My 2023 Prius Prime cannot be charged normally in hot weather without triggering a thermal-protection fault on its high-voltage charging system. Avondale Toyota diagnosed it in writing: "CODE P1CE996 CHARGER COOLING FAN REVOLUTION SENSOR COMPONENT INTERNAL FAILURE. THE ELECTRIC CHARGER ASSEMBLY HAS A INTERNAL OPEN CIRCUIT CAUSING IT TO BE INOP. RECOMMEND REPLACEMENT ADD PN# G9090-47061." Quote: $2,516.99 plus ~$200 diagnostic. The vehicle cannot verify its high-voltage charger's cooling fan is operating. In Phoenix heat (100F+), normal charging repeatedly sets the fault and check engine light. I verified by controlled testing: reduced-current charging in cool hours avoids the fault, but any normal charge in heat triggers it — in factory configuration, no obstruction. I am forced to restrict charging to nighttime reduced-current sessions to avoid operating the charger with unverified cooling. A plug-in vehicle that cannot safely complete a normal charge in summer conditions is a safety concern. This is a documented defect: Toyota TSB T-SB-0054-23 (8/7/23), "MIL ON With DTC P1CE996," states the charger assembly "has been modified to reduce the possibility of this condition," superseding part G9090-47060 with G9090-47061 — the part my dealer recommended. The TSB covers VINs before XXX; mine ([XXX]) is in range. Toyota denied coverage at all levels (Case XXX), citing third-owner status and declined unrelated services. A Brand Engagement supervisor acknowledged this identical repair would be fully covered under the PZEV emission warranty if registered in [XXX] — conceding it is a warrantable emissions defect — while denying it for Arizona registration. I request NHTSA review whether a recall or warranty extension is warranted for affected VINs. Hot-climate owners bear full cost of a documented defect on a high-voltage component. After coming to a complete stop at a stop sign or stop light, then trying to go, the vehicle will stall. It will not go when given gas. The first time it happened, I was at a busy intersection with oncoming traffic coming towards me with the pedal to the floor, nothing would happen for 5-10 seconds. It was terrifying. Thank god the car coming towards us went around in time. It then happened again in my neighborhood a week later. Thankfully that time I wasn’t in traffic. I have noted that it seems to happen when very hot outside (Florida July summer) and air is on full blast. Also seems to be happening only during the first trip of the day, within the first 5-10 minutes of said trip. The dealership has not been able to recreate this to see for themselves. I think it would have to be with them overnight and they start it/drive it for the first time that day. It has now happened a total of 3 times since I’ve been driving it (2.5 months). I have 3 children and very worried I will get t-boned one day when it stalls next. I love my car so much so it’s heartbreaking that I have to drive it in fear. No lights/warnings/messages when this happens.

I am the original owner of a 2020 Honda Accord 1.5 Turbo. The vehicle has developed a check engine light, rough engine operation with misfires, and reduced fuel economy at Mileage: 72,207. The vehicle was diagnosed at Honda of Reseda with failed fuel injectors. American Honda reviewed the matter under Case #16435337 and offered a one-time goodwill accommodation that would require an out-of-pocket payment of $1,058.58 plus applicable fees and taxes. I respectfully declined the offer because fuel injector failures causing these exact symptoms appear to be a known and recurring issue on many 2018–2022 Honda Accord 1.5 Turbo models, often occurring at relatively low mileage. Honda has issued technical communications regarding injector-related diagnostic trouble codes (such as P030x and P0172) on these vehicles. I am filing this complaint to document the defect and request a thorough investigation so that a proper recall or comprehensive remedy can be implemented for all affected owners.

The 10 speed automatic transmission shakes and jumps between gears. The transmission also shakes and rumbles at cruising speeds. The diesel engine version of this truck is already recalled. My truck is a gas engine and should also be recalled. This is the same defective 10 speed transmission used in all GM vehicles and recalled in many. This vehicle needs to be recalled and fix before the wheels lock up and kill someone.
