The rental car cost will be reimbursed for the rental period of maximum 1 day at
the rental rate of maximum $45/day. Use “RT” sublet type and “LNM” as the
sublet reason code under opcodes ZLZ031, ZLZ032, ZLZ034, and ZLZ035.

For all models, except Lexus ES 350, the replacement P/N will be a campaign
04005/04007-XXXXX P/N. Please identify the correct service P/N in the EPC and
then use the chart below to reference the appropriate campaign P/N for this
program. If there is a campaign P/N listed in the table below, the service P/N
