Summary
On March 17, 2023, the Office of Defects Investigation (ODI) opened PE23-004 to investigate instances of drag link separation in model year (MY) 2016-2020 Vision school buses manufactured by Blue Bird Body Company (Blue Bird). Since drag links are serviceable items that can wear out, ODI opened this investigation to understand why two drag links separated at the steering arm end shortly after having passed a routine inspection by multiple qualified entities.Two buses used in the same rural school district, owned and maintained by different bus contractors, had both undergone similar annual and semi-annual inspections prior to the start of the school year.Both failed within a week of each other, approximately a month into the school year. ODI interviewed the bus contractors and surveyed the industry to learn what inspection methods were being used and which ones were best at detecting wear in the components.ODI learned that the failed buses had not been inspected using the most thorough methods. ODI collected the drag links from each of the failed buses. Assisted by the Pennsylvania State Police Motor Carrier
Enforcement Division and using best practice inspection methods, ODI identified and collected a pre-separation drag link from a neighboring school district. The drag links underwent a teardown inspection at the Vehicle Research and Test Center (VRTC) jointly with Blue Bird and THK-Rhythm Automotive, the drag link manufacturer.In all cases, the visual analysis of each drag link end revealed that the grease at the steering arm end was dirty, discolored, and dry in texture, while the pitman arm end was still clean looking and fluid. ODI reviewed the information request data submitted by Blue Bird.No other separations had been reported.ODI contacted additional fleets to determine whether any other separations had occurred that had not been included in the submitted warranty and claim data.None were found. The grease fitting on the steering arm end is obscured by the steering arm itself.Therefore it is believed that inadequate maintenance resulted in the failures that had not been detected during the less-thorough prior inspections.Blue Bird has agreed to issue a notification to all owners to clearly identify all lubrication points and ensure all owners understand the proper lubrication and inspection methods.Blue Bird will also update their service manuals to convey the industry best practices of having someone turn the steering wheel while someone else inspects the drag links while the wheels are on the ground. Since the drag link failures appear to be caused by maintenance and inspection deficiencies and not due to a design, material or manufacturing issue, this investigation is closed.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency reserves the right to take additional action if warranted by future circumstances.

Summary
On March 17, 2023, The Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE23-005) to access the alleged defect of momentary increase in steering effort in model years (MY) 2022 and 2023 Honda Civic vehicles. The complaints report that the momentary increase in steering effort (described as "sticky steering") occurs mostly at highway speeds after driving for a certain amount of time. The reports have been received over the past 2 years with most occurring with low vehicle mileage. The steering gear contains a unit that includes a worm gear and a worm wheel. Honda stated this condition of momentary increase in steering effort occurs due to two factors within this unit. During manufacturing, the worm wheel goes through annealing and component conditioning processes. These processes caused internal stress and strain within the worm wheel. This strain was slowly released over the first few months of the vehicle life. Over time, the released strain caused the deformation of the teeth on the worm wheel, causing the worm gear to catch on the worm wheel. This results in the consumer's momentary increased in steering effort. Also, the manufacturing process did not guarantee consistent grease application and therefore, some vehicles within the scope received too little grease which contributes to the momentary increase in steering effort. Analysis of all relevant data indicates that the subject condition occurs early in the vehicle's life primarily in winter months. Additionally, the subject vehicles need to be driven in a straight line for a period of time, possibly until the vehicle is warmed up, to recreate the condition. The condition does not illuminate a malfunction indicator light (MIL). Some complaints allege Honda dealerships are unable to recreate the condition or state this is a normal vehicle operation. However, Honda released Service Bulletin 23-037 in July of 2023 which accurately describes the condition. To address this issue, Honda directs dealerships to remove the electronic power steering (EPS) gearbox and replace with a new gearbox. Further, Honda stated that the worst case steering effort from all warranty returned parts tested was 2.4 pounds. ODI complaint traffic remains steady. ODI has received 13 crashes to date, 11 of which allege roadway departure due to not being able to overcome the momentary increased steering effort prior to their vehicle leaving the roadway. The remaining 2 incidents claim overcorrection of the steering wheel. PE23-005 has been upgraded to an Engineering Analysis (EA23-003) to further assess the scope, frequency and potential safety related consequences of the momentary increase of steering effort. Further the scope has been expanded to include assessment of the Acura Integra and Honda CR-V models. The Office of Defects Investigation (ODI) is aware of two reports of complete detachment of the steering wheel from the steering column while driving in 2023 Model Year Tesla Model Y vehicles.Both vehicles were delivered to the owners missing the retaining bolt (P/N1036655-00-A) which attaches the steering wheel to the steering column. Both vehicles received an end of line repair requiring removal and re-installation of the steering wheel. A friction fit maintained the connection between the steering wheel and the column splines during operation until separation. Sudden separation occurred when the force exerted on the steering wheel overcame the resistance of the friction fit while the vehicles were in motion. Both known incidents occurred at low vehicle mileage. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition.
