Driving for 10 minutes when I started smelling a burnt smell. 30 to 45 seconds later I noticed smoke coming out the vents. A few seconds later (while at a stop sign at this point) I see smokecoming out of the hood. Vehicle would not go into park. After several attempts I finally was able to get the vehicle in park and get it to shut off. Opened the hood, saw a small 1 inch flame behind the motor near the firewall on the driversside and immediately called the fire department or emergency responders. Within 3 mins my vehicle was up in flames for a catastrophic vehicle loss.

The power sliding door actuator motor drive cables on my 2022 Kia Carnival snapped on both the passenger and driver sides. The doors are stuck shut, even in manual mode, creating a safety hazard for passengers since the only way to exit in an accident would be through the rear hatch or the front doors. The Kia dealer said they couldn’t open the doors without removing the interior sliding door panels, which would require damaging them because the doors are stuck shut. There were no warning signs before either cable broke. I have also researched online and it seems many Kia Carnival Owners are having the same sort of issue.

On October 16, 2017, we were driving this vehicle on the interstate when we noticed smoke in the rearview mirror. We pulled over right away, and within seconds of getting out, the car was engulfed in flames. It burned so fast that by the time firefighters arrived, about seven minutes later, there was nothing left of it. Neither of us was physically injured, but we got out with almost no time to spare. The fire department responded and a report was filed. The car was equipped with the 1.6L GTDI EcoBoost
engine and falls within the population of Safety Recall 17S09 / NHTSA 17V-209, which covers localized cylinder head overheating and cracking that can cause a pressurized oil leak and an engine compartment fire. At the time our car burned, the interim recall notice had gone out but no remedy parts were available; the actual recall repair did not begin until January 5, 2018. Ford later contacted us and offered a small settlement, conditioned on signing away any right to bring future claims. We declined.

2022 Lexus NX 350h, VIN JTJGKCEZ6N2002622. On multiple documented occasions since 2022, while backing up, the vehicle's rear camera/backup system has detected an object that was not present and automatically applied the emergency brake, causing a sudden hard stop with no actual obstruction — at least 7 times per dealer records (repair orders dated 8/11/2022 and 7/19/2023, Lexus of Riverside), including once with my child in the vehicle. The dealer was unable to duplicate or repair the condition on either visit. Separately, the vehicle has twice become inoperative and trapped me inside — once in April 2025 near 100°F outside temperature, and once in November 2025 for over an hour, requiring AAA to replace the battery. I'm concerned this vehicle poses an ongoing safety risk from unpredictable automatic braking and electrical/lock malfunctions.

Safety Defect: Moonroof Sunshade Sags and Obstructs Rearward Driver Visibility I am reporting a safety concern involving the fabric sunshade/moonroof cover in my 2022 Ford Explorer. When the moonroof fabric sunshade is closed/engaged, the fabric sags substantially downward from the roof. The sag is severe enough that the sunshade extends well below its intended position and can obstruct the driver’s view through the rear window. This is not simply a cosmetic issue. When the sunshade is engaged, the sagging fabric creates a significant obstruction to rearward visibility from the driver’s seating position. This can prevent the driver from seeing vehicles, pedestrians, or other objects behind the vehicle while driving. The attached photograph shows the condition from inside the vehicle. The fabric can be seen hanging substantially below the surrounding headliner and roof structure. The amount of sag is sufficient to interfere with the driver’s line of sight toward the rear of the vehicle. The driver should not have to operate the vehicle with the moonroof shade open simply to maintain adequate rearward visibility. The purpose of the shade is to block sunlight while remaining within the designed roof area; it should not hang down into the passenger compartment far enough to obstruct the driver’s view. Because this condition can directly affect the driver’s ability to observe traffic behind the vehicle, I believe it presents a potential motor-vehicle safety hazard and should be investigated as a possible defect affecting the moonroof/sunshade assembly. Vehicle: 2022 Ford Explorer I request that NHTSA investigate whether this condition is occurring in other Ford Explorer vehicles and whether the design, material, mounting, or tensioning system of the moonroof sunshade can cause it to sag and obstruct rearward visibility.
