NHTSA received a petition involving subject production year 2015 Britax Frontier ClickTight XE booster seats commonly used for children 2-years old and older weighing between 25-90 pounds while traveling in a vehicle. The Office of Defects Investigation (ODI) opened Defect Petition DP20-003 for a November 2019 crash event involving a subject seat. The crash involved a Model Year (MY) 2016 Nissan
Rogue striking a telephone pole in a frontal collision. A subject seat, installed at the driver side rear seat, experienced a structural failure allowing the harness to pull through the seat back. According to a police report, this failure contributed to the death of a 4-year-old occupant. Notably there were two other occupants in the Rogue who survived the crash with injuries, the driver and a 3-year-old who was seated in a different make/model car seat
at the rear passenger side seat position.During the DP, ODI sent Britax
a letter (dated April 17, 2020) requesting, amongst other things, a count of allegations of the subject seat failing structurally during, or otherwise incurring structural damage as a result of, a vehicular crash event. Subject seats were defined as all Frontier Clicktight seats, regardless of production year. In its response of May 28, 2020, Britax
identified a single allegation, the same crash event that was the subject of the Defect Petition received by ODI. Additionally, ODI conducted an inspection of the incident seat and vehicle, confirming the failure of the seat back and restraint system, and the significant damage to the front of the subject vehicle.To date, no determination as to the cause of the incident seat structural failure has been established, either by ODI or Britax

The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE) on April 1st, 2020 to investigate six consumer complaint allegations of brake fluid leaking from the front brake calipers on their 2019-2020 model year (MY) BMW
R1250 motorcycles. At the closing date of this investigation, the number of ODI complaints has risen to fourteen.During the investigation, ODI contacted several consumers to validate and confirm their complaints.ODI learned from these conversations that the subject motorcycles were leaving brake fluid
puddles on or near the front tires. One consumer forwarded pictures of a subject motorcycle leaking brake fluid
on a BMW
dealer showroom floor. In response to an ODI Information Request (IR) letter, BMW
supplied ODI with 139 dealer field reports. Many of them also described leaking brake calipers that failed at 0 to 4 miles shown on the bike odometer. ODI was also able to gather pictures from BMW
related to the field reports and they too showed calipers leaking brake fluid
. BMW
disclosed in the IR response that they switched to a Hayes
designed and manufactured front caliper starting with MY 2019 of the subject motorcycles. The previous model years used Brembo
manufactured calipers. ODI analyzed warranty claims and production data provided by BMW
. ODI found that 1,169 vehicles were repaired for front brake caliper leaks. A total of 838 subject vehicles were repaired before the date of first sale.The motorcycles, found to be leaking and serviced prior to the date of first sale, appeared to be in violation of Federal Motor Vehicle Safety Standard (FMVSS) 122 for ?Motorcycle Brake Systems?, under Section 5.2.3. The FMVSS requires that ?during all tests and on their completion, there shall be no friction material detachment and no leakage of brake fluid
. Discussions between the NHTSA Office of Vehicle Safety Compliance and BMW
resulted in the manufacturer recalling MY 2019-2020 R1250 GS, GS Adventure and R1250 RT, 2020 S1000 RR and S1000 XR motorcycles for front brake calipers leaking when parked, failing to comply with the requirements of the Federal Motor Vehicle Safety Standard number 122, Motorcycle Brake Systems.As part of the safety recall, BMW
will notify owners, and dealers will inspect, and if necessary, replace the front brake calipers with an improved design free of charge. The recall is expected to begin October 6, 2020.Owners may contact BMW

will take the following actions: 1) issue a letter to all original equipment (OE) customers with important safety information warning them about the safety risk of attaching plumbing to the exhaust port when the subject component is used as a park control valve; and 2) affix a warning label on all aftermarket parts warning not to install pipe-away exhaust port plumbing. ) notified the Office of Defects Investigation (ODI) of a safety defect that may exist in certain model year (MY) 2017-2020 J4500 vehicles. An ODI review of Early Warning Reporting (EWR) submissions by MCI
, on MY 2017-2019 J4500 buses, identified ruptured heater hoses on multiple motor coaches leading to the vehicles becoming disabled. ODI communications with fleets found that a low coolant level light with an audible warning would provide notice to the bus driver to pull off the roadway before the bus shut down to prevent catastrophic engine damage. According to the fleets interviewed, there is not sufficient time between the warning and the engine shutting down to allow the driver to pull to a safe location. Multiple reports/interviews describe buses requiring passenger transfer to a second bus at roadside. Analysis of data provided by MCI
in response to an ODI Information Request (IR) letter showed an elevated frequency of heater hose ruptures. The heater hoses may fail, resulting in a low coolant warning followed by an engine shut down sequence, as well as the potential for engine coolant spilling on the roadway. Sky vehicles.An analysis of General Motors
(GM) complaint data and information in NHTSA?s databases identified thirteen unique vehicle identification numbers (VINs) with reported incidents that could pertain to blower motor overheating, smoking, or melting in the 90,938 MY2006-2010 Saturn
Sky and Pontiac
Solstice vehicles produced. Two of the thirteen cited incidents reported smoke and thermal damage that may be attributed to the HVAC blower motor although neither incident was confirmed through vehicle inspection. One incident, a MY2008 Saturn
Sky with substantial field exposure from a frontal collision, could not have the root cause identified. The other, a MY2007 Pontiac
Solstice, did not inform the manufacturer nor take the vehicle to a dealer for evaluation.Most of the 13 incidents reported an overheated or discolored connector with attendant loss of HVAC blower fan function. GM reported 354 warranty claims with the subject components for the Sky and Solstice, none of which included a report of fire and only one report of smoke. Over a period of ten to fourteen years in service, there are two reports of smoke or thermal damage.While the subject Sky and Solstice vehicles and the recalled Hummer
H3 and H3T vehicles do share common components, the Sky and Solstice vehicles have demonstrated very different behavior and their HVAC system has a lower power draw than the recalled Hummer
H3 and H3T vehicles. The recalled H3 and H3T vehicles generate over triple the failure rate of the Sky and Solstice vehicles. GM suggested in its response to the ODI?s information request that this difference was due to the larger interior volume of a recalled Hummer
vehicle imposing a heavier electrical load and duty cycle on its HVAC blower. The duty cycle is defined as how long it takes for the vehicle to cool down enough before the HVAC blower motor fan speed is lowered or turned off completely.Additionally, due to the size differential between the Sky and Solstice vehicles and the recalled Hummer
H3 and H3T vehicles, the parts have a different power draw. The Sky and Solstice vehicles, due to their smaller size, draw less wattage, which reduces the likelihood of a thermal event. This condition in the Sky and Solstice vehicles typically leads to an inoperable HVAC blower motor as opposed to a thermal event.Despite the commonality of parts in the Sky and Solstice vehicles with those that were the basis of a safety recall, the subject vehicles have not demonstrated a safety defect trend that would likely lead to a safety recall or merit further investigation by the ODI. The subject vehicles have a low rate of reported thermal events over the ten to fourteen years they have been in service with the most recent occurring in 2017 with no confirmed fires. The HVAC blower motor was not confirmed to be the root cause for either thermal event cited above.After thoroughly assessing the material submitted by the petitioner, information already in NHTSA?s possession, information submitted by GM in response to an information request, and the potential risks to safety implicated by the petitioner?s allegation, NHTSA does not believe that the petition warrants a formal investigation at this time.
