F-150, F-250, F-350, F-450, F-550, Explorer, Mustang
, Transit, Expedition, Escape, Ranger and Edge, and Lincoln
Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford
's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford
's chronology in the Part 573, during the period February-April 2020, Ford
identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford
reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford
's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford
the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford
met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford
to discuss the recall status and additional VOQs. Ford

vans, configured for use as Amazon delivery vehicles or ambulances, roll away shortly after being shifted to park using the Auto-P function. A rollaway vehicle with no operator behind the steering wheel could potentially strike pedestrians, moving or parked vehicles or buildings, resulting in injury, fatality and/or property damage. Analysis of data provided by MB in response to the ODI Information Request (IR) letter showed a high failure rate. Following discussions with ODI about the failures, MB decided to conduct a safety recall. On December 15, 2021, Daimler Vans
USA, LLC filed recall 21V-972 covering 48,400 model year (MY) 2019-2020 Mercedes-Benz
(MB) Sprinter
vans. With recall 21V-972, this investigation is closed. The affected vehicle population determined by MB is Sprinter
vehicles (VS30, Platform 907) produced between June 5, 2018 and November 30, 2020 with a manual parking brake and a 7-speed automatic transmission that was manufactured between May 1, 2018 and October 31, 2019, in the Hedelfingen transmission plant in Germany.Further use of Agency resources does not appear to be warranted. The Office of Defects Investigation (ODI) has opened this Preliminary Evaluation (PE) on Haldex Gold Seal GC3030LCW air brake chambers installed in model year (MY) 2015-2020 vehicles based on allegations of air loss resulting in vehicle fires. ODI is aware of eleven (11) Vehicle Owner Questionnaires (VOQs) that allege, without discernable warning to the driver, the failure of a Haldex
brake chamber. Seven (7) of those cases involved thermal events causing extensive fire damage to the vehicle and, in some cases, the cargo too. In at least one of the fires, a fire investigation report identified a wheel end fire as the origin. Analysis of other failed units found Haldex
brake chamber diaphragms punctured by the power spring.ODI discussed the VOQs with the complainants. Through these discussions, ODI learned that multiple fleets were having issues with numerous vehicles. ODI further learned that Haldex

Summary
The Office of Defects Investigation (ODI) opened PE21-016 on July 7, 2021, to investigate 364 complaints alleging that one or both sliding doors on 2016 Model Year (MY) Grand Caravan or Town and Country vehicles could not be opened. The door(s) could not be opened using the mechanical handle or electronic remote switches. Many of the complaints described having to remove passengers seated in the rear of vehicles through the front doors, the second sliding door (if it was operational), the window, or the car?s rear hatch. The complainants have expressed concerns that if the sliding door(s) cannot be opened in the event of an emergency or crash, it could trap passengers or delay their egress.On June 4, FCA published TSB# 23-017-20 instructing dealers to replace the door lock actuator instead of the entire door latch assembly on the subject vehicles when consumers complained about either one or both of the sliding door locks not functioning and/or emitting a loud buzzing noise during lock/unlock operation. On June 16, 2021, FCA
approved a Quality Warranty Extension for fifteen years/150,000 miles from the vehicle's in-service date for sliding door lock actuators on vehicles built between May 1, 2015, through January 9, 2017. FCA
has started notifying owners about the new program. FCA
will provide reimbursement for owners who previously paid for sliding door repairs.Consumers whose sliding doors are inoperative will have the opportunity to have their vehicle diagnosed, and if a fault is identified, have the sliding door actuator repaired at no charge.Analysis of the FCA
IR response and ODI complaint data found that the majority (96%) of sliding door failures occurred prior to the calendar year 2020 when the subject vehicles had approximately 4 years in service. At the time of the closing of this investigation, 99% of the subject vehicles are passed the age where failures typically occur. The number of new reports has been decreasing since 2020 and further reduced following the FCA
Warranty Extension program. ODI found that prior to a failure of the sliding door(s) there is normally a loud buzzing noise/sound generated that alerts the operator to the actuator starting to wear out.ODI has not identified any crashes or injury allegations due to the sliding door being inoperable on the subject vehicles following nearly six years of exposure. ODI will continue to monitor complaints received for the incidence of sliding doors becoming inoperative.Accordingly, the investigation is closed. The Office of Defects Investigation (ODI) opened this Preliminary Evaluation (PE21017) on July 9, 2021 for model year (MY) 2016-2019 Triumph Thruxton and Speed Triple model motorcycles based on allegations of loss of front braking performance.Vehicle Owner Questionnaires (VOQ) indicated riders were experiencing a soft front brake lever
that could result in an unexpected extended stopping distance. On July 28, 2021, ODI sent an Information Request letter to Triumph
to learn more about the subject components and alleged defect.On September 24, 2021, Triumph
submitted its response, which ODI analyzed.ODI learned that the majority of the VOQ incidents occurred two or more years after the motorcycle was manufactured. The manufacturer recommends changing the brake fluid
every two years, regardless of mileage. As the manufacturer's warranty lasts for two years from the date of sale, many dealers covered the complicated brake fluid
replacement procedure under warranty. However, most of the motorcycles involved in these incidents were overdue to have their brake system serviced. Triumph
conducted testing of the brake system to better quantify what riders were experiencing.Triumph
found pulling the lever a second time immediately after an initial pull did result in less lever travel for the same force exerted on the lever.This reduction was two millimeters or less.Additional testing was conducted to ascertain the caliper force generated relative to various lever forces.Triumph
found that the caliper force varied +/- 2% between the first and second pull of the front brake lever
and any given applied force. Triumph
claimed that the lack of proper maintenance was the cause of the soft brake lever
.Triumph
conducted additional testing to quantify this claim.The lever travel between the first and second pull increased 150% on an improperly maintained system versus a properly maintained system.However, the caliper force only decreased 2-3% on average with this increased lever travel, further indicating that the subject motorcycles are capable of fully stopping. ODI located and evaluated a subject motorcycle.Measurements were taken and confirmed there was a small decrease in lever travel between the first and second pull.ODI then conducted a series of hard braking events, which did not produce evidence that the difference in lever travel resulted in an extended stopping distance. ODI also considered Fatal Accident Reporting System (FARS) data.ODI identified six fatal events that involved a subject vehicle.The service history records were reviewed for each motorcycle, however,none of the records included any mention of braking problems and thus there is no indication that the alleged defect may have contributed to the fatal crashes. As of this time there are no VOQ or manufacturer claims reporting an injury, fatality, or crash arising from this issue.Based on the available information, including the absence of such events after nine years of service on the oldest motorcycles, ODI is closing this investigation.The closing of this investigation does not constitute a finding by NHTSA that no safety-related defect exists. The agency reserves the right to take further action if warranted by the circumstances.
