On September 30, 2019, ODI opened Defect Petition DP19-004 to evaluate the petitioner?s request for an investigation. In August 2020, the petitioner notified the Agency that he was withdrawing his petition. Summary
In September 2019, the Office of Defects Investigation (ODI) opened PE19-013 to assess the potential safety related effects of a malfunction of the Passenger Sensing System (PSS) in 2012-2013 Model Year (MY) Chevrolet Volt vehicles. Consumers reported that due to a malfunction of the PSS, the air bag system malfunction indicator light (MIL) was activated. Many of the complaints noted the malfunction occurred after the warranty period expired and mentioned high repair costs as a major deterrent to the repair of the vehicle.The PSS sensor mat is embedded in the front passenger seat cushion and is intended to detect child seats
and small statured occupants who could be harmed by deployment of the frontal passenger air bag. The PSS may develop a malfunction due to bunching of the sensor mat in the seat trench. The bunching of the sensor mat could cause the mat to repeatedly fold in the same location. Over time, the repeated folding can cause an increased resistance in the sensor mat. An increased resistance in the PSS sensor mat is detected as a fault in the air bag system diagnostic. If this fault is detected, a diagnostic trouble code will be stored in the air bag control unit and the air bag MIL will illuminate.Further, if the fault is detected, the passenger frontal and knee air bags will be suppressed in a crash of sufficient severity to warrant a commanded deployment regardless of the nature of size of the occupant of the front passenger seat.The report counts shown above include only those reports that specifically address the issue covered by this investigation, failure of the PSS sensor mat. Reports alleging only that the air bag light is on with no cause stated, or where a different cause was otherwise identified, were excluded as the air bag light could be illuminated for system faults other than a PSS malfunction.The subject vehicles have been in service for approximately ten (10) years. An analysis of General Motors
(GM) warranty data for the subject vehicles, including a comparison with other agency and manufacturer data pertaining to similar occupant detection system failure investigations, indicates the subject vehicles experience a comparatively low complaint rate for this issue given the number of years the vehicles have been in service.GM has instituted a cost reduction program for the repair/replacement of the PSS in the subject vehicles. Based on this cost reduction program as well as:1) a comparatively low warranty rate; 2) no reported injuries, deaths or asymmetric deployments; and 3) the age of the vehicles; ODI is closing this Preliminary Evaluation. The closing of this investigation does not constitute a finding by NHTSA that a safety related defect does not exist. The Office of Defects Investigation (ODI) opened this Preliminary Evaluation to investigate complaints alleging improper retraction of the front seat belts (driver or passenger) due to the plastic guide of the D-ring cracking in model year (MY) 2014 Jeep Grand Cherokee vehicles. The purpose of the investigation was to understand the scope, frequency and safety related consequence of the alleged defect. The subject condition occurs when the plastic material encapsulating the metal substructure of the front seat belt, B-pillar mounted D-ring cracks. The structural integrity of the D-ring remains intact; however, cracking of the plastic material pinches the webbing at the D-ring, thereby preventing it from properly retracting, and leaving the belt loose on the occupant. In addition to the detectability of the loose belt, the cracks in the plastic guide are also apparent by visual inspection, and many reports indicate the webbing can be manually fed back into the B-pillar/retractor to improve seat belt fit. ODI review of the available data has not identified any injuries attributable to this failure.In its November 6, 2019 response to ODI's September 8, 2019 information request letter, which was scoped to include MY 2013 to 2015 Grand Cherokee and Dodge
Durango vehicles, FCA
providedinformation (portions of which were submitted with a request for confidentiality) detailing several material and process changes involved in the production of the subject D-ring. Based on these changes, FCA
sub-divided certain production periods into population groups representing different design levels of the D-ring. FCA
's response also confirmed the same D-rings were used in Dodge
Durango front seat belts. ODI notes that installation of seat belts during vehicle production involves highly controlled processes, in comparison to field service, which ensures all mechanical and electrical connections are properly secured.ODI analyzed complaints submitted directly to the agency and complaint data received from FCA
. During this review, ODI identified elevated failure rates for Grand Cherokee and Durango vehicles produced from mid-June 2013 through July 2015 when compared to vehicles produced both prior to and after this period. Additionally, review of FCA
warranty data indicated a similar pattern of elevated warranty claims in the suspect July 2013-July 2015 period. The primary factors for the failure rate differences appear to be a D-ring material change (from nylon to a Celcon plastic) starting in mid-June 2013 and a subsequent change (to a Delrin material) in August 2015, however other production changes occurred during the suspect period, as discussed in greater detail in FCA
's November 2019 response. ODI's analysis of the various data sources showed vehicles produced in the suspect period experienced failure rates approximately 2 to 4 times higher, depending on the data source and production period evaluated. However the analysis also indicated a declining failure trend.Given the detectability of the condition, the failure frequency combined with a currently declining failure trend, and lack of injuries attributable to this condition despite significant time in service, ODI is closing this Preliminary Evaluation without further action. On September 16, 2019, the Office of Defects Investigation (ODI) granted Defect Petition DP18-004 and opened Preliminary Evaluation PE19-012 to investigate allegations of deficient frame welds and their potential safety related effects on 2018-2019 Jeep Wranglers. Due to a previous steering related recall that was caused by a misaligned weld on the front track bar (NHTSA Recall No. 18V-675), ODI decided to include reports of steering shimmy or wobble, loose steering, and steering stiction (loss of power steering) as part of this investigation and parent petition (DP18-004). ODI received information from the vehicle manufacturer Fiat
Chrysler
Automobiles (FCA
) via information request letters dated March 8, 2019, and February 6, 2020. Supplemental requests for information occurred between July 2020 and July 2023. A review of the responsive manufacturer information showed that vehicles presenting an unexpected reduction in steering ability due to a welding tool misalignment were correctly addressed via recall 18V-675. General complaints concerning the weld appearance were not associated with a realized risk to motor vehicle safety. ODI identified a limited number of reports involving weld separation on vehicles that were out of scope of recall 18V-675. FCA
identified various causes of weld quality issues in early production (build dates prior to March 2018) including improper welding techniques on galvanized frames and component part variation leading to off-seam welds. These issues were addressed in production for subject vehicles built after July 2018 and preceded a drop in weld failure reports. ODI reviewed manufacturer and agency reports of weld failures out of scope of recall 18V-675 and did not find any that resulted in a loss of vehicle control. There are no known crashes, injuries, or fatalities as a result of weld failures on vehicles not included in recall 18V-675. Due to the steering related consequence associated with Recall 18V-675, ODI opted to include a request for information regarding various steering issues experienced by vehicles included in this investigation. ODI reviewed manufacturer as well as agency data and determined these various steering issues were not related to any weld defect. Since the opening of this investigation, FCA
has addressed the following steering concerns; a. Steering looseness due to material choice in the steering box internal components. FCA
released an updated part with an internal component material less sensitive to temperature variances (TSB 08-003-21 REV. A).NHTSA will take further investigative action if justified. b. Steering wobble or shimmy due to a manufacturing and design deficiency in the steering damper. FCA
released a part via Customer Satisfaction Notification (CSN) V41 in June 2019 which implemented supplier quality control improvements and design changes due to the presence of air in the base valve which degraded damping performance.NHTSA will take further investigative action if justified. c. Steering stiction, or loss of power steering, due to an underpowered steering pump calibration that occurred at highway speeds during cold weather. In December 2021, FCA
released CSN Y95 which corrected this issue via software update. Due to the decreased number of consumer complaints, the service campaign appears to have addressed the issue related to an underpowered steering pump. NHTSA will take further investigative action if justified. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. NHTSA received a defect petition (DP19-001) from the Center for Auto Safety (CAS) on March 21, 2019 alleging that the Automatic Emergency Braking (AEB) system on Model Year (MY) 2017-2018 Nissan Rogue and Rogue Sport vehicles was engaging in cases where there was no obstruction in the path of the vehicle. The CAS petition additionally alleges that, based on Nissan
's actions, the manufacturer is aware of the issue due to issuing a Technical Service Bulletin (TSB) launching two "Quality Actions," and initiating a "Customer Service Initiative" in relation to it. CAS contends that Nissan
's actions do not represent an adequate long-term solution to the problem, as the action is only available during the basic coverage period of the warranty. CAS also argues that the alleged failures are an unreasonable risk to safety and views Nissan
's communications as not acknowledging the potential safety issue involved, instead treating the alleged problem as a performance update.The Office of Defects Investigation (ODI) analyzed Vehicle Owner Questionnaire (VOQ) complaints received from consumers and identified a total of 129 unique subject vehicle reports (for all submission dates) alleging a false positive triggering of AEB. Of these complaints, three were identified that contained allegations of a collision resulting from the activation of the system, and all three indicating related injuries.In response to ODI's April 15, 2019 Information Request letter, Nissan
provided data indicating that it has received 750 unique subject vehicle complaints relating to false positive activation of the AEB system. Of these complaints, 12 were identified that contained allegations of a collision resulting from the activation of the system. Four complaints contained allegations of injuries, three of which were the result of a collision. These complaints included duplicates of complaints that ODI received. The injury allegations were limited to soft tissue type injuries such as whiplash and bruising.Based on its review of the information identified to date, ODI has granted the petition and is opening this preliminary evaluation in order to evaluate the cause, scope and frequency of AEB false positive activations, as well as to evaluate the suitability of the manufacturer's campaign to address the concern or improve performance of the AEB system.
