Summary Description: Power steering hose failure


Summary

The Office of Defects Investigation (ODI) analyzed complaint data provided by ChryslereBay as well as complaints submitted to ODI from consumers. ODI's analysis identified a total of 24 unique reports indicating power steering hose blow-offs resulting from a dislodged power steering cooler turbulator fin in some early production vehicles.According to ChryslereBay, a dislodged fin can occur when the turbulator is inserted into the power steering cooler or loaded into the assembly machine.A dislodged fin may block the flow of power steering fluid creating excess back-pressure in the power steering system leading to the possibility of a return hose blow-off.ChryslereBay and its power steering cooler supplier, DanaeBay, modified the cooler assembly process to address factors that may contribute to hose blow-offs. The redesigned turbulator assembly stations were completely implemented in production by April 2012.During PE12-020, ChryslereBay identified occurrences where the power steering return hose may blow-off at the power steering cooler.This is a condition that typically occurs early in the life of the vehicle with 65% occurring at less than 2,000 miles and 96% occurring with less than 8,000 miles. The data also shows that 88% of the conditions occurred with less than 4 months in service. Over 50% of the blow-off complaints occurred on vehicles with build dates between November 22, 2011 and December 23, 2011. None of the conditions occurred with more than 12,000 miles or 7 months in service. ChryslereBay?s complaint analysis indicates up-to 14 additional vehicles may experience the hose blow off condition.None of the 24 unique reports with the hose blow-off condition alleged smoke or fire in the engine compartment. In the event of a blow-off, it is highly unlikely for power steering fluid to reach any of the ignition sources in the engine compartment and result in an engine fire. Complaint analysis identified five reports alleging engine compartment fires that resulted from other or unknown causes, including one attributed to an overheated transmission fluid. Two fires of unknown cause cited in the opening resume for PE12-020 involved vehicles equipped with V8 SRT 6.4L engines which are not equipped with the steering system affected by the turbulator fin concern. ODI will continue to monitor field experience in JeepeBay Grand Cherokee vehicles equipped with that engine.There have been no reports of loss of steering control, crashes or injuries as result of this condition.Additionally, all affected vehicles that may have experienced a hose blow-off condition were repaired under ChryslereBay?s 3 year/36,000 mile manufacturer?s warranty at no cost to the owner and supplier?s process improvements put in place have eliminated the likelihood of power steering hose blow offs. There is no indication of loss of motive power or unreasonable safety risk associated with the alleged defect in the subject vehicles. In a letter dated October 2, 2012, ChryslereBay Group LLC (ChryslereBay) submitted a Defect Information Report (DIR) to NHTSA identifying a safety defect in approximately 48,058 vehicles built with an axle pinion nut that can loosen, which could result in lock-up of the rear axle and/or separation of the rear drive shaft from the rear axle (NHTSA Recall No. 12V-474).The vehicles involved in this recall are model year (MY) 2009 and MY 2010 DodgeeBay RameBay 1500 (46,677) and DodgeeBay Dakota (1,381) vehicles manufactured between July 1, 2009 and November 30, 2009 built with 9.25? rear axles built at ChryslereBay?s Detroit Axle Assembly Plant.ChryslereBay stated it determined during this time frame there were instances of 9.25? axles built with rear axle pinion nuts that were missing the adhesive patch.The remedy for these vehicles is to install a pinion nut retainer.In a review of data in its complaint database and data submitted by ChryslereBay, the Office of Defects Investigation (ODI) identified 194 reports (including 3 crash reports) of rear axle lock up and/or drive shaft separation outside of the scope of recall 12V-474.In a letter dated February 5, 2013, ChryslereBay submitted a second DIR to NHTSA identifying another safety defect in approximately 278,150 vehicles built with 9.25? rear axles and outside of the scope of recall 12V-474 that could also result in lock-up of the rear axle and/or rear drive separation (NHTSARecall No. 13V-038).ChryslereBay stated that analysis of field return parts from this vehicle population, undersized pinion splines were found as the likely factor contributing to the loosening of the pinion nut.The vehicles involved in this recall are:266,091 MY 2009?2012 DodgeeBay RameBay 1500 vehicles manufactured from 2/28/09 through 6/30/09 and 12/1/09 through 10/20/11 (including 183,421 MY 2009-2010 subject vehicles); 8,475 MY 2009?2010 DodgeeBay Dakota vehicles manufactured from 2/18/07 through 6/30/09 and 12/1/09 through 7/9/10; 2,346 MY 2009 ChryslereBay Aspen vehicles manufactured from 1/19/07 through 12/18/08; and 1,238 MY 2009 DodgeeBay Durango vehicles manufactured from 1/17/07 through 12/18/08.The remedy for these vehicles is the same as for 12V-474.The failure counts in this resume are for the subject MY 2009-2010 DodgeeBay RameBay 1500 vehicles only.The recalled vehicle counts are based on current estimates from ChryslereBay and may differ from estimates in the original DIR's.This Preliminary Evaluation is closed.The ODI report cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification (ODI) numbers:10489593,10489091,10488952,10485174,10485096,10484596,10484490,10483394,10483370,10482853,10482816,10482438,10479806,10479352,10478724,10477460,10477354,10476923,10476691,10475939,10475241,10475158,10473147,10472488,10470655,10470516,10470231,10470152,10469969,10469548,10469435,10468759,10468024,10467819,10467805,10467683,10467196,10466630,10464240,10448778,10445208,10443542,10439902,10375394,10362531,10353440

In a letter dated July 25, 2012 FordeBay Motor Company (FordeBay) submitted a Defect Information Report (DIR), NHTSA Recall No. 12V-353 to NHTSA identifying a safety defect involving inadequate clearance between the engine cover and the speed control cable connector, which could result in a stuck throttle condition.The vehicles involved are model year (MY) 2001 through MY 2004 FordeBay Escape vehicles equipped with 3.0L V6 engines and speed (cruise) control.A redesigned engine cover was implemented in production beginning in MY 2005, which provided additional clearance of the speed control cable to the engine cover.Similarly on July 26, 2012 MazdaeBay North American Operations (MazdaeBay) on behalf of MazdaeBay Motor Corporation of Hiroshima, Japan submitted a DIR to NHTSA, NHTSA Recall No. 12V-357, identifying the safety defect described above in MY 2001 through MY 2006 and MY 2008 MazdaeBay Tribute vehicles (no Tribute vehicles were produced in MY 2007) equipped with 3.0L V6 engines and speed control.Unlike the FordeBay Escape recall scope, the MazdaeBay Tribute scope is broader as MazdaeBay Tribute vehicles incorporated a similar engine cover design as related to the clearance to the speed control cable in MYs 2001 through 2006 and MY 2008.The failure report summary above reflects reports identified by ODI in review of data submitted to the Office of Defects Investigation (ODI) and reports submitted to FordeBay and to MazdaeBay.ODI identified 44 reports likely related to the safety defect described in the DIRs; 39 involving FordeBay Escape vehicles and 5 involving MazdaeBay Tribute vehicles. On March 21, 2012, Carlisle Transportation Products (Carlisle) notified the NHTSA that it had decided a defect exists in certain 908 Yuma, 910 Intruder, and 921 Intrepid series Black Rock toy hauler trailer wheels.These trailer wheels were manufactured from August 9, 2010, through February 7, 2012, and were sold and installed as original equipment for use on various boat, campereBay, and utility trailer vehicles.These wheels were manufactured with machine dimensions for the application of a center cap and the application of steel lug inserts that were not controlled to the required blue print specifications.As a result, the wheel may not be tightened to proper torque requirements during the mounting process.This condition could allow the wheels to loosen, leading to wheel separation, and possibly a vehicle crash.A detached wheel also becomes a road hazard.This matter was assigned recall number 12E-011.On July 26, 2012, the Recall Management Division (RMD) mailed Information Request (IR) letters to ten (10) companies identified through a purchaser list submitted by Carlisle.On October 1, 2012, RMD mailed a second IR letter to three (3) companies that did not respond to the earlier letter.All companies contacted did eventually file a written response with RMD.Three (3) companies explained that their affected trailer wheels were returned to Carlisle prior to installation onto new or used trailers.The remaining seven (7) companies filed Part 573 Reports and conducted safety recalls for their affected trailers. The following seven (7) recalls were filed as a result of this EQ:12V-385California Trailer Works 12V-396B&M Mfg., Inc. 12V-414McClain Trailers 12V-418Lippert Components (Extreme Trailers) 12V-512Express Trailers 12V-517Forest River Texas Group 12V-520Aluminia, Inc.All wheels affected by this safety defect have been identified and a recall campaign conducted where applicable.Accordingly, this EQ is closed.
