, Business Class M2, Cascadia, Coronado, Sterling 9500 and Western Star 4700 trucks manufactured from July 7, 2008 through December 15, 2011, originally equipped with a specific perpendicular oriented air brake reservoir support bracket (P/N 12-22981-000).To date ODI is unaware of any bracket failures that resulted in a complete separation of the reservoir from the vehicle.Affected owners will be notified by DTNA beginning October 15, 2012 and affected vehicles will have the subject air brake reservoir support brackets replaced with a more robust bracket.Repairs will be performed by DTNA authorized service facilities.Copies of DTNA's reimbursement plan for pre-notification remedies will be submitted to ODI as a supplemental report when available.Accordingly, this investigation is closed.

On May 27, 2014 and amended June 2, 2014, Ford Motor Company (Ford) submitted a Defect Information Report (DIR) to NHTSA describing a safety defect that may result in a sudden loss of power steering assist in approximately 179,027 model year (MY) 2011 through 2013 Ford Explorer vehicles equipped with electric power assisted steering (EPAS), including 82,328 MY 2011 Explorersthat are the subject of PE12-017.Ford has assigned the recall number 14S06.The NHTSA recall number is 14V-286.Ford's DIR indicates that loss of power steering assist while driving would require higher steering effort at lower vehicle speeds, which may result in an increased risk of a crash.Sudden loss of power steering assist while driving can occur in the subject vehicles if the system loses electrical power or whenever the system detects a fault that requires it to enter fail-safe mode, which removes power from the EPAS motor and defaults to manual steering.Ford's DIR indicates that the majority of steering assist failures in the recalled vehicles have been caused by an intermittent electrical connection in the Power Steering Control Module (PSCM) that can lead to a loss of the motor position sensor signal.Ford's remedy instructs dealers to check the PSCM for diagnostic trouble codes (DTC).If upon initial inspection a loss of steering assist DTC is present, dealers will replace the steering gear at no charge to the owner and update the PSCM with revised software.Following detection of a motor position sensor signal fault, the revised software will provide audible and visual warnings to the driver that a power steering system fault has been detected and will maintain steering assist for the remainder of that drive cycle.If no DTC is present, dealers will reprogram the PSCM with the revised software. Analysis of information from all sources identified a total of 969 complaints and 4,059 warranty claims related to loss of power steering assist while driving in the MY 2011 through 2012 Explorer vehicles, resulting in a complaint rate of 5.4 incidents per thousand vehicles and a warranty claim rate of 2.3 percent.ODI identified 15 crashes with evidence indicating loss of power steering assist may have been a factor.All 15 crashes involved low-speed impacts with roadside objects during turning maneuvers, resulting in minor vehicle damage or no damage.In one incident that occurred in a low-speed curve, the driver attempted to brake in response to the sudden increase in steering effort and inadvertently applied the accelerator pedal instead.This caused the vehicle to run off the side of the road and strike a wooden pole at approximately 15 mph.Another incident occurred when the driver was unable to to negotiate a left-turn at an intersection and ran off the road into a shallow ditch, resulting in moderate front-end damage to the vehicle and minor injuries to the driver that did not require medical treatment. During PE12-016, Chrysler identified a condition in model year (MY) 2011 Chrysler 200 and Dodge Avenger vehicles equipped with 3.6L engines that may result in engine stall when stopped or travelling at very low speeds.According to Chrysler, this condition occurs when the Powertrain Control Module (PCM) initiates a purge monitor check while the engine is at idle which can result in an overly rich vapor condition and cause the engine to stall while stopped.Chrysler developed a software countermeasure to address the condition and has initiated a customer satisfaction campaign to reprogram the PCM on all affected vehicles. The bulletin was sent to dealers on September 28, 2012 and the owner letter mailing began on October 5, 2012.See the investigative file for copies of Chrysler's bulletin and owner letter.The Office of Defects Investigation (ODI) analyzed complaints related to engine stall that were provided by Chrysler or submitted to ODI from consumers.The predominant failure mode identified involved stalling when stopped or travelling at low speeds.The complaint counts shown in this resume include incidents in which enough detail was available to verify the symptoms were related to the defect condition identified by Chrysler.Two complaints alleged engine stall resulting in collisions, one involving very minor damage and one involving a single vehicle road departure crash in which the vehicle was totalled.In the first incident, the vehicle stalled while stopped at an intersection and rolled back into a following vehicle resulting in a minor scratch in the rear bumper.In the second incident, the vehicle was totalled from damage sustained from striking a tree after failing to negotiate a sharp turn in wet road conditions.The police report does not include any references to stalling as a cause or contributor, but cites excessive speed entering the turn.Although there was evidence or prior history with stalling when stopped, there is no evidence that engine stall was a factor in the crash.The condition that is causing the majority of stalling incidents in the subject vehicles occurs at a stop or very low speed and allows the vehicle to be restarted immediately.The condition represents a low risk to motor vehicle safety and is adequately addressed by Chrysler's service campaign.
