The Office of Defects Investigations (ODI) opened Preliminary Evaluation (PE15-013) on 2012-2013 model year (MY) Navistar Prostar vehicles for throttle pedals failing to return to the idle position after being depressed.EWR field reports supplied by Navistar
describe the non-returning throttle pedals as preventing the engine brake from activating and elevated engine speed making it difficult to stop the vehicle.ODI contacted complainants who alleged the accelerator pedal didn?t return to the idle position.Some drivers indicated they were annoyed, while others stated that at idle they were able to keep the vehicle stopped but need to apply more pressure to the brake pedal.Some consumers indicated that the engine speed was so high with a manual transmission that the engine-brake would not work, while other stated they were able to pull the accelerator pedal back from underneath with their foot and bring the engine speed back to normal.In Navistar
?s response to an ODI information request letter, they made ODI aware of 24 consumer complaints concerning non-returning pedals, mostly on manual transmission vehicles.There were no crashes, property damage, injuries or fatalities reported.Navistar
identified 1,233 warranty clams related to all accelerator pedal issues, with 70.3% of these claims were on manual transmission vehicles.Navistar
informed ODI that they became aware of an unauthorized part change to the throttle control unit assembly (manufactured by Williams Controls) from April 2012 to March 2013.This unauthorized change was corrected and all vehicles were being retrofitted with the new/original designed part.The design of the subject vehicle throttle pedal is a solid-state rotary position sensor device, which transfers voltage based on shaft rotation in either direction from a reference angle.Shaft rotation is obtained by the driver stepping on the accelerator pedal or allowing the pedal to return back to closed throttle (CT) position.Navistar
has described how the throttle does not return completely to the CT position due to stack up tolerance issues and backlash in the pinion/lever arm gear set.The accelerator pedal sensor stator can move slightly on the mounting bracket if the accelerator pedal is pushed with extra force.On two separate occasions an ODI investigator drove several of the subject vehicles.The vehicles that had a high idle with automatic transmissions were reasonable to stop even at the elevated engine speeds.Due to the architecture of an automatic transmission, the engine speeds never got high enough to demonstrate a safety related defect while being driven by the ODI investigator.However, the manual transmission vehicles showed a propensity to increase engine speeds in excess of 2,300 RPM?s while the vehicle was at rest.This condition occurs when the clutch is depressed the engine can rise to the elevated governed RPM, resulting in difficulty selecting gears while shifting.In the condition of a loaded vehicle going down a long grade, the inability to down shift could cause difficulty in maintaining or reducing vehicle speed with just the service brakes.Navistar

The Office of Defects Investigation (ODI) opened this investigation based on a Vehicle Owner Questionnaire (VOQ) which reported an injury related to the unintentional deployment of the electric entry step on a subject vehicle.The owner reported to Winnebago that the steps extended while in motion.Service personnel from Winnebago
instructed the owner to disconnect the wire harness at the steps to prevent them from extending while traveling and then reconnect when needed.The owner did this and was subsequently pinned beneath the steps when they extended upon reconnecting the wire harness.When deployed the steps extend approximately thirty (30) inches from the side of the vehicle.This reach facilitates easy entry into the motor home, but creates a hazard when unintentionally deployed in transit.The driver may not be immediately aware the stepsare extended which increases the possibility of striking another object or person.During the investigation, ODI sent an Information Request (IR) letter to collect information from Winnebago
.ODI analyzed this data and determined that a significant number of claims reported entry steps failures.Many of these claims specifically stated the steps extended while the vehicle was in transit. Winnebago
service personnel offered multiple options to temporally disable the entry steps so that travelers could safely drive the vehicle.Depending on the situation fixes ranged from sending a field service technician, to instructing the individual to disconnect the wire harness the same as the owner who submitted the VOQ. A review of the time to failure indicated that most failures occur early in the vehicle's life.The average time to failure was five (5) months in service.Most reported failures occurred within the first year of ownership.ODI determined no design or installation changes had been made to the subject components prior to opening this investigation.Over the course of the investigation Winnebago
identified several opportunities for improvement.Changes were made to current production to improve entry door reliability and durability in August 2015.Winnebago
has submitted recall 15V-866.This recall covers vehicles produced between August 1, 2014 and August 31, 2015.With the recall action taken by Winnebago

) submitted a Defect Information Report (DIR) to NHTSA describing a safety defect that may result in a sudden loss of power steering assist while driving in approximately 746,067 model year (MY) 2008 through 2011 Ford
Escape vehicles equipped with electric power assisted steering (NHTSA 14V-284, Ford
14S05). Ford
's DIR described the defect condition as a poor signal to noise ratio [SNR] in the torque sensor within the Electric Power Assisted Steering (EPAS) system [which] does not allow the PSCM to determine the driver's steering input. When the system detects this fault condition, it transitions the EPAS system to the fail-safe/manual steering mode.Ford
's DIR indicates that loss of power steering assist while driving would require higher steering effort at lower vehicle speeds, which may result in an increased risk of a crash.Ford
's remedy instructs dealers to check the Power Steering Control Module (PSCM) for Diagnostic Trouble Codes (DTCs) to determine the proper repair procedure. If no DTCs are present, dealers are to update the PSCM and instrument cluster module software. The updated PSCM software changes the torque sensor fault strategy so that the SNR condition does not result in a loss of power steering assist while driving.In addition, audible and visual warnings are provided for torque sensor faults.If DTCs indicating faults in the torque sensor, PSCM or Power Steering Motor (motor) are present in the initial recall inspection, the dealer performs the following repairs: 1) replaces the torque sensor for torque sensor faults (DTC B2278); or 2) replaces the steering column for faults related to the PSCM (DTC B1342) or motor (DTC B2277).Continued in attachment pages

In a letter dated October 2, 2014 the Office of Defects Investigation (ODI) was initially petitioned by the North Carolina Consumers Council, Inc. requesting a defect investigation of an alleged defect condition resulting in headlight and/or exterior lighting failure on 2003-2005 Ford Crown Victoria and Mercury
Grand Marquis vehicles.ODI analyzed Vehicle Owner Questionnaire (VOQ) complaints received from consumers and identified a total of 605 reports (for all submission dates) alleging headlight failure.The complaints indicate failures of both low beam headlights typically while driving, a defect condition that was evaluated under a prior ODI investigation (PE08066).Most consumer VOQs indicate that the headlights failed suddenly and without warning leaving the driver with no forward lighting, however some report the headlights flickered or dimmed prior to turning off.In some cases drivers were able to turn the headlights back on after a period of time while others reported the headlights would not come back on at all.Consumers noted they were able to hold the flash to pass lever on the steering column mounted stalk to activate the high beams however this is only effective as long as the lever is actuated.ODI identified 4 VOQ reports alleging minor crash or loss of control incidents due to headlight failures.Drivers indicated running off of the road due to a lack of headlights and one driver reportedly struck a deer.No injuries were indicated in the VOQ crashes.In response to ODI's April 7, 2015 Information Request letter, Ford
provided data indicating it has received 3,092 complaints of failed headlights related to the Lighting Control Module (LCM), a device that controls the headlights and other lighting functions.This number includes the complaints Ford
reviewed and classified as headlight failure due specifically to the LCM.Within Ford
's complaints, there are 11 reports alleging a crash occurred.These drivers reported hitting objects, including other vehicles after the headlights failed causing damage to the affected vehicles.One driver was injured when their car struck an unseen object in the road, jerking the steering wheel from their hands and injuring their shoulder.In total 15 crash allegations were identified, all of which have occurred since ODI's prior evaluation of this issue.This petition has been granted.Preliminary Evaluation PE15-028 has been opened to assess the scope, frequency, and safety-related consequences of the alleged defect.The ODI reports cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SarchNHTSAID under the identification (ODI) numbers found in the attached list.
