VEHICLE INFORMATION Year/Make/Model: 2020 Ford
Escape Hybrid
Titanium Component: Electrical / Instrument Cluster / Digital Display Unit (IPC) Hazard: Critical Visual Hazard & Operational Safety Defect STATEMENT OF SAFETY CONCERN The 12.3-inch digital instrument cluster in the 2020 Ford Escape Titanium is experiencing severe pixelation and flickering, indicating imminent screen failure. Because this trim lacks mechanical gauges, screen failure blinds the driver to vital real-time operating metrics: Speedometer: Prevents real-time speed monitoring, increasing collision risk. Engine Temperature: Hides thermal warnings, risking sudden engine breakdown or overheating. Fuel Gauge: Obscures fuel levels, risking sudden loss of propulsion in active traffic. Warning Lights: Hides critical alerts for Brake System, Airbags, Tire Pressure, and High-Voltage Hybrid
System faults. OPERATIONAL HAZARD ANALYSIS Driving with a malfunctioning instrument panel creates immediate hazards: Driver Distraction: Constant screen flickering directly in the driver's line of sight creates a dangerous visual distraction, especially at night. Power Loss Risk: Lack of fuel/hybrid
monitoring increases the likelihood of stalling on highways or busy intersections. Loss of Safety Warnings: The vehicle cannot communicate critical restraint or braking system failures to the driver. REQUESTED ACTION / RESOLUTION This condition represents an active safety defect. I request: Immediate diagnostic and replacement of the digital instrument cluster unit (IPC) or control module under applicable technical/safety service guidelines. Formal documentation of this defect with Ford Motor Company and reporting to federal regulators (NHTSA).

Summary
On multiple occasions, my vehicle's 12v battery has completely discharged presumably from parasitic drain from connected services communications. The first time the 12v was completely discharged was on 6/10/26 following between 24-48 hours of sitting idle without being driven. It was recharged and the following day had discharged almost completely, giving a warning that it needed to be charged back up. This was after ~24 hours without being driven. I ultimately brought it to the dealership on 6/17/26 where they were unable to replicate the issue initially and cleared the vehicle to be picked up on 6/19/26. Upon my arrival to pick up the car the following day (6/20/26), the 12v battery was dead on the lot. My vehicle then remained at the dealership for continued investigation before ultimately being returned to me after resetting the vehicle software and recharging the battery. At the point that I picked the car back up on 7/15/26, I installed a battery monitor device (Ancel BM200) and since then I have observed a cyclic pattern of voltage decline in a sawtooth pattern that begins 3 hours after turning the vehicle off and continues until the vehicle is driven again. I have also observed - via an outdoor security camera with IR capability at my home - a corresponding constantly repeating cycle during the sawtooth draining pattern where there is an IR light (presumed the driver monitoring camera) that turns on and off every 65 seconds. Although it has not fully discharged again, it is only because I have monitored the voltage and recharged or driven it every time I note the voltage getting too low (24 hours without recharging it, the battery would completely discharge again. My safety and the safety of my family have been put at risk due to the risk of being stranded without the ability to operate the vehicle due to a fully discharged 12v battery in the vehicle.

Summary
The truck is in for its second front end alignment. I bought it brand new in October 2024 and it has about 43000 miles on it. I'm concerned for my family's safety. If it is going out of alignment this often, I don't feel safe driving it. The dealership has no explanation of why its going out so fast. I know I'm not the only one having this problem. There are thousands of people saying they are having the same issues. It was out of alignment from the factory. Pretty much all of them are. Something needs to be done about this. I put today's date but the first time I took it in was less than a year and 20000 miles ago.

Summary
On XXX I leased this 2026 Silverado and I have discovered that this truck tramlines, basically rocks side to side on roads with longitudinal grooves in the concrete, meaning lines that go the same way you're driving, the truck will violently swag side to side. I have been pushed into oncoming traffic from this. On 7/17.26 I took my truck to Fox Chevrolet
of [XXX] to get service about this issue. I took the general manager of theirs, Marc and he confirmed the issue. He said he would send the DOT number to GM and wait for their reply. He called me on 7/20 saying that GM informed him that this has been reported by others and there's nothing GM is willing to do and to just deal with it. I told him that I find that unacceptable and it needs to be looked at my someone higher up. He passed this to his service manager since he's going on vacation for the next week, Ron. Ron called me and said that he talked to the local dealer rep and that he claims there's a law that says GM can't change tires that didn't come on the truck and that this is a known issue with these tires and again to just deal with it. This is completely unacceptable and flat out dangerous! I had a 2024 Silverado before this truck and it never did it. It had different tires than this truck and that's my argument is the tires are the problem. The manufacturer, Michelin
even said that the solution is to get different tires and it's known on these ones. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Vehicle Model: MDX TYPE S

Summary
While making a left turn from an on-ramp, I heard a loud crunch from the drivetrain and experienced a momentary loss of power. Immediately afterward, the front tires began spinning under acceleration. I realized the vehicle was no longer sending power to the rear wheels and had effectively lost its all-wheel-drive capability. The instrument display continued to indicate that power was being sent to the rear wheels even though the rear wheels were not receiving power. This gave an inaccurate indication of the vehicle’s operating condition. The vehicle was inspected by an authorized Acura
dealer. The dealer confirmed that the splines between the transmission and the front power takeoff unit had stripped. The transmission/front differential assembly and power takeoff unit were replaced under warranty at approximately 52,280 miles. The dealer stated that this is a problem they have been seeing on the MDX Type S and also commented on the number of revisions that have been made to the power takeoff unit. The replacement unit installed was part number 29000-61E-070. The failure created a safety concern because it caused a momentary loss of propulsion while turning into traffic, followed by unexpected front-wheel spin and loss of all-wheel drive. There were no warning lights or advance warning before the loud crunch and loss of power. The failed components were replaced by the dealer; whether they remain available for inspection is unknown.

Summary
The lower aluminum engine oil pan developed a structural hairline fracture ("micro-crack") directly along the factory casting ridge adjacent to the drain plug. The component failed without any external impact, road hazard, scraping, or underbody damage. The failure was officially confirmed and documented by Eckert Hyundai
during a service visit on July 13, 2026 (Case #42971835). Following the dealer oil service, the fracture degraded into an active, worsening oil drip that requires a catch tray under the vehicle to capture leaking fluid. The vehicle is currently available for inspection. Safety Risk: An active engine oil leak poses a significant safety hazard. Rapid or continuous loss of engine oil can cause sudden oil starvation, severe engine seizure, or total power loss without warning while driving at highway speeds, increasing the risk of a crash. Furthermore, dripping oil contacting hot exhaust or engine components presents a potential fire hazard. Prior Symptoms / History: This vehicle has a well-documented history of severe internal engine issues and thermal stress, including excessive oil consumption (running 1.2 quarts low in 1,127 miles), valve cover gasket failure, and severe drivetrain vibrations from failing front axles. The excessive internal crankcase pressure and thermal stress from these underlying defects contributed directly to the material fatigue and structural failure of this aluminum oil pan.
