On March 12th, 2018, Ford Motor Company submitted a Defect Information Report to NHTSA describing a defect in the steering wheel fastening system of approximately 1,301,986 model year (MY) Ford Fusion and Lincoln MKZ. On March 1, 2018, Ford discovered a design change that affected the MY 2014-2018 subject vehicles. The affected population are MY 2014-2016 Fusion and MKZ with manual and power adjust steering columns and MY 2017-2018 Fusion and MKZ equipped with manual adjust steering columns only. The affected populations have steering shafts with a reduced amount of threads for the steering wheel bolt to engage. The thread length was decreased by 5mm in the affected models. Ford stated that another contributing factor was the placement of a nylon locking patch torque retention on the fastening bolt.As a result, the steering wheel bolt may not maintain torque allowing the bolt to loosen and eventually separate from the steering shaft, resulting in a loose or detached steering wheel. The two crashes and one injury noted in Ford?s Defect Information Report are from two legal claims. The first accident is a claim where the driver reported the steering wheel twisted freely on the shaft which caused the driver to lose control.An injury was alleged but no details were provided. The second claim resulted in minor damage to the driver side of the vehicle including a punctured tire. No injury was reported.Ford?s recall remedy procedure will use a bolt that is 17mm longer, providing a more robust steering column thread and torque retention patch engagement. The torque retention feature will be 13mm longer to ensure proper engagement. This investigation is closed based on Ford's recall.

Through Early Warning Reporting (EWR) data review of 2016 Q1-Q3, several field reports were found that identified failures of seat belt latch casings on MY 2016 Van Hool motorcoaches.While reviewing the EWR data related to the latch issue, ODI was contacted by the National Transportation Safety Board (NTSB) regarding seat belt latch issues they discovered while investigating a crash between a train and a 2016 Van Hool bus in Biloxi, Mississippi.Through inspections of buses similar to the bus involved in the crash, the NTSB found that casing failures were occurring on seat belt latches manufactured by Fasching Salzburg.These concerns were forwarded to ODI suggesting the latch casing failures may be a wide spread problem found on MY 2016 Van Hool buses.As a result of the information provided by the NTSB along with the related EWR field reports, ODI opened a defect investigation (PE17-006) on the subject vehicles. The intent of the investigation was to better understand if there was a safety related defect involving the latches.During the investigation ODI reviewed warranty claims submitted by VAN Hool and found that many identified situations where passengers could not get the seat belt buckles to latch or in some case unlatch.This presented a situation where passengers were either unrestrained during a potential crash or could not get out of their seats to exit the vehicle in a potential emergency.With recall action 18V-209 taken by Van Hool this investigation is closed.Further use of agency resources does not appear to be warranted.The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exists on other model or MY vehicles outside of the recall scope. The agency reserves the right to take further action if warranted by the circumstances.

By letter dated April 15, 2013, Motor Coach Industries (MCI) notified the Office of Defects Investigation (ODI) of a safety defect that may exist in certain model year (MY) 2012 and 2013 MCI D-model coaches built from December 22, 2011 to May 19, 2012 equipped with the Cummins 2010 EPA ISX 12 Liter engine.These vehicles may have had an incorrect installation of the exhaust pipe.If installed incorrectly, the elbow pipe may prematurely fail and if a Diesel Particulate Filter regeneration event occurs, the hot exhaust gases could cause a fire, resulting in injury to persons and/or damage to property.MCI conducted an inspect and repair if necessary recall campaign identified by NHTSA Recall No: 13V-142. At the time MCI believed that 13V-142 covered all the vehicles potentially having the safety defect. However, ODI review of EWR data indicated that other vehicles outside the scope of 13V-142 also experienced the same defect condition. Subsequently, in May 2017, MCI expanded the scope of the previous recall to include certain MY 2011-2012 MCI D-model coaches manufactured from January 31, 2011 to May 19, 2012, identified by NHTSA Recall No: 17V-337.In April 2013 when MCI conducted recall 13V-142 to remedy incorrectly installed bellows pipes on certain MCI model year 2012 and 2013 D model vehicles they also changed the bellows pipe assembly process in then current production.With recall 17V-337 MCI expanded the first recall by an additional 178 vehicles they felt also had not had the bellows pipe properly installed.Following the receipt of complaints and EWR data alleging bellows pipe failures on vehicles built after the implementation of the improved assembly process, NHTSA opened this Recall Query to assess the scope and adequacy of these two safety recalls and MCI's recall decision making process.Analysis of the failure data provided by MCI to this investigation found that the improved assembly process reduced the failure rate but did not eliminate the defect all together.In response to this investigation, MCI agreed to conduct recall 18V-309. This latest recall includes and expands upon the vehicle population of the prior recalls, and implements a new remedy that adds a mesh component to the bellows assembly to reduce vibration.This remedy was also implemented in production in 2015.Vehicles built after this date do not have an elevated failure rate even with a significant amount of time in service.With recall action 18V-309 taken by MCI this investigation is closed. Further use of agency resources does not appear to be warranted. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist on other model or MY vehicles outside of the recall scope. On February 26, 2016, Bergstrom Inc. notified NHTSA of a safety defect in approximately 10,819 of its ParkSmart auxiliary electrical temperature management units. These units were installed on Freightliner Cascadia Class 8 trucks that may overheat due to high resistance, resulting in possible fires. One of the contributing factors which led to this recall was a plastic vented cap covering the compressor connectors allowing increased flow of air to the overheated connectors, which increased the risk of a fire.ParkSmart units equipped with non-vented caps, according to Bergstrom, were not affected. See Bergstrom recall 16E-017.On March 7, 2016, Daimler Trucks North America (DTNA), the parent company of Freightliner, filed its corresponding defect decision as to the impacted Freightliner trucks.DTNA identified 8,865 model year 2015-2016 Freightliner Cascadia trucks installed with the ParkSmart units.On September 18, 2017, the Recall Management Division (RMD), opened this investigation to resolve the discrepancy between the larger number of units Bergstrom recalled with the smaller number of trucks DTNA identified as impacted, and to pursue additional recall actions as needed. RMD issued an information request to DTNA on Oct. 26, 2017 inquiring about the discrepancy, as well as field experience information.DTNA filed several responses of different dates that provided shipping and distribution information as well as field reports.DTNA explained that their recalled population was comprised only of vehicles they had produced for the U.S. market.They identified that 1,282 vehicles were produced for the Canadian market, and 122 vehicles for the Mexican market.DTNA said that the remaining balance of parts were likely supplied to parts depots for service (service loops) for other purchasers, such as Freightliner dealers sourcing custom vehicle requests. RMD also requested and reviewed shipping and distribution materials from Bergstrom. These materials identified Park Smart units intended for MY 2014 Cascadia trucks (e.g., vehicles outside the DTNA recall population).RMD confirmed with DTNA that the MY 2014 trucks had the non-vented cap design used on them and so were properly excluded from the recall population.In addition to collecting information and documentation from the involved companies, ODI reviewed early warning reporting information. This search identified nine reports of thermal events. All events, however, occurred on MY 2014 Cascadia trucks that, for reasons explained above, were not part of the DTNA recall and the event linked to other causes. In view of the information available to it at this time, RMD is satisfied that the scope of the DTNA recall is adequate.Accordingly, this RQ is closed.
